Postural hypotension: its meaning and management in the elderly.

Aging is associated with a considerable number of alterations in function of the autonomic nervous system and with systems involved in the control of cardiovascular response to postural changes. However, these alterations themselves do not generally lead to symptomatic orthostatic hypotension. In combination with other factors, older patients can develop marked problems with orthostatic hypotension--notably, certain drug regimens, some degree of underlying heart failure, or such common geriatric illnesses as parkinsonism and diabetes. Treatment regimens must be designed to minimize side effect…

Comparison of neurothermography and contrast myelography.

Eighty consecutive patients who first had an electronic infrared thermogram (neurothermogram) and a subsequent oil myelogram were studied retrospectively. All patients had cervical and/or lumbosacral radiculopathy for 90 days. Negative neurothermograms were predictive of negative myelograms in 93% of our series. Positive neurothermograms were predictive of positive myelograms in 71% of the patients. The neurothermogram is not a specific test for intraspinal axis lesions as a cause of radiculopathy. Proximal neuropathic lesions of other causes will also produce abnormalities of the sympathetic

[Sympathalgia of the cervicobrachial region (author's transl)].

Pain syndromes in the cervicobrachial region may be an expression of irritation of the periarterial autonomic nervous system. They show a vasal, arterial topography (here of the subclavian artery). If the cervical sympathetic chain is involved in the irritation, the area supplied by the carotid artery, i.e. the homolateral half of the head is also affected. Characteristics of these disturbances are their abnormal topography, which cannot be classified either as a radicular nor a segmental pattern. In this region the perception of pain is delayed. The quality of pain is protopathic (dull, inten…

Potassium metabolism.

In man, mechanisms for potassium excretion are complex and highly developed, while potassium conservation is potentially inadequate. Potassium balance is regulated by alterations in excretion in the distal renal tubule, where mineralocorticoid hormones and Na-K ATPase are the major regulating factors. The distribution of potassium across cell membranes is influenced by changes in acid-base status, by pancreatic hormones and by the autonomic nervous system. Potassium stimulates insulin and aldosterone secretion and increases Na-K ATPase in the distal nephron, so promoting its own redistribution…

Anesthetic Management of Patients With Morvan Syndrome: A Case Report and Literature Review.

Morvan syndrome is a rare condition distinguished by hyperactivity within the central, autonomic, and peripheral nervous systems. Due to the limited number of cases, this presents clinical challenges stemming from the scarcity of published literature. We present a successful anesthetic approach for a patient diagnosed with Morvan syndrome scheduled for elective major intra-thoracic surgery to remove metastases from a thymoma. The patient had previously undergone thymectomy, with the syndrome being diagnosed only one year after the surgery. Additionally, we conducted a literature review on the

Two Uncommon Paraneoplastic Neurological Syndromes in a Child With Hodgkin Lymphoma.

Paraneoplastic neurological syndromes (PNS) are rare, remote effects of cancer that are usually caused by an altered immune response to the tumor and not due to the tumor mass, metastasis, infection, ischemia, or metabolic derangements. PNSs can affect any area of the central, the peripheral, and the autonomic nervous systems. These are rare in lymphomas compared with solid tumors attributed to their presentation even in late stages and the absence of onconeural antibodies. We present a child with stage IIB Hodgkin lymphoma who presented with dual PNS, achalasia cardia, and Holmes Adie pupil o…

[Micturition disorder as a sequela of sacral autonomic diabetic neuropathy].

Diabetic neuropathy may manifest itself also in the autonomous nervous system and thus lead to a functional disorder of the urinary bladder. Urodynamic studies carried out in 11 diabetic patients with unclear bladder dysfunction allowed 2 forms of bladder anomalies to be identified: 1. detrusor hyperreflexia, 2. hyporeflexia and areflexia of the detrusor. Whereas hyperreflexia is clinically characterised by signs of motor urgency, hyporeflexia is mainly identifiable by residual urine formation and recurrent infections of the urinary tract. Therapy depends on the type of dysfunction and consist

Long-term social isolation during adolescence exacerbated cardiac dysfunction after myocardial infarction.

Prolonged social isolation (SI) and negative emotion are associated with an increased risk of cardiovascular diseases (CVDs). However, it re … mechanisms involved in this process may include negative mood, dysregulation of the reward system, autonomic nervous system modulation, and HPA axis activation. These findings will contribute to our better understanding of heart-brain connections.

Autonomic cardiovascular regulation in patients with obstructive sleep apnoea: a study based on spectral analysis of heart rate variability.

The aim of this study was to assess the function of the autonomic nervous system in patients with obstructive sleep apnoea syndrome (OSAS). The study was designed as a cross-sectional case-control study. Fifty-one patients were included, and the findings were compared with those in 66 controls. Spectral analysis of heart rate variability (HRV) during supine rest, during controlled breathing and after tilting was performed in each patient and control case. The patients performed overnight sleep recordings the night before the HRV recordings. Individuals with an apnoea-hypopnoea index (AHI) abov…

Heart rate variability as a prognostic tool in cardiology. A contribution to the problem from a theoretical point of view.

BACKGROUND: Recent clinical studies have proposed standard deviation of heart rate as a diagnostic tool for the outcome of cardiac infarction. Mathematical analysis of heart rate variability shows that heart rate is influenced by different frequency components derived from different parts of the autonomous nervous system. In the experimental part of this study, we investigated the possibility of calculating a variable describing the parasympathetic branch of the autonomous nervous system exclusively. METHODS AND RESULTS: In 60 healthy volunteers, heart rate was measured to 1 millisecond durin

Cardiac autonomic function in bronchiectasis and age and gender-matched healthy participants: case-control study.

This study aims to evaluate and compare cardiac autonomic function, specifically through heart rate variability (HRV), between individuals with bronchiectasis and their age- and gender-matched healthy counterparts. This study employed a case-control design, involving 60 participants diagnosed with bronchiectasis (cases) and a control group of healthy individuals matched by age and gender. HRV data was collected over a five-minute interval, focusing on frequency domain parameters including total power, very low frequency (VLF), low frequency (LF), high frequency (HF), and the LF/HF ratio. Computational frameworks for automated detection and quantification of paroxysmal sympathetic hyperactivity among traumatic brain injury patients.

Paroxysmal sympathetic hyperactivity (PSH) is a syndrome that occurs in a large subset of critically ill traumatic brain injury (TBI) patients and is associated with complications and poor recovery. PSH is defined by recurrent episodic vital sign elevations in the appropriate clinical context. However, standard diagnostic criteria rely heavily on subjective judgment, leading to challenges and delays in recognition, monitoring, and management. The objective of this study was to develop automated PSH detection and quantification tools that exclusively utilize objective bedside continuous vital s

CoolCuddle and Autonomic Regulation in Infants With Hypoxic-Ischaemic Encephalopathy: An Intervention Study.

AIM: We evaluated the impact of 'CoolCuddle', parental cuddling during therapeutic hypothermia intervention for hypoxic-ischaemic encephalopathy, on heart rate variability (HRV). METHODS: In this prospective, single-group, interventional study with repeated measures, we included infants ≥ 36 weeks' gestation undergoing hypothermia and CoolCuddle with HRV data. Time-domain HRV was assessed before, during, and after each cuddle. Primary outcomes were heart rate, mean RR interval (ANN), standard deviation of normal RR intervals, and root mean square of successive differences (RMSSD). RESULTS: I

Vagal modulation in pediatric disorders of gut-brain interaction: the role of 24 h heart rate variability.

BACKGROUND: Disorders of gut-brain interaction (DGBI) are common in pediatrics. Though the name clearly implies a neural contribution, the role of the autonomic nervous system remains unclear. Heart rate in healthy subjects (HC) follows a circadian pattern with dipping during the night with increased high frequency (hf) heart rate variability (HRV) and root mean square of successive differences (RMSSD). Our hypothesis was that reduced vagal modulation in adolescents with DGBI is associated with blunted rise in nocturnal vagal modulation. METHODS: An institutional review board (IRB) approved t…

Laser therapy: palliative care for the Harlequin syndrome?

Harlequin syndrome is a rare condition characterized by facial dysautonomia with anhidrosis, hemifacial blush, and other nonfacial manifestations without conventional treatment. This report presents a case study of the potential treatment of Harlequin syndrome using laser therapy. A single-case AB design was used, allowing for a systematic comparison between the baseline and intervention phases. The baseline phase involved data collection during a period without intervention (control condition), that is before laser therap. As evaluation criteria, a clinical assessment of skin turgor, sweating
