The use of oesophageal pH-monitoring for the evaluation of gastrooesophageal reflux after resection of the cardia and total gastrectomy.

We intended to estimate the applicability of oesophageal pHmetry for the evaluation and diagnosis of reflux oesophagitis after resection of the cardia and total gastrectomy. We applied the system of glass calomel electrode type GK 282 C (Radiometer-Kopenhagen). The pH probe was introduced transnasally in the remnant of the stomach or into the jejunum. Its position was controlled by fluoroscopy. Then the probe was drawn for 5 cm subsequently to the final position of 5 to 10 cm above the oesophagogastric or oesophagojejunal anastomosis. During the examination the patients were sitting or lying b

Long-term esophageal function following repair of esophageal atresia.

Primary repair of esophageal atresia restores gastrointestinal continuity, but does not ensure normal esophageal function. To date 22 patients, six to 32 (average 15) years after repair of their esophageal atresias, have been evaluated by personal interview and esophageal manometrics and acid reflux testing. Previous barium swallow examinations had demonstrated varying degrees of anastomotic narrowing (12 patients), abnormal esophageal motor function (11 patients), gastroesophageal reflux (two patients), and hiatal hernia (one patient). Ten patients experience intermittent dysphagia for solid

Pharmacology of the esophageal motor funciton.

In the striated muscle of the upper esophageal sphincter, tonic maintenance of closure is probably mediated via tonic central excitation of the extrinsic motor innervation; relaxation represents central inhibition of this mechanism. The motor nerves are probably cholinergic and act through nicotinic receptors like those of somatic striated muscle. In the striated muscle of the esophageal body, swallowing-induced contraction is also probably a cholinergic and nicotinic response. In the smooth muscle of the esophageal body, the control of contractions is cholinergic and muscarinic in part, but t

Aortic Aneurysm Presenting As Deglutition Syncope.

Deglutitive syncope is defined as a neurally mediated syncope in which loss of consciousness occurs during or immediately after swallowing. The causes of deglutitive syncope vary widely and range from intraluminal causes, as well as extra-esophageal compression. In this case report, we present a rare case of deglutitive syncope caused by a thoracic aortic aneurysm compressing the proximal esophagus, a clinical entity described in the literature as dysphagia aortica.

Capsule endoscopy in gastrointestinal disease: Evaluation, diagnosis, and treatment.

Capsule endoscopy, also known as wireless capsule endoscopy or video capsule endoscopy, is a noninvasive procedure that uses a swallowed capsule-shaped miniature camera for direct visual and diagnostic evaluation of gastrointestinal (GI) disease. Although originally intended as a tool to examine the small intestine, which is mostly beyond the reach of conventional endoscopy, capsule endoscopy is now also being used to examine the entire length of the GI tract.

[Isolated neurofibroma of the tongue].

A clinical case of successful treatment of isolated tongue neurofibroma presented patient K., 64 years old, referred to the clinic complaining of a feeling of «discomfort» on swallowing. With a comprehensive examination, a diagnosis of an isolated tongue neurofibroma was established. Surgical treatment was performed. The diagnosis was confirmed morphologically. The presented clinical case of isolated tongue neurofibroma in an elderly patient is casuistic in nature, of scientific and practical interest due to the rarity and unusualness of the disease.

Laryngeal webs and laryngotracheoesophageal clefts: State of the art.

Laryngeal webs and laryngotracheoesophageal clefts (LTECs) are rare malformations of the larynx. Both can have a wide range of symptoms. While webs typically affect voice and airway and LTECs typically affect swallowing and airway, this is not always the case. A high index of suspicion is required for timely and accurate diagnosis. A discussion of management and treatment options will be presented. Both endoscopic and open surgical techniques exist for both pathologies and will be reviewed.

Vagus nerve neuropathy related to SARS COV-2 infection.

A 38-year-old woman with confirmed SARS-CoV-2 infection developed dysphagia to both solids and liquids. A fiberoptic nasolaryngoscopy and a videofluoroscopy swallowing study showed right vocal cord paresis, tenth cranial nerve neuropathy, as well as oral, hypopharynx, and supraglottic hypoesthesia. Orotracheal intubation was not required. The patient was fully recovered after undergoing a multidisciplinary rehabilitation program that included speech and deglutition therapy.

Imaging Features of isolated hypoglossal nerve palsy.

The hypoglossal nerve gives motor innervation to the intrinsic and extrinsic muscles of the tongue. Pathology of this nerve affects the balanced action of the genioglossus muscle causing tongue deviation toward the weak side. Clinically, hypoglossal nerve palsy manifests with difficulty chewing, swallowing and with dysarthric speech herein, we review the anatomy of the hypoglossal nerve as well as common and infrequent lesions that can affect this nerve along its course.

Anatomy and physiology of velopharyngeal closure and insufficiency.

The velopharynx is a complex structure that is responsible for separation of the oral and nasal cavities during speech production and swallowing. Incompetence of this mechanism can lead to hypernasality, with nasal air emission and incomprehensible speech, as well as nasopharyngeal regurgitation. There can be a significant social stigma associated with velopharyngeal dysfunction, and surgical treatment can be curative in many cases. Knowledge of the normal anatomy and physiology of the velopharyngeal complex is essential when planning for surgical repair.

Labial ankyloglossia associated with oligodontia: a case report.

Developmental disturbances of the tongue (aglossia, macroglossia, microglossia, and ankyloglossia) can adversely affect the development of the surrounding structures including the palate, alveolar process, and teeth. These developmental disturbances impair functions such as mastication, speech, and swallowing. Ankyloglossia is of various types and may be associated with other syndromic features like cleft palate. We report a rare type of non-syndromic ankyloglossia associated with missing of the permanent anterior tooth.

Central hypoventilation and brainstem dysgenesis.

We report on a newborn with congenital hypotonia, unilateral facial palsy, sucking and swallowing difficulties, velopalatine incoordination, and unilateral impairment of the auditory brainstem responses, attributable to brainstem dysgenesis. On follow-up, the child manifested developmental delay and central hypoventilation syndrome during sleep. The ventilation abnormality during sleep with insensitivity to hypercapnia, associated with unilateral facial paralysis, indicates a pontine lesion, including the parafacial respiratory group.

Rapunzel syndrome: a case report and review of literature.

Bezoars consist of ingested foreign material or organic matter which forms a mass in the gastrointestinal tract, usually in stomach. Trichobezoars formed by swallowed hair present with malnutrition, weight loss, abdominal pain with signs of gastrointestinal obstruction. Gastric trichobezoar with a tail reaching the small intestine is called Rapunzel syndrome. Although endoscopic methods are available to remove bezoars, majority of trichobezoars with Rapunzel syndrome undergo surgical removal.

Controlling sialorrhoea: a review of available treatment options.

Sialorrhoea (drooling or excessive salivation) is a common symptom of many neurological diseases (e.g., amyotropic lateral sclerosis, cerebral palsy and Parkinson's disease) and otolaryngologic diseases (tumours of upper aerodigestive tract). It is mostly caused by poor oral and facial muscle control in patients with swallowing dysfunction (secondary sialorrhoea). However, some cases of sialorrhoea are due to hypersecretion of saliva. These cases represent so-called primary sialorrhoea.

Neurenteric cyst of the posterior mediastinum.

We report a case of a large posterior mediastinal neurenteric cyst in a neonate demonstrated by chest radiographs, barium swallow examination, ultrasonography and CT of the thorax. All the investigations revealed a large posterior mediastinal cystic mass with vertebral anomalies in the form of scoliosis and hemivertebra. The cyst was completely excised by a right posterolateral thoracotomy and biopsy showed the features of a neurenteric cyst. The rarity of the lesion prompted us to report this case.

Retropharyngeal abscess: epiglottitis of the new millennium.

From 1993 through 1999, 26 children with retropharyngeal abscess and 2 children with acute epiglottitis were cared for by pediatric otolaryngologists in northern Virginia. Fever, sore throat, dysphagia, refusal to swallow, dysphonia, drooling, and neck extension are common presenting signs and symptoms in acute epiglottitis and in retropharyngeal abscess. Contrast-enhanced computed tomography of the oropharynx was performed in all cases and was the most helpful diagnostic test.

Brown tumor of the hard palate in secondary hyperparathyroidism.

Jaw bones are rarely involved in secondary hyperparathyroidism. We report a case of 13 year old girl who presented with progressive chronic renal failure and secondary hyperparathyroidism. Five months after beginning of hemodialysis, a large brown tumor developed on the hard palate, extending to the maxilla causing difficulty in swallowing and breathing. She died of massive intestinal hemorrhage five months after excision of the mass.

Childhood achalasia--a case report.

The neuromotor disorders of the oesophagus are rare in childhood. The spectrum includes achalasia, vigorous achalasia and diffuse oesophageal spasm. The classical presentation in achalasia is vomiting, failure to thrive and recurrent chest infection. Diagnosis is confirmed on plain film of the chest and a barium swallow which shows the "bird beak" sign. Surgery is the preferred mode of management in children. Adjunctive procedures to surgery like Nissen fundoplication should be selectively performed.

Breaking bad news in amyotrophic lateral sclerosis.

There are many difficulties of communication with patients with amyotrophic lateral sclerosis (motor neurone disease). In France, most physicians have a policy of keeping the fact of this diagnosis from both patients and relatives. This paper describes an alternative approach based on open communication, and applies that principle to handling issues of communication associated with various aspects of four different topics: the diagnosis itself, the development of handicap, swallowing difficulties, and respiratory difficulties.

[The primary reconstruction of soft palate by intraoral buccal transposition flap].

A method for the primary reconstruction of soft palate after radical tumor excision is described. By combination of a cranial pedicled pharynx flaps and a cranial pedicled intraoral buccal transposition flap is it possible to close the resulting defect in a double layer. In additional resection of the combination of the method with a M. masseter-transposition flap is possible. By the method it is possible to have a satisfactory reconstruction of swallowing and speech functions with only small operative troubles.
