Sleep lowers waking theta frequency in the rat hippocampus.

Hippocampal theta oscillations coordinate computations underlying learning and memory. The frequency of theta varies with factors such as locomotion and anxiety, but the effect of an animal's sleep history on theta frequency remains unknown. Using long-duration CA1 recordings in rats, we found that awake theta frequency progressively decreases following sleep but remains elevated during sleep deprivation. These changes were not accounted for by movement but were predicted by the proportion of sleep in the preceding 30 min. In contrast, theta frequency remained relatively stable during rapid ey…

Unsupervised Learning Identifies Sleep Disturbance Subtypes Among Dementia Caregivers.

BACKGROUND: Sleep disturbance is highly prevalent among caregivers of persons with dementia (PWD), affecting approximately 67% of the more than 10 million adults in this role in the United States. Accurately characterizing these disturbances is essential to identify individuals at increased risk for chronic sleep problems and related health consequences. However, few studies have examined how sleep disturbance manifests across diverse caregiver characteristics and contexts. OBJECTIVES: To address this gap, we applied clustering analyses using wearable sleep data to identify distinct subtypes …

Sleep, anxiety, and depression: The bidirectional brain-behavior axis.

Sleep, anxiety, and depression are interconnected processes that greatly influence mental health throughout life. Recent studies indicate that sleep issues are not just symptoms of mental illness but can also cause emotional and cognitive problems. Anxiety and depression can disrupt sleep patterns, and ongoing sleep difficulties can raise the likelihood of mood disorders. This reciprocal relationship involves neural pathways, neurochemicals, hormones, immune responses, and social factors. This chapter explores how sleep, anxiety, and depression interact from various scientific and clinical vie…

Sleep spindles stabilize human thalamocortical networks, inhibiting pathological disruptions.

The thalamus coordinates sleep-dependent brain function including memory consolidation, sensory gating, and network stability through spindle oscillations. Thalamocortical circuits are also affected in many neurological and neuropsychiatric disorders. Yet how human thalamocortical circuits respond to pathological disruptions remain poorly understood. Here, we leveraged interictal epileptiform discharges (IEDs) as spontaneous perturbations to probe spindle-generating circuits and thalamocortical dynamics in vivo. Using multi-night intracranial recordings spanning multiple thalamic nuclei and co…

Chronic sleep restriction induces oculomotor impairment with inadequate compensation.

STUDY OBJECTIVES: Chronic sleep loss is widespread and can cause deficits in vigilant attention. There are few practical biomarkers that can be used to assess the impact of chronic sleep loss on alertness and performance. Those that are available, such as tests of vigilant attention, cannot discriminate chronic sleep loss from other impairments, like alcohol intoxication, limiting the use of such tools. We aimed to test whether a suite of behavioral oculometrics would be sensitive, and perhaps specific, to chronic sleep restriction (CSR). METHODS: We conducted a randomized, counterbalanced, c…

Mechanisms Linking Sleep-Disordered Breathing and Cardiometabolic Disease Risk.

Sleep-disordered breathing (SDB) is a group of disorders defined by intermittent closure or narrowing of the upper airway, caused either by mechanical obstruction or dysregulation of the respiratory centers in the brainstem. The effects of SDB on cardiovascular and metabolic health and disease have been an area of growing interest. Many studies have shown mechanistic links between physiological changes seen in SDB and important cardiometabolic outcomes. In particular, SDB induces alterations in autonomic function, swings in intrathoracic pressure, systemic inflammation, sleep fragmentation, an…

Towards long-term sleep staging via wearable reflective photoplethysmography.

STUDY OBJECTIVES: Sleep staging is usually performed by manual scoring of polysomnography (PSG), which is expensive, laborious, and poorly scalable. We propose an alternative to PSG for ambulatory sleep staging using wearable photoplethysmography (PPG) recorded by a smartwatch and automated scoring. METHODS: We previously trained a deep learning model on public datasets, with the specific purpose of performance generalizability to unseen datasets. In the present work, the model was assessed on two datasets of reflective PPG collected from wrist-worn devices: (1) 68 overnight recordings and (2…

EEG Microstates and Related Brain Networks During Light Sleep.

Light sleep, including Stages N1 and N2, constitutes more than half of total human sleep duration. It serves essential functions in transitioning from wakefulness to deep sleep and in memory processing. Previous studies have identified four consistent electroencephalogram (EEG) microstates during wakefulness and sleep. Simultaneous EEG and functional magnetic resonance imaging (fMRI) studies have shown that EEG microstates are associated with specific brain functional networks during wakefulness and slow wave sleep. However, the relationship between microstates and brain networks during light …

Microglia stabilize sleep homeostasis via adenosine A3 receptor signaling.

Sleep homeostasis maintains the sleep-wake balance through sleep pressure, a process partly orchestrated by the accumulation of extracellular adenosine (eADO). Microglial Ca2+ activity has been implicated in sleep regulation, but the mechanism whereby microglia sense sleep pressure remains unclear. Here, we show that microglia regulate sleep homeostasis through brain state-dependent calcium ion activity driven by adenosine A3 receptor (A3R) signaling. Using miniaturized two-photon microscopy in freely behaving mice, we demonstrate that microglial calcium ion activity is rapidly altered by brai…

Sleep disturbances as early indicators of incident psychiatric disease.

PURPOSE: Sleep disturbances are common in individuals with psychiatric diseases and may emerge years before diagnosis. It remains unclear whether they reflect modifiable risk factors or early manifestations of psychiatric illness. We aimed to investigate how different sleep dimensions relate to the risk of subsequent psychiatric illness, and how sleep symptoms evolve before and after diagnosis. METHODS: We analyzed data from 24,038 participants in the Swedish Longitudinal Occupational Survey of Health (2010-2018), linked to national health registers. Associations between baseline sleep charac…

Pharmacologic Treatment for Obstructive Sleep Apnea in Older Adults.

Older individuals frequently suffer from obstructive sleep apnea yet present with less severe symptoms than younger counterparts. Diagnosing obstructive sleep apnea in older adults poses unexpected challenges owing to poor specificity of screening tools and difficulty attending polysomnography. Continuous positive air pressure remains recommended, although its benefits in this population are not clear and many patients refuse to even attempt the therapy. Emerging research regarding the pathophysiology of obstructive sleep apnea has facilitated greater understanding of potential pharmacologic t…

Diagnosis of sleep-disordered breathing using few-shot learning.

Sleep-Disordered Breathing (SDB) is a common and clinically significant disorder characterized by recurrent airflow limitation and oxygen desaturation during sleep, which can lead to serious cardiovascular and metabolic complications. Accurate and early diagnosis of SDB is crucial for timely clinical intervention and risk stratification, yet diagnostic results are often influenced by substantial variations in physicians' clinical experience and diagnostic skills across different regions, particularly in large-scale screening and real-world medical settings. However, existing diagnostic methods…

Syncope and obstructive sleep apnea: Mechanisms and clinical scenarios.

Obstructive sleep apnea (OSA) is a highly prevalent sleep disorder affecting a considerable proportion of the adult population. It is characterized by chronic intermittent hypoxia (CIH), resulting from recurrent partial (hypopnea) or complete obstruction (apnea) of the upper airway during sleep. Clinically, OSA produces fragmented sleep, loud snoring, excessive daytime somnolence, cognitive dysfunction, and nocturnal diaphoresis. Importantly, OSA is recognized as an independent risk factor for systemic hypertension and is associated with cardiac arrhythmias and stroke. The CIH contributes to t…
