NLRP3 Activation and Impaired TGF-β Anti-Inflammatory Pathways Predict Vascular Risk in PWH on ART.

IMPORTANCE: The immune mechanisms driving vascular disease remain incompletely understood. People with HIV (PWH), even during effective antiretroviral therapy (ART), exhibit persistent immune activation and inflammation, which may contribute to higher rates of vascular disease and mortality compared with people without HIV (PWoH). Leveraging a cohort of U.S. military personnel followed from HIV diagnosis through long-term ART suppression, we sought to identify immunologic pathways underlying increased vascular risk. OBJECTIVE: To identify plasma biomarkers reflecting distinct immune mechanism

Estrogen exposure from modern contraceptives and vascular risk in women with migraine: A nationwide electronic medical record database study.

BackgroundMany guidelines list migraine with aura (MwA) as a contraindication to estrogen-containing combined hormonal contraceptives (CHCs) due to vascular risks. However, current evidence is based on small sample studies with potential influence by confounding factors. Additionally, few studies have examined the vascular risk associated with modern CHCs with lower dose estrogen, particularly in relation to aura status. The present study aims to investigate the vascular risk of modern CHCs in women with migraine with and without aura.MethodsWe used a de-identified electronic medical record da

Risk factors associated with the development of complications after a hip fracture.

AIMS: Mortality after a hip fracture has declined in recent years, but the risk of complications remains high. The aim of this study was to identify non-modifiable and, specifically, modifiable factors associated with the development of complications after hip fracture. METHODS: This was a multicentre, prospective cohort study of adults aged ≥ 60 years with a hip fracture who were treated in 77 hospitals in England, Wales, and Northern Ireland between July 2014 and November 2021. A total of 24,523 patients were enrolled into the study. Cox proportional hazards regression models were used to a

Associations of body fat and inflammation with non-communicable chronic diseases and mortality: a prospective cohort study of the UK Biobank.

OBJECTIVE: Certain leading medical organisations are considering alternatives to the Body Mass Index (BMI) as a predictor of the risk for non-communicable chronic disease (NCD) or death. Our objective was to evaluate the associations between various measures of body fat and the risk of incident NCDs or mortality, independent of inflammation. DESIGN: Population-based prospective cohort study (the UK Biobank cohort). SETTING: The UK. PARTICIPANTS: Adults (aged between 40 and 69 years) were accrued between March 2006 and October 2010 and followed until December 2022. There were 500 107 partici

Association Between Myocardial Infarction and In-Hospital Complications in Cervical Spinal Cord Injury: A Retrospective Cohort Study.

Cardiovascular disease (CVD) is a leading cause of morbidity and mortality in individuals with spinal cord injury (SCI), with a particularly high burden in patients with upper cervical spine injuries. Outcomes of SCI in patients with chronic CVD have been the topic of extensive research, but the incidence and impact of myocardial infarction (MI) in the acute post-injury phase are not well established. In this study, we examined the incidence of MI in cervical SCI and its impact on in-hospital complications, mortality, and resource utilization. We conducted a retrospective cohort study using da

Combined strategy of transcatheter edge-to-edge repair of mitral valve and percutaneous mechanical circulatory support device to treat high-risk patients with severe mitral regurgitation and cardiogenic shock.

BACKGROUND: Acute mitral regurgitation (MR) associated with cardiogenic shock (CS) is a medical emergency with excessive in-hospital mortality. The vast majority of these patients are turned down for corrective surgery. The combined strategy of transcatheter edge-to-edge repair of mitral valves and percutaneous mechanical circulatory support devices to treat high-risk patients with severe mitral regurgitation and cardiogenic shock has not been evaluated in randomized controlled trials. AIMS: In this multicenter registry we aimed to assess the outcomes (all-cause mortality, other important sec

Outcomes of operative and nonoperative management of blunt thoracic aortic injury.

OBJECTIVE: Although the current guidelines for the management of blunt traumatic aortic injury (BTAI) have recommended intervention for grade 2 injuries or higher, a national trend has occurred for aggressive endovascular treatment of low-grade BTAIs. Little is known about the natural history of grade 1 and 2 injuries treated nonoperatively. We hypothesized that most of these low-grade injuries would remain stable with nonoperative management. METHODS: We performed a review of BTAIs at a large referral level 1 trauma center from 2004 to 2020. The injuries were graded using a standard 1 to 4 s

Carotid endarterectomy in SAPPHIRE-eligible high-risk patients: implications for selecting patients for carotid angioplasty and stenting.

OBJECTIVES: Carotid angioplasty and stenting (CAS) has been proposed as an alternative to carotid endarterectomy (CEA) in patients excluded from the North American Symptomatic Carotid Endarterectomy Trial and the Asymptomatic Carotid Atherosclerosis Study and in those considered at high risk for CEA. In light of recently released CAS data in patients at high risk, we reviewed our experience with CEA. METHODS: The records for consecutive patients who underwent CEA between 1998 and 2002 were retrospectively reviewed, and risk was stratified according to inclusion and exclusion criteria from a "

Electrophysiological laboratory, electrophysiologist-implanted, nonthoracotomy-implantable cardioverter/defibrillators.

BACKGROUND: Implantable cardioverter/defibrillators (ICDs) have conventionally been implanted in the operating room by surgeons. However, technological developments have reduced size and increased simplicity, bringing the procedure into the realm of the electrophysiologist. The purpose of this study was to evaluate the safety and efficacy of implantation of the entire ICD system by electrophysiologists in an electrophysiology laboratory. METHODS AND RESULTS: Between July 1993 and February 1994, 23 patients (21 men; age, 64 +/- 11 years) underwent transvenous ICD implantation by electrophysiol

Double Mandibular Osteotomy for Access to High-Carotid Pathology.

BACKGROUND: Anecdotal experience demonstrates the existence of patients with superiorly located carotid stenosis, neoplasms, or aneurysms where the mandible obstructs effective surgical access using standard techniques. As carotid pathology extends anatomically beyond the limits of standard operative technique, additional exposure becomes paramount to safely and effectively address the lesion. Double mandibular osteotomy (DMO) is one of several techniques to obtain additional exposure to high-carotid pathology; however, there is no large series to address the outcomes of patients undergoing th
