Tear Completion Versus In Situ Repair for 50% Partial-Thickness Bursal-Side Rotator Cuff Tears: A Biomechanical and Histological Study in an Animal Model.

BACKGROUND: Tear completion followed by repair (TCR) and in situ repair (ISR) have been widely used for bursal-side partial-thickness rotator cuff tears (PTRCTs). Both techniques have shown favorable results; however, controversy continues in terms of the best management. PURPOSE: To compare the histological and biomechanical outcomes of these 2 techniques for 50% partial-thickness bursal-side rotator cuff tear repair in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 27 rabbits were used in this experimental study. Seven rabbits were sacrificed at the beginni

Internal Derangement of the Shoulder Joint in Asymptomatic Professional Baseball Players.

RATIONALE AND OBJECTIVES: To evaluate the influence of throwing activity on shoulder morphology and the difference in shoulder morphology on MRI between asymptomatic professional baseball players and volunteers who play baseball as a recreational activity. MATERIALS AND METHODS: This retrospective case-control study included 68 asymptomatic professional baseball players (32 pitchers, 36 batters) and 30 male volunteers. Morphologic changes in the following shoulder structures were assessed on MRI: rotator cuff, glenoid labrum, humeral head, subacromial-subdeltoid bursa, subcoracoid bursa, long

Should We Have a Better Definition of Pseudoparalysis in Patients With Rotator Cuff Tears?

Pseudoparalysis has been previously described as the inability to elevate the arm over 90° in the setting of a rotator cuff tear. Controversy exists regarding the optimal treatment for patients with a pseudoparalytic shoulder with a rotator cuff tear with options including rotator cuff repair and reverse shoulder arthroplasty. Recent literature suggests that pseudoparalysis can reliably be corrected with a rotator cuff repair and is more cost effective than reverse shoulder arthroplasty. We believe that the arbitrary cutoff of 90° is too generous and leads to confusion. We believe that the def

Management of Proximal Biceps Pathology in Overhead Athletes: What Is the Role of Biceps Tenodesis?

Overhead throwing is a common causative factor in disorders of the proximal biceps in athletes. In recent years, arthroscopic repair of unstable superior labral tears or biceps tendonitis involving the long head of the biceps tendon anchor has become the standard of care. However, in some cases, superior labrum anterior-posterior (SLAP) repair requires additional evaluation and even revision surgery, which contributes to patient dissatisfaction. Differentiating overall biceps tendonitis from superior labral tears alone can be a challenge. More recently, biceps tenodesis has been suggested as a

Prognostic factors influencing the outcome of rotator cuff repair: a systematic review.

PURPOSE: To identify prognostic factors significantly associated with rotator cuff repair outcome and define the strength of these associations. METHODS: Search was performed using electronic databases. Studies reporting prognostic factors affecting rotator cuff repair outcome were included. Primary outcomes were: structural integrity, Disabilities of the Arm, Shoulder and Hand score, American Shoulder and Elbow Surgeons score, and Constant score. Each other outcome was considered as secondary outcome. Descriptive statistics was used. When possible, meta-analyses were performed. Methodologica

Advantages and techniques of utilizing anterolateral portal in delaminated rotator cuff repair.

Typically, rotator cuff repair is performed two-dimensionally while visualizing the subacromial space. To achieve a more complete repair, sutures can be retrieved from inside the joint utilizing a penetrating suture device through the anterolateral portal, which permits visualization of the articular side and bursal side of the rotator cuff tear. Utilizing other portals can leave the sutures out of sight and reach. The anterolateral portal helps capture both leaves of a delaminated tear and places the sutures in the center of the field for retrieval and tying. The anterolateral portal is locat

Surgical management of isolated greater tuberosity fractures of the proximal humerus.

Because the greater tuberosity is the insertion site of the posterior superior rotator cuff, fractures can have a substantial impact on functional outcome. Isolated fractures should not inadvertently be trivialized. Thorough patient evaluation is required to make an appropriate treatment decision. In most cases surgical management is considered when there is displacement of 5 mm or greater. Although reduction of displaced greater tuberosity fractures has traditionally been performed with open techniques, arthroscopic techniques are now available. The most reliable techniques of fixation of the

[Critical considerations in arthroscopic stabilization of the unstable shoulder joint].

There is increasing interest in arthroscopic techniques for stabilisation of unstable glenohumeral joints. Consequently, plenty of new surgical techniques are developed over the last few years. However, most of the authors just report on short term results. In the presented paper, the current indications for surgical treatment in shoulder instability are discussed. The different arthroscopic techniques are described in detail. The advantages and disadvantages in relation to the conventional open Bankart-operation are presented, and the technique related risks are discussed in detail. Consideri

[Surgical approach to the axillary nerve via an anterior trans-coracoid route].

The surgical approach to the circumflex nerve is difficult. Based on an illustrative clinical example, the authors describe a method using a coracoid osteotomy that permits a simplified approach to the circumflex nerve. This trick permits en bloc retraction of the superficial plane of the brachial plexus without sectioning of the muscle and without excessive traction on the musculocutaneous nerve. The stable fixation of the coracoid at the end of the procedure permits early and effective rehabilitation.

Loss of the fibrocartilaginous lining of the intertubercular sulcus associated with rupture of the tendon of the long head of biceps brachii.

Fibrocartilage lines the intertubercular sulcus of the humerus and protects both the bone and the tendon of the long head of biceps brachii where the tendon passes through the sulcus. It provides a smooth, resilient, lubricated gliding surface on the bone. The fibrocartilage is highly metachromatic and organised into distinct superficial and deep zones. In the superficial zone, the cells are small and the fibres run parallel to the articular surface. In the deep zone, the cells are large and rounded and the coarse bundles of fibres are interwoven. In 6 of the 26 dissecting room cadavers examin

[Destructive arthropathy of the shoulder].

Destructive arthropathy of the shoulder consists of cartilaginous deterioration combined with subchondral osteolysis. The various diseases able to induce joint destruction (infection, neurological disease, chondrocalcinosis, tumour...) are excluded from this description which only concerns the forms with no obvious cause. Since the description of "senile haemorrhagic caries of the shoulder", several authors have reported, under various names, very similar diseases whose common denominator is destruction of the shoulder joint. The pathogenic factors remain unclear, but various theories have bee

Massive synovial cyst of the shoulder causing vascular compromise. A case report.

A 47-year-old paraplegic patient is described who developed a massive synovial cyst of the upper extremity. The cyst developed in combination with a tear of the rotator cuff and the long head of the biceps tendon and involved the fasica surrounding the long head of the biceps. The increased pressure in the cyst resulted in a compartment syndrome of a portion of the long head of the biceps muscle. A review of the literature revealed no similar case.
