BACKGROUND: Mastocytosis is a clonal mast cell disorder (CMD) characterized by the proliferation and accumulation of mast cells (MC) in different tissues. Anaphylaxis, resulting from massive MC activation and mediators' release, is reported in 22%-49% of mastocytosis cases, with drugs being potential triggers. Proper counseling on drug safety is crucial. We aim to demonstrate that, after a careful evaluation of clinical history and allergic work-up, drug provocation tests (DPT) are a safe and effective diagnostic tool in patients with CMD.

We describe the case of a 58-year-old patient who developed chest pain and an anaphylaktoide reaction after ingestion of contamined fish containing histamin. Histamin intoxication from food poisoning (also known as scombroid intoxication) can be mistaken for an anaphylactic reaction and occasionaly lead to cardiac symptoms, even in patients without atherosclerotic changes. This condition is called Kounis syndrom and has to be recognized as a separate syndrom with specific clinical features.

This is an update on the treatment of anaphylactic shock in pregnant women. Most guidelines recommend adrenaline as first-line treatment in all patients, but in pregnant patients some recommend ephedrine due to a potential risk of utero-placental hypoperfusion with adrenaline. However, ephedrine is a less potent vasopressor than adrenaline and does not possess the same anti-inflammatory or broncho-dilating effects. As immediate and correct treatment is essential for both the maternal and the fetal outcome, adrenaline should be first-line treatment also in pregnant women.
