Restoration of normal motor control in Parkinson's disease during REM sleep.

Although normal subjects do not move during REM sleep, patients with Parkinson's disease may experience REM sleep behaviour disorder (RBD). The characteristics of the abnormal REM sleep movements in RBD have, however, not been studied. We interviewed one hundred consecutive non-demented patients with Parkinson's disease and their bed partners using a structured questionnaire assessing the presence of RBD. They rated the quality of movements, voice and facial expression during RBD as being better, equal or worse than in awake ON levodopa condition. Night-time sleep and movements were video-moni

Night-to-night rapid eye movement sleep variability: A relevant marker of early amyloid-β deposition.

INTRODUCTION: Sleep disturbances are prevalent in patients with Alzheimer's disease (AD) and may emerge before overt clinical symptoms. We characterized sleep alterations in cognitively unimpaired older adults with cerebral amyloid deposition, and assessed their associations with regional amyloid deposition and cognitive and psychoaffective outcomes. METHODS: Seventy-six older adults (69.1 ± 3.4 years, 63.2% female) underwent a multi-night (4.5 ± 0.8 nights) objective sleep assessment using the Somno-Art wearable device, Florbetapir-positron emission tomography (PET) scanning, and an extensiv

Joint effects of sleep duration and depressive symptoms on the risk of cardiovascular disease and mediating role of depressive symptoms in middle-aged and older Chinese adults: A nationwide prospective cohort study.

BACKGROUND: Cardiovascular diseases (CVD) are the leading cause of mortality in China. Depression and sleep disturbances are recognized as risk factors for CVD, but their interplay and combined effects remain unclear. This study examines how sleep duration and depressive symptoms interact to influence CVD risk. METHODS: We analyzed data from five waves of the China Health and Retirement Longitudinal Study (CHARLS) between 2011 and 2020. The final sample included 12,050 individuals aged 45 and above. Sleep duration was categorized as 6 h or less, 6-8 h and > 8 h. Depressive symptoms were asses

The effect of sleep on gastrointestinal functioning in common digestive diseases.

Sleep quality and sleep disorders affect symptom manifestation and the pathogenesis of digestive diseases. Sleep is largely regulated by the light-dark cycle and associated circadian rhythms. These occurrences are closely regulated through several mechanisms with direct effects on the gastrointestinal tract. Misalignment of the circadian system is a common cause of sleep complaints, which play an important role in the presentation of many gastrointestinal disorders. This Review will focus on sleep disorders and how these alterations in sleep play an important role in many commonly encountered

Impact of rapid ascent to high altitude on sleep.

PURPOSE: Sleep disturbance at high altitude is common in climbers. In this study, we intended to evaluate the effect of rapid ascent on sleep architecture using polysomnography (PSG) and to compare the differences between subjects with and without acute mountain sickness (AMS). METHODS: The study included 40 non-acclimatized healthy subjects completing PSG at four time points, 3 days before the ascent (T0), two successive nights at 3150 m (T1 and T2), and 2 days after the descent (T3). All subjects were transported by bus from 555 to 3150 m within 3 h. AMS was diagnosed using self-reported qu

[Prevalence and stress factors of sleep disorders in children starting school].

Epidemiological studies of sleep disturbances in preschool and school-aged children are under-represented in international clinical research. Based on this fact we investigated the prevalence of insomnia and parasomnia in a representative sample of school-novices in Cologne in 2002 (Cologne Children's Sleep Study). We have also considered the relationship between sleep problems on the one hand and sleep hygienic variables, physical and somatic healthy factors and behavioral problems on the other. A high prevalence of insomnia problems (15%) and repeated nightmares (14%) in children of this age

Delayed sleep phase syndrome: a review of its clinical aspects.

OBJECTIVE: Delayed sleep phase syndrome is a common but little reported cause of severe insomnia. Since it was first described, few detailed reports of delayed sleep phase syndrome have appeared, and treatment methods have not been reviewed. From the literature, the authors provide diagnostic descriptions and review treatment methods, and from their sleep disorder clinic, they describe the management and outcome of the largest series of patients with delayed sleep phase syndrome thus far reported. METHOD: The authors reviewed all articles with primary data on delayed sleep phase syndrome publ

Pharmacotherapy in obstructive sleep apnoea: a clinical guide.

PURPOSE: Obstructive Sleep Apnoea (OSA) is a common, yet underdiagnosed disorder, with significant health implications. Continuous positive airway pressure (CPAP) remains the reference standard and first-line therapy. However, successful CPAP usage is influenced by factors such as access, cost, tolerance, patient education, early troubleshooting, and long-term adherence, all of which can be challenging and resource-demanding. Adding therapeutic options to the armamentarium is therefore welcome. There is a need for clinicians to be abreast of all the available therapeutic options for treatment

Circadian Rhythm and Psychiatric Features in Wolfram Syndrome: Toward Chrono Diagnosis and Chronotherapy.

Background/Objectives: Wolfram syndrome is a rare neurodegenerative disorder primarily known for its multisystemic manifestations. Although classically associated with diabetes insipidus, diabetes mellitus, optic atrophy, and deafness, emerging evidence suggests a consistent pattern of executive dysfunction in many affected individuals. Methods: Based on findings from a scoping review and results obtained through the Dysexecutive Questionnaire in a Spanish patient cohort, we propose that WFS1 gene mutations-via chronic endoplasmic reticulum stress-disrupt serotonergic and cholinergic neurotran

Targets for obstructive sleep apnea pharmacotherapy: principles, approaches, and emerging strategies.

INTRODUCTION: Obstructive sleep apnea (OSA) is a common and serious breathing disorder. Several pathophysiological factors predispose individuals to OSA. These factors are quantifiable, and modifiable pharmacologically. AREAS COVERED: Four key pharmacotherapeutic targets are identified and mapped to the major determinants of OSA pathophysiology. PubMed and Clinicaltrials.gov were searched through April 2023. EXPERT OPINION: Target #1: Pharyngeal Motor Effectors. Increasing pharyngeal muscle activity and responsivity with noradrenergic-antimuscarinic combination is central to recent breakthro

Which Actigraphy Dimensions Predict Longitudinal Outcomes in Bipolar Disorders?

Bipolar disorder (BD) is characterized by recurrent mood episodes. It is increasingly suggested that disturbances in sleep-wake cycles and/or circadian rhythms could represent valuable predictors of recurrence, but few studies have addressed this question. Euthymic individuals with BD (n = 69) undertook 3 weeks of actigraphy recording and were then followed up for a median duration of 3.5 years. Principal component analyses were used to identify core dimensions of sleep quantity/variability and circadian rhythmicity. Associations between clinical variables and actigraphy dimensions and time to

The key role of daytime sleepiness in cognitive functioning of adults with attention deficit hyperactivity disorder.

BACKGROUND: Adults with attention deficit hyperactivity disorder (ADHD) frequently suffer from sleep problems and report high levels of daytime sleepiness compared to neurotypical controls, which has detrimental effect on quality of life. METHODS: We evaluated daytime sleepiness in adults with ADHD compared to neurotypical controls using an observer-rated sleepiness protocol during the Sustained Attention Response Task as well as electroencephalogram (EEG) slowing, a quantitative electroencephalographic measure collected during a short period of wakeful rest. RESULTS: We found that adults wi

Electroencephalography in holoprosencephaly: findings in children without epilepsy.

OBJECTIVE: To evaluate the electroencephalographic characteristics of patients with holoprosencephaly (HPE) without epilepsy. METHODS: We evaluated the electroencephalograms (EEGs) of 18 children with HPE who lacked a history of seizures. Neuroimaging studies were assessed for severity of HPE and thalamic non-separation and the presence of dorsal cysts and cortical malformations. RESULTS: Hypersynchronous theta activity occurred in 50 and 60% of EEGs during wakefulness or drowsiness/sleep, respectively, and correlated with the grade of thalamic non-separation (p<0.05). Hypersynchronous beta
