Sleep abnormalities in schizophrenia may suggest impaired trans-thalamic cortico-cortical communication: towards a dynamic model of the illness.

Schizophrenia is associated with a wide range of symptoms. These include auditory hallucinations, delusions, and experiences that one is not in control of one's own thoughts and actions, but that they are inserted by an outside agency. It has been proposed that a disturbance in the sense of self may account for many of these symptoms. This disturbance in turn may be associated with source monitoring deficits. In other words, individuals with schizophrenia may misattribute the source of their own thoughts and actions to an outside agency, which then results in the experience of psychosis such a

Sleep-related screaming as a manifestation of frontal lobe epilepsy in a 8 year-old girl: case report with diagnostic polysomnographic and neuroradiological findings.

We report the case of a 8-year-old girl with a 3-month history of sleep-related screaming during both the night and daytime naps. A terrified expression accompanied the episodes of screaming, often with dream mentation and clouded consciousness. Her parents witnessed up to 5-6 episodes every night, with each episode lasting for less than 1 minute. A polysomnographic study documented six episodes of paroxysmal screaming without limb movement, with all episodes emerging from stage 2 non-REM sleep, and episode duration usually lasting 30-60 seconds. Suppression of the generalized background EEG a

Sleep disturbances in the rotenone animal model of Parkinson disease.

Parkinson disease (PD) is characterized by the degeneration of dopaminergic neurons in the substantia nigra pars compacta (SNc) and the presence of intracytoplasmatic inclusions known as Lewy bodies. Chronic administration of rotenone (RT) produces Parkinson's-like symptoms in rats. Because PD patients have disrupted sleep patterns, we determined if chronic RT administration produces similar changes in rat sleep. RT was administered for 28 days to rats. Basal and vehicle (VH) rats received saline or dimethyl sulfoxide and polyethylene glycol (1:1), respectively. VH infusion induced a progressi

Sleep-related non-epileptic motor phenomena.

We describe the clinical, neurophysiological and polysomnographic characteristics of some involuntary motor nocturnal events occurring during sleep or at the transition from wakefulness to sleep.

Non-rapid eye movement sleep parasomnias.

Parasomnias are unpleasant or undesirable behavioral or experiential phenomena that occur during sleep. Once believed unitary phenomena related to psychiatric disorders, it is now clear that parasomnias result from several different phenomena and usually are not related to psychiatric conditions. Parasomnias are categorized as primary (disorders of the sleep states) and secondary (disorders of other organ systems that manifest themselves during sleep). Primary sleep parasomnias can be classified according to the sleep state of origin: rapid eye movement sleep, non-rapid eye movement sleep, and

A factor replication of the Sleep-Wake Activity Inventory (SWAI) in a Mexican population.

The purpose of this study was to describe the factor structure of the Sleep-Wake Activity Inventory (SWAI) in a Mexican population. In a sample of 722 Mexican college students, we replicated five of the six factors originally described in the SWAI. Retained factors included: excessive daytime sleepiness (similarity coefficient of 0.735), psychic distress (0.609), social desirability (0.638), individual's ability to relax (0.864), and nocturnal sleep (0.660). These results confirm the factor structure and extend the possible utility of the SWAI in a siesta culture.

Driving vigilance simulator test.

Drivers suffering from sleepiness at the wheel run the risk of being involved in car accidents. To evaluate whether objective data of driving performance can be assessed in patients with excessive tendency of falling asleep at the wheel, two test versions of a computerized driving program were created to fit an advanced driving simulator. For the evaluation 15 male drivers with habitual sleep spells whilst driving were selected among patients with the clinical features of the sleep apnoea syndrome. The brake reaction time and the deviations from straight road-line were significantly increased

Peptic ulcer is prevalent among shift workers.

To elucidate sleep disturbances in the etiology of peptic ulcers, the prevalence of peptic ulcer disease was compared among shift workers and daytime workers. The subjects (N = 11,657) were employees of various institutions, such as factories, banks, or schools, and had undergone a mass x-ray examination of the upper gastrointestinal tract. With present shift workers (N = 2269), the prevalence of gastric ulcers was 2.38% (N = 54) and that of duodenal ulcers, 1.37% (N = 31). With the past shift workers (N = 2111), the prevalence of gastric ulcers was 1.52% (N = 32) and that of duodenal ulcers,

[Epileptic sleep seizures in childhood (author's transl)].

64 children with seizures occuring during sleep were studied. No claim is made to definitive clinical results, but the clinical peculiarities of certain epileptic crises that tend to occur more frequently in sleep are presented. In 62.5% of all our cases are being treated as benign epilepsy, with centro-temporal E.E.G. foci and typical characteristics of age, hour of appearance and therapeutic response. In 32.5% are generalized seizures, and these have the widest age range at onset. The fits are at times unrelated to the length of previous sleep and respond less favourably to treatment. In 5%

"The soft embalmer of the still midnight": a problem of prescribing hypnotics in general practice.

Was it euthanasia or was it even murder? That is the rather dramatic question with which the case history ends. However, none of those taking part in this conference had any doubts that it was neither murder nor euthanasia. But all had doubts as to whether the patient received the best care which the medical and allied professions can command. The barrister declared that in law there would have been no case to answer but the consensus of opinion of the other participants was that as no real diagnosis had been made the treatment was essentially superficial - ensuring sleep for the patient by pr

Drug self-administration and sleep-awake activity in rats dependent on morphine, methadone, or l-alpha-acetylmethadol.

Morphine post-dependent rats were prepared with chronic cortical EEG and temporalis muscle EMG electrodes and i.v. jugular cannulae and permitted to self-administer morphine (10 mg/kg/injection) on a fixed-ratio schedule of reinforcement to reestablish and maintain dependence. Morphine was then replaced by methadone or 1-alpha-acetyl-methadol (LAAM) at a dose of 2 and 1 mg/kg, respectively. The rats developed stable patterns of self-administration of each narcotic and maintained the dependent state. The mean (+/- S.E.) interinjection interval was 2.5 +/- 0.1 h for morphine, 1.4 +/- 0.1 h for m

The Pickwickian syndrome with hypertrophy of tonsils: a re-appraisal.

Pronounced tonsilar hypertrophy was found in two obese patients suffering from hypersomnolence, periodic attacks of apnea and disturbing snoring at night. Both patients underwent tonsillectomy. Immediately after the operation the hypersomnolence disappeared, the breathing became normal, and the disturbing snoring at night ceased. Follow-up over a period of three years did not reveal any recurrence of these symptoms, even though the patients had not lost any weight during this period.
