The distribution of pathogenic anaerobes in the environment and the relationship with diseases in animals are discussed. A distinction between the spore-bearing anaerobes (clostridia) and the Gram-negative non-spore-forming anaerobes is necessary. A 17-year-old female patient with hemoglobin SS sickle cell disease, Moyamoya disease, a history of stroke, and pulmonary embolism was incidentally found to have a large pedunculated right atrial mass on cardiac magnetic resonance imaging. The differential diagnosis included thrombus, tumor, and vegetation. She underwent cardiac catheterization for therapeutic removal and pathologic confirmation of the diagnosis. This case highlights the diagnostic challenges of right atrial masses and the importance of integrating clinical context with multimodality imaging.

Chromoblastomycosis (CBM), also known as chromomycosis, is a rare, granulomatous fungal infection caused primarily by pigmented fungi. It typically affects the skin and subcutaneous tissue and is usually acquired through traumatic implantation of fungal spores from contaminated soil, wood, or vegetation. The most common manifestations are verrucous or nodular lesions. Cystic forms are rare and can mimic other dermatologic conditions, leading to diagnostic challenges. We report a case of CBM in a 43-year-old male presenting with an unusual cystic lesion.

Herein we describe the case of a Black adolescent who was found to have widely metastatic melanoma originating from a primary vulvar lesion. The lesion presented as a pink, vegetative nodule of the clitoral hood which grew in size over several years and was confirmed to be melanoma on shave biopsy. This patient's amelanotic presentation in conjunction with the rare incidence of vulvar melanoma contributed to the delay in diagnosis. This case exemplifies the challenge of early recognition of potentially malignant vulvar lesions for primary care providers in adolescents.

We hereby describe two cases of patients with ocular sporotrichosis who presented with different spectrums of clinical manifestations. Both patients had an antecedent history of zoonotic and vegetative contact. The first patient presented with acute granulomatous anterior uveitis, and the second patient presented with granulomatous conjunctivitis. Patients received topical antibiotics, steroids, and cycloplegics. Systemic oral antifungals were added until full recovery was achieved. Both cases were treated without any episodes of relapse or recurrence.

Aortic valve abscess is a fatal complication of infective endocarditis. Transthoracic echocardiography is the initial imaging obtained in suspected infective endocarditis. However, its accuracy in detecting cardiac complications remains low, thus should be followed by transesophageal echocardiography if the clinical situation permits. Here, we present a case of a bicuspid aortic valve infective endocarditis caused by Streptococcus agalactiae and complicated with aortic valve abscess and acquired Gerbode defect, which appeared as a tricuspid valve vegetation on transthoracic echocardiography.

Vacuum-assisted thrombectomy (VAT) is a mechanical suction device that can be deployed to aspirate thrombi in the heart and vascular system. A young female complained left ear fullness for over three months. She has a history of tympanocentesis one year before. Nasal endoscopic inspection reported that left nose pharynx ministry is derived from the vegetations of eustachian tube. Pure tone audiometry found that the left ear took conductive deafness. We could see the reteacerd tympanic membrane with eardrum effusion by using of endotoscpe. Finally the nasal endoscopic surgery confirmed the excision of lesion of pharyngeal opening auditory tube.

An unoperated 45-year-old man with double-inlet single left ventricle, transposed great arteries, ventricular septal defect, and pulmonary stenosis presented with mitral and pulmonary valvular vegetations unresponsive to antibiotic therapy. The initial diagnosis was made with echocardiography, and the morphology was delineated with computed tomography and magnetic resonance imaging. The patient underwent mitral valve replacement with a mechanical valve and pulmonary valve vegetectomy. He recovered well and he is currently well two years after the operation.

A protective antigen (PA) based coagglutination test was optimized in the present study for the specific and sensitive identification of bacteria causing anthrax in a cost effective and less risky manner. The test showed 100% specificity and sensitivity up to 9 × 10(3) formalinized vegetative cells or 11 ng of PA. The optimized test also detected anthrax toxin directly from the serum as well as blood of anthrax infected animals indicating the potential application for direct diagnosis of anthrax under field conditions.

Cutaneous infections due to Listeria monocytogenes are rare. Typically, infections manifest as nonpainful, nonpruritic, self-limited, localized, papulopustular or vesiculopustular eruptions in healthy persons. Most cases follow direct inoculation of the skin in veterinarians or farmers who have exposure to animal products of conception. Less commonly, skin lesions may arise from hematogenous dissemination in compromised hosts with invasive disease. Here, we report the first case in a gardener that occurred following exposure to soil and vegetation.
