Reaching for more: How reproduction transforms the maternal intestine.

Several organs undergo changes during pregnancy to accommodate fetal growth, including the small intestine. However, the signals that trigger this intestinal remodeling are not well understood. In a recent study, Ameku et al. describe an anticipatory intestinal growth program that is partially irreversible and supported by SGLT3a-dependent increase of Fgfbp1-positive progenitor cells.

The Role of Intestinal Fungi in the Pathogenesis and Treatment of Ulcerative Colitis.

Ulcerative colitis (UC) is a chronic inflammatory bowel disease closely associated with dysbiosis of the gut microbiome, encompassing not only bacterial communities but also fungal populations. Despite the growing recognition of the gut microbiome's role in UC pathogenesis, the contribution of intestinal fungi has only recently garnered significant attention. In this review, we comprehensively examine the characteristics of intestinal fungi in both healthy individuals and UC patients, elucidating their role in disease pathogenesis and their interactions with bacterial communities. Additionally

Pathogenesis and Management of Intestinal Failure-Associated Liver Disease.

Long-term parenteral nutrition (PN) has considerably improved the management of intestinal failure (IF) in children and adults, particularly those with short bowel syndrome; however, it carries a significant risk of hepatotoxicity, specifically, intestinal failure-associated liver disease (IFALD), also known as PN-associated liver disease. This review provides an update on the latest understanding of IFALD pathogenesis, emerging therapies, and ongoing challenges in the management of this complication. A number of factors are associated with the development of IFALD. PN lipid emulsions, phytost

Lactulose as a prebiotic improved the intestinal health and metabolism of geese.

Lactulose is highly valued for its unique role in promoting the growth of intestinal probiotics. The objective of this research was to investigate how varying levels of dietary protein combined with lactulose supplementation influence the intestinal health of geese, utilizing intestinal metabolomics as the analytical basis. A total of 210 one-day-old Yangzhou geese were randomly assigned to three dietary treatments, with each group consisting of 7 replicates of 10 birds each. The diet of the control group (CP) contained 18.18% crude protein, the high-protein group (HP) was provided a diet form

The intestinal barrier: a pivotal role in health, inflammation, and cancer.

The intestinal barrier serves as a boundary between the mucosal immune system in the lamina propria and the external environment of the intestinal lumen, which contains a diverse array of microorganisms and ingested environmental factors, including pathogens, food antigens, toxins, and other foreign substances. This barrier has a central role in regulating the controlled interaction between luminal factors and the intestinal immune system. Disruptions of intestinal epithelial cells, which serve as a physical barrier, or the antimicrobial peptides and mucins they produce, which act as a chemica

Lactiplantibacillus plantarum strengthens the intestinal barrier: involvement of the endocannabinoidome.

Probiotics have been suggested to ameliorate intestinal epithelial homeostasis and barrier function. They also modulate several mediators and receptors of the expanded endocannabinoid system, or endocannabinoidome (eCBome), potentially explaining their beneficial effects on intestinal function. We aimed to study the effects of probiotic strains on gut barrier functions and the possible involvement of the eCBome in these effects. We cocultured three strains of Lactiplantibacillus plantarum with murine small intestine epithelial organoids and explored the involvement of eCBome signaling and infl

Mapping mesenchymal diversity in the developing human intestine and organoids.

The organization of diverse mesenchymal populations during human intestinal development is critical for tissue architecture and function yet remains poorly defined. To construct a comprehensive, tissue-scale map of the developing human small intestine, we leveraged single-cell RNA-sequencing data to build a custom Xenium spatial transcriptomics gene panel covering the diversity of cell types in the human intestine. Analysis was focused on the developing mesenchyme populations (also referred to as fibroblasts or stroma) given the lack of spatiotemporal information about these cell populations.

Effects of midazolam and dexmedetomidine on liver and intestinal mitochondrial respiration.

BACKGROUND: Mitochondrial dysfunction frequently occurs in critically ill patients and is thought to contribute to metabolic shutdown and hepatic or gastrointestinal failure. In addition to hypoxemia and inflammation, sedatives are considered potential contributors to further impairment of mitochondrial function. OBJECTIVE: This study investigated the effect of the sedatives midazolam and dexmedetomidine on liver and colon mitochondrial function. Especially colon mitochondrial affection by both drugs was evaluated here for the first time. DESIGN: Liver and colon tissue homogenates from healt

A Case of Primary Gastric Carcinosarcoma with Small Intestine Metastasis.

INTRODUCTION: Primary gastric carcinosarcoma is extremely rare. Herein, we report a case of primary gastric carcinosarcoma with small intestinal intraluminal metastasis. CASE PRESENTATION: The patient was a 77-year-old man who was referred to our hospital for further examination of occult fecal blood. At this time, the patient reported lightheadedness, and severe anemia was confirmed, with a hemoglobin level of 6.5 g/dL. Upper gastrointestinal endoscopy revealed a mass with bleeding spots in the middle 3rd of the stomach, which was thought to be the cause of the anemia. The patient's overall

A rare case of adult intestinal malrotation: A case report.

INTRODUCTION AND IMPORTANCE: Intestinal malrotation is a congenital disorder resulting from the failure of the normal embryologic fetal sequence of bowel rotation and fixation. Adult midgut malrotation is extremely uncommon, with incidence estimates ranging from 0.0001 % to 0.19 %. In adults, there is a slight female predominance. Mid gut volvulus leads to obstruction and warrants surgical intervention. This case is significant because there are few case reports around the world. CASE PRESENTATION: An 18-year-old Ethiopian man presented with cramping abdominal pain that worsened over a one-mo

Prostatic schwannoma presenting with acute urinary and intestinal obstruction.

Our case involves a 56-year-old man experiencing acute urinary retention and signs of acute intestinal obstruction. Imaging revealed a distinct large multiloculated cystic lesion (15.0 × 7.5 × 14.0 cm) originating from the right lobe of the prostate. Emergency laparotomy successfully achieved complete resection of the cystic lesion. Histopathology confirmed ancient schwannoma, displaying degenerative changes. The case indicated acute intracystic hemorrhage as a probable cause of sudden symptoms. This highlights the potential of prostatic schwannomas for cystic presentation and the capacity to

Protective mechanism of Bifidobacterium on intestinal mucosal barrier in sepsis.

Prior research has examined the advantageous impacts of probiotics in preventing and treating sepsis; however, the protective actions of Bifidobacterium against sepsis are still not understood. In this review, we innovatively discussed the mechanisms by which Bifidobacterium can prevent and treat sepsis by regulating the intestinal mucosal barrier during homeostasis and when damaged. Surprisingly, we found that the mechanisms include blocking NF-κB and p38 MAPK signaling pathways, regulating AHR/NRF2/NLRP3 inflammasome pathways, inhibiting NLRP3/ASC/Caspase1 signaling pathways, regulating ente

Unusual gastrointestinal bleeding - Small intestinal venous ectasias.

A 34-year-old man presented to our emergency department with a 6-day history of bloody stools. He had no complaints of alteration in bowel habits in the stool. He did not consume alcohol and was a non-smoker. The patient had no any previous notable medical history or family history of similar complaints. Physical examination was unremarkable. In the local hospital, laboratory studies showed a hemoglobin level of 6.0g per deciliter (normal range, 11.5 to 15.5). After blood transfusion, an upper gastrointestinal endoscopy and complete colonoscopy did not reveal any bleeding lesion. In our hospit

Tight junction proteins and intestinal health in preterm infants.

The intestinal epithelial barrier consists of a single layer of cells that separate the gut lumen from the rest of the body. It regulates and protects the intestinal tract immunologically and mechanically. Necrotizing enterocolitis (NEC) is one of the most common conditions requiring emergency surgery in the neonatal intensive care unit and is associated with a septic shock-like state contributing to multi-organ dysfunction. NEC affects 6-10% of very low birthweight infants and remains a leading cause of death. This review presents current evidence of the role of tight junction (TJ) proteins i

Effects of gestational diabetes mellitus on the intestinal microbiota of the offspring.

Gestational diabetes mellitus (GDM) significantly impacts maternal and neonatal health, yet its influence on the neonatal intestinal microbiota remains poorly characterized. This study investigated the effects of maternal GDM on offspring microbiota by analyzing meconium samples from newborns of GDM-affected mothers (n = 16) and healthy controls (n = 16). Microbiome analysis revealed distinct composition alterations in the intestinal microbiota of GDM-exposed neonates. Significant differences in microbial abundance were observed between groups: at the phylum level, the GDM group exhibited a si

Synchronous Intestinal Tuberculosis and Ulcerative Colitis: A Diagnostic Challenge.

Intestinal tuberculosis (ITB) is a rare manifestation of extrapulmonary tuberculosis that often mimics inflammatory bowel diseases, particularly ulcerative colitis (UC), complicating the diagnostic process. We report the case of a 54-year-old woman with poorly controlled type 2 diabetes who presented with a five-month history of abdominal pain, diarrhea, bloating, fatigue, and low-grade fever. Initial imaging suggested features consistent with inflammatory bowel disease (IBD), and colonoscopy revealed mucosal ulcerations. Histopathological examination confirmed a diagnosis of ITB, and the pati

Intestinal endometriosis: A rare cause of acute care surgery.

BACKGROUND: Intestinal endometriosis is a rare condition that can cause gastrointestinal symptoms such as abdominal pain, constipation, and diarrhea. It occurs in approximately 5%-15% of women with endometriosis. Although it rarely leads to obstruction or perforation, there is no clear consensus on its optimal management. Hormonal therapy is considered the first-line treatment for endometriosis; however, the best approach for intestinal involvement remains controversial. This study aims to contribute to the literature by evaluating patients with intestinal endometriosis who underwent surgery f

Incidence, predictors and outcome of neonatal-onset intestinal failure.

BACKGROUND: Neonatal-onset Intestinal Failure (IF) and Short Bowel Syndrome (SBS) are associated with significant morbidity. We aimed to estimate incidence, identify predictors and determine outcomes of neonatal IF and SBS. MATERIALS AND METHODS: Retrospective review of children admitted to Queensland Children's Hospital between April, 2018 to March, 2022 who received parenteral nutrition (PN) in the neonatal period. IF was defined as those who required PN> 60 days from a gastrointestinal cause. Population-based estimate of incidence was calculated using census data from the national bureau o

Intestinal Obstruction and Neurotoxicity Associated With Water Bead Ingestion.

Water beads are superabsorbent polymer balls. They were originally marketed for agricultural and decorative applications and are now sold as sensory toys. They can be harmful to children in 2 ways. Upon ingestion, water beads absorb liquid and, because they can increase in volume by more than a hundredfold, intestinal obstruction can result. In addition, they are made of polymerized acrylamide monomer. Acrylamide monomer can be toxic at high levels. Here, we present a case of intestinal obstruction in the setting of water bead ingestion temporarily associated with signs of neurotoxicity. The p
