Long-term complications of endometrial ablation: cause, diagnosis, treatment, and prevention.

At first, total endometrial ablation seemed extremely safe in the short term. However, as time passed, certain unique long-term complications became evident. The problem is that after this procedure, intrauterine scarring and contracture can occur. Any bleeding from persistent or regenerating endometrium behind the scar may be obstructed and cause problems such as central hematometra, cornual hematometra, postablation tubal sterilization syndrome, retrograde menstruation, and potential delay in the diagnosis of endometrial cancer. The incidence of these complications is probably understated be…

Dupuytren's disease following acute injury in Japanese patients: Dupuytren's disease or not?

This paper reports the development of Dupuytren's disease following acute injury in 16 hands in 14 Japanese patients. The patients included six women and eight men. Five patients developed disease following trauma, one following infection and eight following elective surgery. In the present series, the patient age and sex are irrelevant. The disease was unilateral, confined to a single digital ray, and without ectopic lesions in most cases. Disease presented predominantly in the ring or middle finger rays. There were only three patients who underwent surgery for definite flexion contracture. D…

A modified dynamic mouth splint for burn patients.

The prevention and management of microstomia contracture is very important for the quality of life in a patient who has experienced facial burns. Mouth opening is important for speech, eating, dental hygiene, expression, social interaction, psychosocial well being, and administrating general anesthesia. Principles of scar management state a need for opposing horizontal, vertical, and circumferential forces for effective management of microstomia. At the Royal Brisbane and Women's Hospital, horizontal and circumferential stretch was provided well; however, a suitable vertical stretch component …

Anesthetic management of a patient with progressive supranuclear palsy.

A 74-year-old woman with progressive supranuclear palsy (PSP) was scheduled for laryngotracheal separation surgery. Her neck showed severe b … uscle relaxant. Monitoring of the train-of--four ratio in the ulnar nerve was impossible because of contracture of the fingers. Patients with PSP may have some serious associated deformities, and specific management, especially for the airway, may be necessary for general anesthesia.

Surgery for Dupuytren's disease in Japanese patients and a new preoperative classification.

The surgical outcome of Dupuytren's disease was evaluated in 73 hands of 57 patients in a Japanese population. Subtotal fasciectomy was performed in all cases. Surgical results were evaluated using the percentage improvement of extension in each finger joint. Statistical analyses were performed on the risk factors associated with recurrence and extension. The surgical outcome depended on the degree of contracture of the proximal interphalangeal joint. Recurrence of disease occurred in eight patients (14%) and extension occurred in nine (16%). Recurrence and extension frequently occurred in tho…

[Juvenile hyaline fibromatosis: 2 twin brothers affected].

INTRODUCTION: Juvenile hyaline fibromatosis is a rare hereditary genetic disease characterized by a cutaneous tumoral fibroblastic proliferation with joint involvement. CASE-REPORT: We report the case of 2 homozygous twin brothers both presenting a diffuse and invalidating form of juvenile hyaline fibromatosis. The 2 boys' born of a twin pregnancy' presented at the age of 2 months a clinical picture made up by the classic association: infiltrated papulo-nodules, gingival hyperplasia and joint contracture. The diagnosis of juvenile hyaline fibromatosis suggested clinically was confirmed by the…

[Obstetric brachial plexus palsy--clinics, pathophysiology and surgical treatment].

Obstetric brachial plexus palsy is a rare but sometimes severe traction injury. Peripheral nerve microsurgery (neurolysis, interfascicular grafting) and secondary procedures including muscle and tendon transfers altogether with contracture releases have improved the prognosis over the last 20 years. This article includes a historical review, the ongoing discussion about the pathophysiology (frequent traction injury vs. rare intrauterine maladaptation) and the clinics. Based on a patient group of 500 children with 100 microsurgical plexus reconstructions, we describe the surgical technique and …

Prevention of the parrot beak deformity in fingertip injuries.

The nail is supported by the dorsal tuft of the terminal phalanx. Following distal fingertip trauma, varying amounts of nail support may be lost resulting in the nail curving palmarwards. This curvature is dependent upon the degree of bony loss, the amount of remaining nail bed and the degree of scar contracture at the hyponychial-pulp interface. The parrot beak or hooked nail deformity is most commonly caused by tight closure of a fingertip amputation and excessive palmar tension at the hyponychial-pulp suture line. A simple technique using a hypodermic needle, that eliminates tension from th…

In a 4 year period (1996-1999), 42 total wrist fusions in 25 men and 17 women were performed using the AO/ASIF Titanium wrist fusion plate. The median age of the patients at the time of surgery was 41 (range, 19-72) years. The indication for fusion was post-traumatic arthritis in 29 wrists, Kienböck's disease in eight, rheumatoid arthritis in three, mono-arthritis in one and Volkmann's contracture in one. All patients were reviewed at a median follow-up of 23 (range, 6-50) months. The Buck-Gramcko and Lohmann score for functional evaluation was excellent in 35, good in 5 and satisfactory in 2 …

A biomechanical comparison of the Z step-cut and basilar crescentic osteotomies of the first metatarsal.

The basilar crescentic osteotomy is a popular procedure for the correction of moderate to severe hallux valgus deformity. Dorsiflexion due to malposition of the osteotomy, fixation failure, or late contracture of the soft tissues can lead to lateral metatarsalgia. This study compares the mechanical properties of the Z step cut osteotomy Scarff to those of the crescentic osteotomy in cantilever-bending. The ultimate load, maximum bending moment and energy absorbed were significantly greater for the Scarff osteotomy compared to those in the crescentic osteotomy (p < 0.001). Values for stiffness …

The aesthetic unit dorsal nasal flap: rationale for avoiding a glabellar incision.

The dorsal nasal flap was first introduced by Rieger in 1967. Since that time, it has proven reliable in the coverage of dorsal nasal soft-tissue defects; however, the glabellar component of the flap can leave a conspicuous scar and/or a contracture band. The authors present their experience with 48 patients who had an aesthetic unit dorsal nasal flap and their technique of incisional interface resurfacing, which obviates the need for the glabellar component. Objective independent assessment of the outcomes revealed overall excellent results, with no flap loss, hematoma, or dehiscence. Two cas…

An unusual toxic reaction to axillary block by mepivacaine with adrenaline.

An increase in blood pressure, accompanied by atrial fibrillation, agitation, incomprehensible shouts and loss of consciousness, was observed in an elderly, ASA classification group II, cardiovascularly medicated male, 12 min after performance of axillary block with mepivacaine 850 mg containing adrenaline 0.225 mg, for correction of Dupuytren's contracture. After intravenous administration of labetalol, metoprolol and midazolam the patient's condition improved, and 15 min later he woke up. The block was successful and surgery was conducted as scheduled despite persisting atrial fibrillation. …

The Israeli Diagnostic Center for Malignant Hyperthermia: 7-years' accumulated experience.

Malignant hyperthermia (MH), a rare pharmacogenetic trait, can be lethal when susceptible individuals are exposed to triggering agents during general anesthesia. We present our experience with the caffeine-halothane in vitro contracture test (IVCT) for the diagnosis of malignant hyperthermia susceptibility. Out of 75 patients that were referred for consultation to the MH diagnostic center over a period of 7 years, we performed muscle biopsies and IVCT in 21 patients. A total of 6 patients were found to be MH-positive. Appropriate recommendations for future anesthetic management, and additional…
