Risk Estimation of Late Rectal Toxicity Using a Convolutional Neural Network-based Dose Prediction in Prostate Cancer Radiation Therapy.

PURPOSE: The present study investigated the feasibility of our automatic plan generation model based on a convolutional neural network (CNN) to estimate the baseline risk of grade ≥2 late rectal bleeding (G2-LRB) in volumetric modulated arc therapy for prostate cancer. METHODS AND MATERIALS: We built the 2-dimensional U-net model to predict dose distributions using the planning computed tomography and organs at risk masks as inputs. Seventy-five volumetric modulated arc therapy plans of prostate cancer, which were delivered at 74.8 Gy in 34 fractions with a uniform planning goal, were include

Symbolic heart rate transition motifs during nocturnal sleep are associated with diabetic complications in type 2 diabetes.

Dysfunction in the autonomic nervous system function during sleep is common in type 2 diabetes mellitus and is associated with increased risk of diabetic related complications. While heart rate variability is a standard non-invasive measure of autonomic function, the utility of symbolic heart rate transition motifs to characterize sleep-specific autonomic dysfunction in diabetes has not been explored. Here, we analysed 5-min electrocardiogram segments during daytime and sleep from 35 male participants ((51 ± 17) years old) with type 2 diabetes mellitus. Frequency-domain heart rate variability

A multimodal lifestyle intervention management system for people at risk of cognitive decline.

Dementia stands as the primary reason for disability among older adults. Research indicates that dementia's risk factors are a blend of pathological, lifestyle, and genetic elements, with nearly 40% of the potential risk factors being modifiable. In the absence of a definitive cure, the foremost strategies for managing dementia involve its early detection and measures to decelerate its progression. In this regard, lifestyle multimodal preventive strategies have emerged, guiding precision prevention of dementia. Based on the LETHE clinical trial, a digitized version of the FINGER protocol, we p

Efficacy of the SUPERvised PREhabilitation with Personalised, Aerobic training, Respiratory muscle training and EDucation (SUPER-PREPARED) multi-model in patients with abdominal surgery: protocol for a single-blind, randomised controlled trial.

INTRODUCTION: Globally, millions of abdominal surgeries are performed annually, placing a substantial burden on public health systems worldwide. Postoperative pulmonary complications (PPCs) are common negative consequences after major abdominal surgery that can worsen recovery outcomes. Although the effect of prehabilitation has been frequently investigated in this population, the feasibility and efficacy of delivering a supervised, intensive exercise programme within a limited preoperative period remain underexplored. This randomised controlled trial (RCT) aims to evaluate the effectiveness of

Socioeconomic status and lifestyle as factors of multimorbidity among older adults in China: results from the China Health and Retirement Longitudinal Survey.

BACKGROUND: Multimorbidity is increasingly prevalent among older adults and poses significant challenges to public health systems. While previous studies have highlighted the role of individual behaviors, the complex interaction between lifestyle factors and socioeconomic status (SES) in multimorbidity remains unclear. METHODS: Using nationally representative data from the China Health and Retirement Longitudinal Study (CHARLS), we developed predictive models to identify key determinants of multimorbidity among individuals aged ≥60 years. A total of 34,755 participants were included, and 17 f

Diagnostic accuracy of artificial intelligence for obstructive sleep apnea detection: a systematic review.

BACKGROUND: Obstructive sleep apnea (OSA) is a highly prevalent sleep disorder. Misdiagnosis might lead to several systemic conditions, including hypertension, vascular damage, and cognitive impairment. The gold-standard diagnostic tool for OSA is polysomnography, which is expensive, time-consuming, and not accessible everywhere. Artificial intelligence (AI) algorithms can facilitate diagnosis by detecting patients' signs and symptoms. In this systematic review, we evaluated the diagnostic accuracy of AI models in detecting sleep apnea. METHODS: We searched six major databases, PubMed®, Cochr

Evaluating the Impact of CFTR Modulator Therapy on Antibiotic Utilization: Insights From a Single Center Retrospective Review and Pediatric Health Information System Database.

BACKGROUND: Cystic fibrosis (CF) leads to chronic airway obstruction, inflammation, and infection, resulting in pulmonary exacerbations (PEx) that negatively impact lung function, quality of life, and mortality. The introduction of highly effective CFTR modulator therapy (HEMT) has improved outcomes in people with CF (pwCF), reducing the frequency of PEx and antibiotics. Using data from a single-center retrospective review and the Pediatric Health Information System (PHIS) database, this study evaluates the impact of HEMT on antibiotic utilization in pwCF. METHODS: A single-center, retrospect

XBusNet: Text-Guided Breast Ultrasound Segmentation via Multimodal Vision-Language Learning.

Background/Objectives: Precise breast ultrasound (BUS) segmentation supports reliable measurement, quantitative analysis, and downstream classification yet remains difficult for small or low-contrast lesions with fuzzy margins and speckle noise. Text prompts can add clinical context, but directly applying weakly localized text-image cues (e.g., CAM/CLIP-derived signals) tends to produce coarse, blob-like responses that smear boundaries unless additional mechanisms recover fine edges. Methods: We propose XBusNet, a novel dual-prompt, dual-branch multimodal model that combines image features wit

Explainable artificial intelligence for pneumonia classification: Clinical insights into deformable prototypical part network in pediatric chest x-ray images.

BACKGROUND: Pneumonia detection in chest X-rays (CXR) increasingly relies on AI-driven diagnostic systems. However, their "black-box" nature often lacks transparency, underscoring the need for interpretability to improve patient outcomes. This study presents the first application of the Deformable Prototypical Part Network (D-ProtoPNet), an ante-hoc interpretable deep learning (DL) model, for pneumonia classification in pediatric patients' CXR images. Clinical insights were integrated through expert radiologist evaluation of the model's learned prototypes and activated image patches, ensuring

A Measurement Science Framework to Optimize CDS for Opioid Use Disorder Treatment in the ED.

In the emergency department-initiated buprenorphine for opioid use disorder (EMBED) trial, a clinical decision support (CDS) tool had no effect on rates of buprenorphine initiation in emergency department (ED) patients with opioid use disorder. The Agency for Healthcare Research and Quality (AHRQ) recently released a CDS Performance Measure Inventory to guide data-driven CDS development and evaluation. Through partner co-design, we tailored AHRQ inventory measures to evaluate EMBED CDS performance and drive improvements.Relevant AHRQ inventory measures were selected and adapted using a partner

Low education and employment status drive cardiometabolic health: a social determinants perspective in Chinese population.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome has emerged a major strain on healthcare systems worldwide, with marked disparities in incidence across different regions, which are partly attributed to social determinants of health (SDOH). While research has touched upon the link between SDOH and CKM, and isolated SDOH factor's influence on cardiovascular health has been probed in China, evaluation of how SDOH collectively affect CKM in Chinese population remains underexplored in a systematic fashion. METHODS: The study drew on Shanghai community population (STONE cohort, n = 4,066

Finasteride Use: Evaluation of Depression and Suicide Risk.

BACKGROUND: Oral finasteride 1 mg/day is indicated for androgenetic alopecia (AGA), while 5 mg/day is for benign prostatic hyperplasia (BPH). Oral finasteride has been linked with depression and suicide; however, a causal association is uncertain. The so-called post-finasteride syndrome (PFS) refers to a "cluster" of side effects experienced by some men (i.e., cis men) after taking oral finasteride. AIMS: The objective of the current study was to evaluate the association of depression and suicide with oral finasteride in males, using data from the United States Food and Drug Administration Ad
