Ultrasound imaging-guided percutaneous treatment of rotator cuff calcific tendinitis: success in short-term outcome.

OBJECTIVE: Rotator cuff calcific tendinitis (RCCT) is a common cause of shoulder pain in adults and typically presents as activity-related shoulder pain. Between non-surgical and surgical treatment options, today a few minimal invasive techniques are available to remove the calcific deposit, and they represent a cornerstone in the management of this painful clinical condition. The aim of the work was a retrospective evaluation of double-needle ultrasound-guided percutaneous fragmentation and lavage (DNL), focused on understanding the factors which are of major importance in determining a quick

Effectiveness of warm water consumption to reduce patient discomfort during colonoscopy: a randomized controlled trial.

OBJECTIVES: Colonic spasm, a relatively common problem during colonoscopy, may interfere with intubation and examination. This study was conducted to compare the effect of warm water consumption before unsedated colonoscopy on the incidence of spasm, pain, and technical difficulties. In addition, we studied the associations between attributes of patients and the degree of pain experienced by them during procedures undertaken after warm water consumption. METHODS: A prospective, randomized, controlled, and investigator-blinded study was conducted in which three groups of patients underwent uns

Relationship between Time Elapsed Since Pain Onset and Efficacy of Pain Relief in Patients Undergoing Lumbar Percutaneous Epidural Adhesiolysis.

PURPOSE: The aim of this study was to investigate the relationship between pain duration and pain relief after epidural adhesiolysis. MATERIALS AND METHODS: Patients with low back pain who underwent lumbar epidural adhesiolysis were enrolled. A clinically significant reduction in pain score was defined as a ≥30% reduction at 6-month follow-up evaluation. Variables were compared based on pain duration categories. Changes in pain scores and pain outcome were also compared. Logistic regression analysis was conducted to identify factors associated with pain relief after adhesiolysis. RESULTS: A

A novel approach to refractory coccydynia: ultrasound- fluoroscopy-guided cryoablation of sacrococcygeal nerve.

Coccydynia, also termed chronic pain in the tailbone, is a complex condition with limited treatment options for refractory cases. This case series introduces a novel approach for treating refractory coccydynia using cryoablation of the sacrococcygeal nerves under combined ultrasound and fluoroscopy guidance. Two female patients, suffering from chronic pain for over six months and unresponsive to conservative interventions, underwent cryoablation. Both patients reported significant pain relief, with more than 50% reduction in their numeric rating scale scores, and no major complications were ob

Percutaneous Spinal Interventions for Chronic Pain Management.

Chronic neck and back pain are two of the most common and disabling complaints seen in primary care and neurology practices. Most commonly these come in the form of cervical and lumbar radiculopathy, lumbar spinal stenosis, and cervical and lumbar facet arthropathy. Treatment options are widespread and include nonpharmacological, pharmacological, surgical, and interventional options. The focus of this review will be to discuss the most common interventional procedures performed for chronic cervical and lumbar back pain, common indications for performing these interventions, as well as associat

Isolated Superior Mesenteric Artery Dissection: An Unusual Etiology of Epigastric Pain.

Abdominal pain is a very common presentation in the emergency department (ED). The pain is often well-characterized and leads to the diagnosis, but often, the presentation is vague and nonspecific. Superior mesenteric artery (SMA) dissection is a rare cause of abdominal pain that presents with nonspecific epigastric pain and is common in males, middle age, and patients of Asian descent. A high index of suspicion is usually helpful with imaging modalities such as computer tomography (CT) scan and ultrasonography in experienced hands. A prompt diagnosis is vital to managing this disease which ma

Experience of fetal scalp blood sampling during labor.

Fetal scalp blood sampling (FBS) is often claimed to be painful for women in labor and difficult for obstetricians to perform. Our aim was to assess women's experience of pain during FBS and obstetricians' experience of difficulty in performing the test. At a tertiary center in Sweden, a questionnaire with answers on a 10-point scale was completed by 51 women and the obstetricians performing the test. Women's experience of pain had a median of 3.5. FBS was well tolerated in women who had epidural analgesia but might be associated with pain in women without. Higher maternal body mass index and

Microsurgical testicular denervation for the treatment of chronic testicular pain initial results.

INTRODUCTION: Chronic testicular pain remains an important challenge for urologists. Currently, the treatment plan is primarily empirical, with the first approach consisting of clinical measures. However, some patients remain in pain despite a conservative treatment protocol and, for them, it is possible to perform a surgical procedure that involves severing the scrotal and spermatic branches of the genitofemoral and ilioinguinal nerve fibers. METHODS: In our institution, 60 patients were evaluated and treated for idiopathic chronic testicular pain between January 2003 and July 2007. Priority

[Therapy of facial pain].

Facial pain is a rare and often misdiagnosed condition. The most common facial pain syndrome is trigeminal neuralgia (TG), which is characterized by paroxysms of excruciating, sharp, burning or lancinating pain. Most cases are caused by a vascular compression of the trigeminal nerve at its entry zone to the pons. Treatment strategies comprise medical treatment and surgical procedures. Anticonvulsants like carbamazepin or gabapentin are widely used for the treatment of TG. The decision on the individual drug therapy may depend on factors like pharmacokinetic properties and individual drug toler

Gluteal artery pseudoaneurysm, a rare cause of sciatic pain: case report and literature review.

This article describes a very unusual case of sciatic pain and motor dysfunction resulting from gluteal artery pseudoaneurysm. A 36-year-old woman with primary infertility sustained an iatrogenic injury to her left gluteal artery during transvaginal ultrasound-guided follicle aspiration. Twenty-five days after the procedure she developed severe left sciatic pain and motor dysfunction. Pelvic computed tomography and magnetic resonance imaging revealed a huge pelvic hematoma. Angiography demonstrated a gluteal artery pseudoaneurysm. Because endovascular occlusion of the aneurysm did not relieve

Breast pain associated with mammographic compression.

Firm breast compression during film-screen mammography is necessary to achieve optimum image quality while minimizing radiation dose. Of 374 women who fully completed a questionnaire following mammography, 225 (60%) reported no pain, 115 (31%) moderate pain and only 3 (1%) reported severe pain. Only one patient stated that the pain from the procedure would prevent her from having a further mammogram. Underlying breast disease (usually fibrocystic disease) is associated with a greater incidence and severity of breast pain, but, no relationship has been demonstrated with regards to the patient's

[The study of neuro-adrenolysis of pituitary gland on cancer pain and experimental approach to reveal its mechanism of pain relief].

Neuro-adrenolysis of the pituitary gland by injection of pure alcohol was done to control intractable pain associated with wide-spread cancer, and pituitary functions were measured before and after the injection. Of a series of 46 patients undergoing a total of 57 neuro-adrenolysis of the pituitary gland, 22 (38.6%) of these procedures obtained complete pain relief 22 (38.6%) showed improvement, and the remaining 13 (22.8%) no effect. Hormonal changes of pituitary gland were not same among the cases and there was no correlation between hormonal changes and the occurrence of pain relief. Experi

Relief of pain by subcutaneous electrical stimulation after ocular surgery.

Electrical stimulation in the treatment of postoperative ophthalmologic pain was applied in 25 patients. In 21, stimulation was applied by means of subcutaneously implanted wires and in four patients the therapy was administered by externally placed electrodes. The subcutaneous electrodes were introduced near the supraorbital and infraorbital nerves at the end of the surgical procedures, and stimulation was immediately begun. Duration of treatment varied between one and three days. The nerve stimulator is a portable, battery-operated apparatus. The implanted electrodes avoid skin resistance an

Efficacy and Safety of Pulsed Radiofrequency of Dorsal Root Ganglion in Elderly Patient Population With Acute and Subacute Zoster-Related Pain.

Background: Herpes zoster (HZ) is typically characterized by a burning, stabbing pain, hyperalgesia, and allodynia. In some patients, despite the lesions resolving, the pain persists and becomes chronic. If the pain continues for more than 6 months after the onset of the pain phase, this condition is called postherpetic neuralgia (PHN). The frequency and severity of PHN increase with advancing age. The pain in PHN can be severe, sometimes resistant to medications, significantly impacting the patients' quality of life. The elderly patient population cannot tolerate the medications due to their

Prolonged Opioid Use and Pain Outcome and Associated Factors after Surgery under General Anesthesia: A Prospective Cohort Association Multicenter Study.

BACKGROUND: There is insufficient prospective evidence regarding the relationship between surgical experience and prolonged opioid use and pain. The authors investigated the association of patient characteristics, surgical procedure, and perioperative anesthetic course with postoperative opioid consumption and pain 3 months postsurgery. The authors hypothesized that patient characteristics and intraoperative factors predict opioid consumption and pain 3 months postsurgery. METHODS: Eleven U.S. and one European institution enrolled patients scheduled for spine, open thoracic, knee, hip, or abd

Physician variation in diagnostic testing for low back pain. Who you see is what you get.

OBJECTIVE: This study examined patterns of diagnostic test use for patients with low back pain. Three specific questions were addressed: 1) What tests do physicians recommend for patients with 3 common types of low back pain? 2) Do physicians in various specialties differ in the tests they would order? and 3) How appropriate are physicians' choices of tests, based on current medical knowledge and expert recommendations? METHODS: A stratified national random sample of 2,604 physicians in 8 specialties was mailed questionnaires asking about the tests they would order for hypothetical patients w

An algorithmic approach for clinical management of chronic spinal pain.

Interventional pain management, and the interventional techniques which are an integral part of that specialty, are subject to widely varying definitions and practices. How interventional techniques are applied by various specialties is highly variable, even for the most common procedures and conditions. At the same time, many payors, publications, and guidelines are showing increasing interest in the performance and costs of interventional techniques. There is a lack of consensus among interventional pain management specialists with regards to how to diagnose and manage spinal pain and the ty
