The irritable bowel syndrome: review and a graduated multicomponent treatment approach.

The irritable bowel syndrome is a common chronic disorder having a broad clinical spectrum of severity. Although only a small proportion of those afflicted seek medical help for their symptoms, a subset have severe and intractable symptoms. A positive diagnosis should be established from the history and physical examination; endoscopic and radiologic investigations should be minimized. We suggest that the physician also assess the severity of the illness based on its symptomatic and functional features and the patient's behavioral response. Classifying the disorder in this manner permits a gra

Treatment-related differences in quality of life and psychological distress among patients with hepatocellular carcinoma: A cross-sectional analysis.

OBJECTIVES: Hepatocellular carcinoma (HCC) is associated with high mortality and imposes substantial symptom and psychological burdens; however, the impact of different treatment modalities on quality of life (QoL) and mental health remains underexplored. This study aimed to examine the associations among symptom distress, depression, and QoL across various HCC treatments. METHODS: A cross-sectional study was conducted with 101 inpatients at a regional hospital in Taiwan (October 2020-December 2021). Patients received hepatic resection (HR), radiofrequency ablation (RFA), transarterial chemoe

Perceived Hypoacusis and Loneliness: A Longitudinal Between- and Within-Individual Association in Mexican Older Adults.

Hearing loss is highly prevalent among older adults and is associated with negative psychosocial outcomes such as loneliness. Loneliness is a major public health concern due to its links with functional decline, morbidity, and mortality. While international studies have identified associations between hearing loss and loneliness, few examine this relationship longitudinally, particularly in Latin America. Furthermore, little is known about whether these associations arise from structural differences between individuals or from changes within the same individual over time. We conducted a longit

Cognitive Side Effects of Antiseizure Medications in Adults with Epilepsy: An Update with a Focus on New Therapeutic Agents.

Antiseizure medications (ASMs) are the mainstay of epilepsy treatment but may adversely affect cognitive functions, deepening the cognitive and psychosocial burden intrinsic to epilepsy. Cognitive side effects vary widely across drug classes, doses, treatment regimens, and individual susceptibility, typically affecting attention, processing speed, memory, language, and executive function. This narrative review provides an updated synthesis of the clinical evidence on the cognitive effects of ASMs, mostly in adults, building on a prior 2009 review and focusing on agents introduced into clinical

Feasibility study on a noninvasive assessment of ALS patient emotional state.

INTRODUCTION: Objective assessment of emotional responsiveness in Amyotrophic Lateral Sclerosis (ALS) remains limited despite its importance for personalized care. This study investigated whether non-invasive speech analysis could identify digital biomarkers associated with emotional coping behaviors in ALS patients. METHODS: We analyzed 28 ALS patient visits using clinician-rated emotional concern scores, ALS Functional Rating Scale-Revised (ALS-FRS-R), forced vital capacity (FVC), and speech acoustic features. We also evaluated the reliability of large language models (LLMs), including Chat

Work stress and healthy aging: A conceptual framework for the Semmelweis study and the Semmelweis-EUniWell workplace health promotion model program.

Chronic occupational stress is increasingly recognized as a modifiable accelerator of biological aging, yet it remains underappreciated as a target of geroscience-informed public health intervention. This review reframes workplace stress as a life-course determinant of aging trajectories and introduces a translational framework that positions occupational environments as platforms for promoting healthy aging. Drawing on insights from the Semmelweis Study, a large longitudinal occupational cohort, and the Semmelweis-EUniWell Workplace Health Promotion Model Program, we examine how sustained wor

Longitudinal Voice Handicap in Adult Recurrent Respiratory Papillomatosis Under Stepwise Treatment.

OBJECTIVES: To characterize longitudinal trajectories of patient-reported voice handicap during stepwise treatment for recurrent respiratory papillomatosis (RRP) and determine if total voice handicap index (VHI) or its domains best represent subjective outcomes compared to endoscopically documented disease regression. METHODS: A longitudinal analysis of 15 adults with severe RRP managed by sequential surgical debulking and adjuvant bevacizumab. We assessed VHI-30 (total and functional, physical, and emotional domains) and objective endoscopic papilloma burden (Derkay scale) across 3 to 6 visi

Physiotherapists' Clinical Reasoning and Therapeutic Strategies in Fibromyalgia: A Qualitative Interpretative Study.

Background/Objectives: Fibromyalgia (FM) is a chronic pain condition characterized by widespread pain, fatigue, sleep disturbances, and psychosocial challenges. Although physiotherapists play a key role in FM management, little is known about how biopsychosocial principles are applied in clinical practice. This study explored physiotherapists' clinical reasoning, therapeutic strategies, and approaches to promoting adherence and functional recovery in FM. Methods: A qualitative study was conducted within a constructivist-interpretative paradigm. Thirteen physiotherapists with experience in FM m

Beyond Rehabilitation: Triangulating Return-to-Activity and Return-to-Sport Decisions in Golfers with Back Pain.

BACKGROUND: Non‑specific low back pain (NSLBP) is highly prevalent among golfers and complicates return‑to‑activity (RTA) decisions. Existing return‑to‑sport (RTS) approaches often focus narrowly on biomedical criteria and lack golf‑specific, theoretically grounded frameworks. This study presents a preliminary conceptual framework (Phase 1) for RTA and RTS in golfers with NSLBP. METHODS AND MATERIAL: A hermeneutic systematic literature review was combined with qualitative expert interviews to integrate scientific evidence and clinical expertise. Seven studies on RTA/RTS and NSLBP were analyze

Emerging microsurgical techniques for facial nerve reconstruction in traumatic skull base fractures: a systematic review and comprehensive evidence analysis.

Traumatic brain injury affects 69 million people annually, with skull base fractures causing devastating facial nerve palsy. Despite representing only 4% of cases, these injuries result in severe functional and psychosocial complications including hearing loss and cranial nerve dysfunction, presenting significant reconstructive challenges. This systematic review followed PRISMA guidelines and examined facial reanimation surgery outcomes in skull base fracture patients through comprehensive database searches and quality appraisal. Study quality was assessed using Joanna Briggs Institute (JBI) c

Examining the factor structure of health-related quality of life in spinal cord injury using confirmatory factor analysis and multidimensional scaling.

PURPOSE: Spinal cord injury (SCI) is a complex medical condition and managing the multiple, co-occurring symptoms and functional and social limitations that follow SCI is a challenging task for clinical care providers. Understanding how the various post-SCI symptoms and limitations are interrelated or cluster together can inform patient care and improve clinical workflows. The purpose of this study was to determine the factor structure of the SCI-QOL measurement system and associated symptom clusters that affect health-related quality of life (HRQOL) in individuals with SCI. METHODS: Particip

Recurrent blood-spitting as a somatic presentation of pediatric functional neurological disorder: a case report.

BACKGROUND: Hemoptysis in children is typically associated with identifiable organic causes such as respiratory infection, bronchiectasis, or coagulopathy, and is often accompanied by respiratory symptoms or anemia. However, approximately 10-25% of pediatric cases remain cryptogenic or represent pseudohemoptysis despite extensive diagnostic evaluation. CASE PRESENTATION: We report a 10-year-old boy presenting with daily recurrent blood-spitting manifested as scant blood-tinged saliva (<5 mL per episode) for 12 months. There were no accompanying respiratory or gastrointestinal symptoms, and no

What's in a name? reframing advanced Parkinson's disease as a multidimensional state.

"What's in a name?" is more than a literary question for advanced Parkinson's disease (APD). Although APD is widely used in clinical practice, what constitutes "advanced" remains poorly defined and inconsistently applied. Existing definitions have often anchored APD to motor complications, treatment complexity, or eligibility for device-aided therapies (DATs). These approaches are useful for referral and treatment planning, but they risk mistaking a treatment phenotype for the whole disease state. They also fail to capture the broader burden of progression, including non-motor symptoms, cognit

The Versatility of the Fat-extended, Muscle-sparing Latissimus Dorsi Flap in Breast Reconstruction.

BACKGROUND: The latissimus dorsi (LD) myocutaneous flap is widely used in breast reconstruction, often combined with implants, due to its reliable vascular axis. However, traditional techniques can result in functional deficits and complications. The fat-extended muscle-sparing LD flap (FEMSLDF) was developed to optimize breast volume restoration while minimizing complications. This study aimed to evaluate the effectiveness, safety, and patient satisfaction associated with the FEMSLDF. METHODS: A prospective study was conducted on 172 patients who underwent FEMSLDF-based breast reconstruction

Accelerated aging in people experiencing homelessness: A rapid review of frailty prevalence and determinants.

INTRODUCTION: Older people experiencing homelessness (PEH) are a rapidly growing population at risk of accelerated aging and the early onset of geriatric conditions. One construct that shows promise in predicting age-related decline is frailty. Better understanding the rates and causes of frailty in PEH may improve understanding of its antecedents, thereby facilitating more targeted health and aged care service interventions. The aim of this study was to conduct a rapid review on the prevalence and determinants of frailty in adult PEH. METHODS: We conducted a rapid review of primary research
