BACKGROUND: Chronic intestinal failure (IF) and long-term parenteral nutrition (PN) impose significant financial burden on patients and the healthcare system. This study aimed to evaluate the healthcare cost of home-PN (HPN) and intestinal transplantation (ITx) at a single-centre.
METHODS: This was a cost-of-illness evaluation based on retrospective analysis of billing records of patients (0-18 years) receiving HPN in 2016-2025 (excluding hospital-PN). Annual HPN cost was calculated based on average cost during the latest 3 years of HPN. Cost of ITx (since 2022) included index hospitalisation

RATIONALE: Castleman disease (CD) is a rare lymphoproliferative disorder that can be associated with autoimmune manifestations. While paraneoplastic pemphigus (PNP) and myasthenia gravis (MG) have each been reported with CD, their simultaneous occurrence is extremely uncommon. Here, we describe a case of mixed-type unicentric Castleman disease (UCD) complicated by both MG and PNP.
PATIENT CONCERNS: A 49-year-old man presented with fluctuating right eyelid ptosis, consistent with ocular MG. Several months later, he developed progressive oral erosions.
DIAGNOSES: The patient was diagnosed with

INTRODUCTION: Castleman disease is a lymphoproliferative disorder of unknown etiology, typically reported as a hypervascular tumor. Here, we report the first known case of unicentric Castleman disease in which a pseudoaneurysm developed within the tumor and ruptured spontaneously.
CASE PRESENTATION: A 55-year-old male was hospitalized due to the sudden onset of epigastric pain. Contrast-enhanced CT revealed a 60-mm hypovascular mass located on the dorsal side of the pancreas, with evidence of extravasation within the tumor. Emergency angiography was performed; nonetheless, the pseudoaneurysm

BACKGROUND: Castleman disease is a rare and heterogeneous lymphoproliferative disorder with variable clinical presentation. The plasma cell variant, particularly when associated with Epstein-Barr virus (EBV), is uncommon and diagnostically challenging. We present a complex and long-lasting case of EBV-associated plasma cell variant Castleman disease with fluctuating systemic symptoms, multiorgan involvement, and delayed definitive diagnosis.
CASE PRESENTATION: A 35year old man (born 1974) first presented in 2009 with high-grade fever, diarrhea, elevated inflammatory markers, hepatomegaly and

BACKGROUND: Primary cutaneous lymphomas (PCLs) are uncommon in children, with mycosis fungoides (MF), lymphomatoid papulosis (LyP), and primary cutaneous small/medium CD4+ T-cell lymphoproliferative disorder (PCSM-TCLPD) representing the most frequent entities. Due to their rarity, available data derive mainly from case reports and small series, and diagnostic delays are frequent. This study aimed to describe the clinical, histopathological, and therapeutic features of pediatric PCLs in a single Italian referral centre and compare them with the literature evidence.
METHODS: We conducted a sin

Chronic lymphocytic leukemia (CLL) is a low-grade lymphoproliferative disorder characterized by clonal proliferation of lymphocytes. The disease is indolent but might be complicated with life-threatening cytopenias, high-grade transformation, or second primary malignancies (SPMs). The incidence of SPMs is higher in CLL compared to the normal population; however, the genetic association and predisposing factors linking CLL to SPMs remain unclear. Synchronous or metasynchronous CLL and lung cancer are relatively uncommon, and the relationship is rarely explored in the literature. We report two c

BACKGROUND: Idiopathic multicentric Castleman disease (iMCD) is a rare polyclonal lymphoproliferative disorder characterised by excessive interleukin-6 (IL-6) production. Interleukin-6 is known to enhance the synthesis of galactose-deficient IgA1 (Gd-IgA1), contributing to the pathogenesis of IgA nephropathy (IgAN). Additionally, vascular endothelial growth factor (VEGF) is also upregulated by IL-6, further exacerbating glomerular injury. Tocilizumab (TCZ), an anti–IL-6 receptor antibody, has been shown to inhibit IL-6 signaling, potentially reducing Gd-IgA1 and VEGF levels. Here, we report a

Idiopathic multicentric Castleman disease is a rare lymphoproliferative disorder that is clinically classified into idiopathic plasmacytic lymphadenopathy (IPL); thrombocytopenia, anasarca, fever, reticulin fibrosis, and organomegaly (TAFRO); and not otherwise specified (NOS). Although each subtype shows varying degrees of hypervascularity, no statistical data on the degree of vascularization have been reported. Additionally, the mechanisms underlying vascularization in each clinical subtype are poorly understood. Here, we aimed to clarify these mechanisms by evaluating the histopathological c

Castleman disease (CD) is a rare lymphoproliferative disorder categorized into unicentric CD (UCD) and multicentric CD (MCD). Surgical intervention is crucial for both diagnosis and treatment. However, comprehensive comparisons and detailed analyses of surgical outcomes remain limited. We retrospectively reviewed thoracic CD patients treated at our hospital from 2010 to 2024. Clinical, pathological, laboratory, and imaging data were compared between UCD and MCD. Treatment outcomes and prognoses were assessed using multivariate Cox regression. We also compared outcomes between video-assisted th

INTRODUCTION: Castleman disease (CD) is a rare lymphoproliferative disorder having a variegated clinical presentation. Diagnosis of the idiopathic HIV- and HHV8-negative multicentric CD (iMCD) subtype poses a challenge given its non-specific clinical manifestations. iMCD presents as diffuse lymphadenopathy with inflammatory manifestations, primarily driven by interleukin-6 (IL-6). Treatment includes suppressing the inflammation by targeting IL-6 with monoclonal antibodies such as siltuximab and, in severe cases, immuno-chemotherapy to control B- and plasma-cell activation.

T-follicular helper (TFH) markers are expressed in the microenvironnement of marginal zone B-cell lymphoma (MZL), and in lymphomas arising from TFH-cells, sometimes making the differential diagnosis difficult. In the skin, the "TFH-spectrum" is poorly defined, going from primary cutaneous lymphoproliferative disorder with small/medium CD4+ T-cells (SMLPD) to cutaneous localizations of systemic angioimmunoblastic T-cell lymphoma (cAITL), and may pass through intermediate forms (primary cutaneous T-follicular helper derived lymphoma, not otherwise specified (PCTFHL,NOS)). INTRODUCTION: Follicular lymphoma (FL) is an indolent, yet heterogeneous, B-cell lymphoproliferative disorder. Although most FL patients respond well to treatment, few with specific traits have a poor prognosis; the latter are difficult to define.
PATIENTS AND METHODS: We retrospectively analyzed data from 143 FL patients treated at the University of Debrecen since 2009 and investigated prognostic factors that may influence the survival of FL patients.
RESULTS: A maximum standardized uptake value (SUVmax) cut-off of 9.85 at the staging positron emission tomography/computed tomography (PET/CT

OBJECTIVES: Immunosuppressive strategies for intestinal transplant have changed over time. However, specific intestinal transplant-oriented protocols and reports on long-term maintenance regimens are scarce. Our objective was to evaluate the impact of 2 different initial immunosuppressive protocols based on thymoglobulin (group A) and basiliximab (anti-interleukin 2 antibody) (group B) and of changes to maintenance immunosuppression over long-term follow-up in intestinal transplant recipients.
MATERIALS AND METHODS: We performed a retrospective analysis of a prospectively established protocol

BACKGROUND: T-cell large granular lymphocytic leukemia (T-LGLL) is a rare lymphoproliferative disorder that starts in T cells and is usually indolent. Long-term use of immunosuppressants, combined with agranulocytosis, is a double-edged sword, as both can lead to serious infections, especially in patients with combined hematologic malignancies and immune defects.
CASE PRESENTATION: A 30-year-old female patient was admitted to the hospital because of agranulocytosis for five years, with chest tightness, fatigue, and fever for two days. Pathology and metagenomic next-generation sequencing (mNGS

BACKGROUND: Previous vascularized composite allograft (VCA) studies from our laboratory have shown that topical FK506 delivery in non-human primates (NHPs) was limited by inadequate dermal penetration and rejection persisted. Herein, we report the first utilization of FK506 via subcutaneously implanted discs to mitigate VCA rejection in NHPs.
METHODS: Full major histocompatibility complex (MHC)-mismatched NHP pairs underwent partial-face VCA and FK506 disc implantation along the suture line. All allotransplants were maintained post-operatively for two months on the FK506 discs, methylpredniso
