On the Cusp-A Questionnaire-Based Assessment of Implementing PIVOTAL Into UK Practice.

BACKGROUND: Anemia is a common occurrence in people with chronic kidney disease and end-stage kidney disease. Intravenous administration of iron is standard treatment for people undergoing maintenance hemodialysis. However, until the recent PIVOTAL randomized control trial, there was uncertainty around clinically effective regimens. This study found that among incident, people receiving hemodialysis in the first year, a proactive high-dose intravenous iron regimen was superior to reactive low-dose regimen, leading to reduced mortality and cardiac events. Our study investigated whether the rese

Trends and variation in the use of andexanet alfa for the reversal of direct oral anticoagulants in NHS trusts in England.

AIMS: Andexanet alfa was recommended by the National Institute for Health and Clinical Excellence (NICE) as an option in the management of life-threatening gastrointestinal bleeding in patients taking apixaban or rivaroxaban in May 2021. A recent UK-wide survey of local hospital protocols for the use of andexanet alfa suggested that practice across the United Kingdom is highly variable. In January 2025, NICE was unable to make a recommendation on the use of andexanet alfa for reversing anticoagulation in adults with intracranial haemorrhage. We set out to report trends and variation in the upt

Opioid prescribing trends and pain scores among adult patients with cancer in a large health system.

BACKGROUND: Opioid stewardship policies could adversely affect pain management for patients with cancer. Yet patients with cancer are also at risk for opioid-related harms. This study sought to determine trends in opioid prescribing by clinical stratum and pain for patients with cancer from 2016 to 2020. METHODS: A retrospective study was conducted of opioid-naive adults with newly diagnosed cancer from 2016 to 2020 (N = 10,232) in a large Connecticut health system. Logistic regression was used to calculate changes in the predicted probability of opioid prescribing from 2016 to 2020. Two subp

Insights Into Pharmacist-Led Deprescribing of Benzodiazepines and Related Drugs: A Qualitative Study With Physicians and Pharmacists.

While deprescribing benzodiazepines and related drugs (BZRDs) is crucial for preventing prolonged use and their associated adverse effects, it presents challenges from a healthcare perspective, because of limited resources and time. Recently, a pharmacist-led deprescribing of BZRDs in the treatment of insomnia was introduced in Helsinki's primary care health centres. To explore pharmacists' and physicians' insights, qualitative semi-structured interviews were conducted via Microsoft Teams with physicians and pharmacists involved in a pharmacist-led deprescribing. The interviews were recorded,

Development of a scale for defensive medicine practices in primary care.

BACKGROUND: Defensive medicine, characterized by the avoidance of high-risk patients or the overuse of diagnostic and therapeutic interventions due to fear of litigation or complaints, poses significant challenges in primary care. Existing scales primarily focus on hospital settings, limiting their applicability to family physicians. This study aimed to develop and validate a scale tailored to measure defensive medicine behaviors in primary care contexts. METHODS: Scale development involved reviewing the literature, conducting focus group discussions with family physicians, and consulting wit
