Aldosterone influences serum magnesium in Gitelman syndrome.

INTRODUCTION: Information about biologically active adrenomedullin (mature AM), a potential new biomarker for sepsis and septic shock, is limited. Here, we investigated the value of mature AM for diagnosis and outcome prediction in sepsis. MATERIAL AND METHODS: Patients admitted to the intensive care unit (ICU) were retrospectively cate-gorised into non-sepsis or sepsis groups, according to the Sepsis-3 definitions. Plasma levels of mature and total (the sum of the levels of intermediate and mature forms) AM were measured, and their usefulness was compared with that of other sepsis biomarkers

Glibenclamide induces apoptosis through inhibition of cystic fibrosis transmembrane conductance regulator (CFTR) Cl(-) channels and intracellular Ca(2+) release in HepG2 human hepatoblastoma cells.

Glibenclamide, an inhibitor of cystic fibrosis transmembrane conductance regulator (CFTR) Cl(-) channels, induced apoptosis in a dose- and time-dependent manner in HepG2 human hepatoblastoma cells. Glibenclamide increased intracellular Ca(2+) concentration, which was significantly inhibited by Ca(2+) release blockers dantrolene and TMB-8. BAPTA/AM, an intracellular Ca(2+) chelator, and the Ca(2+) release blockers significantly inhibited glibenclamide-induced apoptosis. Glibanclamide also increased intracellular Cl(-) concentration, which was significantly blocked by CFTR Cl(-) channel activato

Bradykinin causes contraction in rat uterus through the same signal pathway as oxytocin.

The signal pathway for bradykinin-induced contraction of the uterine smooth muscle was investigated by comparing the effect of blocking agents on bradykinin and oxytocin induced contractions of the isolated rat uterus in organ bath. The phospholipase C inhibitor U-73,122 abolished the effect of both bradykinin and oxytocin. Inhibition of non-voltage-dependent Ca2+ influx by SK & F 96,365 reduced the contraction induced by both agonists to about 20% of control. The tissues failed to contract when they were exposed to bradykinin or oxytocin in Ca(2+)-free Krebs-Henseleit buffer with 2 mM EDTA. B

Phenotype resembling Gitelman's syndrome in mice lacking the apical Na+-Cl- cotransporter of the distal convoluted tubule.

Mutations in the gene encoding the thiazide-sensitive Na+-Cl- cotransporter (NCC) of the distal convoluted tubule cause Gitelman's syndrome, an inherited hypokalemic alkalosis with hypomagnesemia and hypocalciuria. These metabolic abnormalities are secondary to the deficit in NaCl reabsorption, but the underlying mechanisms are unclear. To gain a better understanding of the role of NCC in sodium and fluid volume homeostasis and in the pathogenesis of Gitelman's syndrome, we used gene targeting to prepare an NCC-deficient mouse. Null mutant (Ncc-/-) mice appear healthy and are normal with respe

Regulatory volume increase in rat pancreatic beta-cells.

This study investigated regulatory volume increase (RVI) in rat pancreatic beta-cells. Volume changes in isolated beta-cells were measured by a video-imaging method. Cell shrinkage was induced by exposure to solutions made hypertonic by the addition of 100 mM mannitol. In HEPES-buffered solutions, beta-cells exhibited an RVI which was almost completely abolished by 10 microM bumetanide. These data indicate that Na+-2Cl--K+ cotransporters make a major contribution to RVI in beta-cells. In HCO3--buffered solutions, however, an RVI was observed in the presence of 10 microM bumetanide. This bumeta

Bartter's syndrome, hypokalaemic alkalosis with hypercalciuria, is caused by mutations in the Na-K-2Cl cotransporter NKCC2.

Inherited hypokalaemic alkalosis with low blood pressure can be divided into two groups-Gitelman's syndrome, featuring hypocalciuria, hypomagnesaemia and milder clinical manifestations, and Bartter's syndrome, featuring hypercalciuria and early presentation with severe volume depletion. Mutations in the renal Na-Cl cotransporter have been shown to cause Gitelman's syndrome. We demonstrate linkage of Bartter's syndrome to the renal Na-K-2Cl cotransporter gene NKCC2, and identify frameshift or non-conservative missense mutations for this gene that co-segregate with the disease. These findings de

Involvement of class A calcium channels in the KCl induced Ca2+ influx in hippocampal synaptosomes.

Using Ca2+ channel toxins, we determined the types of voltage-sensitive calcium channels activated by two levels of KCl depolarization in hippocampal synaptosomes. The increase in the intracellular Ca2+ concentration ([Ca2+]i) induced by 30 mM KCl was equally sensitive to either omega-agatoxin IVA (omega-Aga IVA) or to omega-conotoxin MVIIC (omega-CgTx MVIIC), and the inhibition produced by these two peptides was not additive. The present results indicate that omega-Aga IVA and omega-CgTx MVIIC do not distinguish between two different VSCC in hippocampal synaptosomes and that they both inhibit

Combined fractional excretion of sodium and urea better predicts response to saline in hyponatremia than do usual clinical and biochemical parameters.

BACKGROUND: The treatment of hyponatremic patients requires physicians to make a therapeutic choice between saline infusion and water restriction. Therefore, they need readily available and reliable parameters to facilitate making that choice. This study was designed to determine whether the use of clearance ratios can help clinicians recognize saline-responding hyponatremic patients. PATIENTS AND METHODS: Thirty-five nonedematous, hospitalized, hyponatremic patients were classified according to their history and saline response into four groups: diuretic-taking patients, polydipsic patients,

Philadelphia chromosome (Ph1)-positive acute lymphoblastic leukemia (ALL) is resistant to effective therapy for Ph1-negative ALL.

Amsacrine with high-dose cytarabine is effective therapy for Philadelphia chromosome (Ph1)-negative acute lymphoblastic leukemia (ALL). We examined the effectiveness of this regimen in 19 patients with Ph1-positive lymphoblastic leukemia. Four had an antecedent chronic phase of chronic myelogenous leukemia and 15 presented with ALL. There were no complete responders in either group. All 14 patients whose bone marrow could be assessed after completion of therapy showed persistent leukemia. We conclude that patients with Ph1-positive lymphoblastic leukemia have a disease that is resistant to tre

Extracellular pH and stimulated neurons.

The use of ion-sensitive microelectrodes enabled us to follow the dynamic changes in extracellular pH (pHe) together with those in the extracellular concentration of some biologically important ions, particularly K+ and Ca2+. Activity-related changes in pHe were studied in isolated spinal cords of frogs and in spinal cords of rats in vivo. Repetitive electrical stimulation of an afferent input led either to triphasic alkaline-acid-acid changes (90% of frogs) or to triphasic alkaline-acid-alkaline changes (10% of frogs and rats) with the greatest changes in the lower dorsal horns. The transient

Antidiuretic effects of alpha- and beta-adrenoceptor agonists microinjected into the hypothalamic paraventricular nucleus in a water-loaded and ethanol-anesthetized rat.

Effects of catecholamines microinjected into the paraventricular nucleus in the hypothalamus on urine outflow in a rat which was loaded with water and anesthetized with ethanol were studied. L-Norepinephrine, L-epinephrine and L-isoproterenol induced potent antidiuresis with similar time courses to each other. The ED50 values for L-norepinephrine, L-epinephrine and L-isoproterenol were approximately 5, 10 and 5 nmol, respectively. The D-isomer of isoproterenol demonstrated no significant antidiuretic activity. The effect of L-norepinephrine was inhibited strongly by premicroinjection of alpha-

Contractile responses of vasa deferentia from rats with genetic and experimental hypertension.

The changes in hypertension of the characteristics of the contractile responses of the prostatic portion of the vasa deferentia to norepinephrine (NE), K+, Ca2+ and transmural field stimulation were investigated. Responses of strips from Wistar-Kyoto strain normotensive rats (WKY) were compared to those of strips from spontaneously hypertensive rats (SHR). Also strips from regular normotensive Wistar rats (NWR) which failed to develop hypertension after deoxycorticosterone (DOC)-salt treatment (DNR) were compared to those from NWR with DOC-salt induced hypertension (DHR). The origin of the basal cell potential in frog corneal epithelium.

1. As the micro-electrode penetrated through the epithelial cell membranes of frog cornea, a stepwise increase in the potential reaching a maximum value of - 60 mV (;basal cell potential') was observed. Upon penetrating the stroma, the potential dropped abruptly but remained negative (;stroma potential').2. The basal cell potential was determined as a K(+) or a Cl(-) electrode, but the cell membrane was far more permeable to Cl(-) than to K(+). It was also permeable to Na(+), but to a smaller extent.3. When the product [K](o).[Cl](o) was kept constant, for a tenfold increase in [K](o) the slop
