The AO unreamed nail: friend or foe.

Twenty-nine tibial shaft fractures were stabilized with the AO unreamed tibial nail. These included 13 type A fractures, 11 type B fractures and five type C injuries. Fourteen of the fractures were open. The AO unreamed tibial nail was the primary device used in all cases. Our series suggests that this is an easy device to insert and that its multiple locking options confer an advantage for the management of distal fractures. It also has a low soft-tissue complication rate. There was a delay in full patient mobilization and in the progression to fracture union. There was also a significant deg

Rush rod fixation of supracondylar fractures above total knee arthroplasties.

The authors report on 22 displaced supracondylar femur fractures and total knee arthroplasties treated with Rush rod (Berivon, Meridian, MI) fixation. The average age at the time of total knee arthroplasty was 67 years. Fractures occurred at an average of 5 years after surgery. In only five cases was notch in the anterior cortex noted. The follow-up period averaged 7 years. There were no nonunions. All knees healed between 3 and 4 months after surgery. Postoperative flexion averaged 108 degrees (preoperative average, 102 degrees). Only two knees demonstrated a flexion contracture before surger

The use of the unreamed nail in tibial fractures with concomitant preoperative or intraoperative elevated compartment pressure or compartment syndrome.

Twelve patients with tibial shaft fractures and evidence of compartment syndrome or with documented elevated compartment pressures were treated with an unreamed locked intramedullary nail and a single-incision lateral four-compartment fasciotomy. There were six closed fractures and three grade I and three grade II open fractures. Ten fractures have achieved a solid union without shortening or significant angulation at an average follow-up of 8.1 months (range 4-26). Two patients were lost to follow-up. There were two delayed unions and one nonunion, all of which healed after additional treatme

[Tibio-calcaneus fusion].

From January 1977 to August 1992 22 tibiocalcaneal fusions were performed at the Co-Operative Trade Association Hospital for Accidents, at Tübingen, Germany. 21 of these fusions healed both clinically and roentgenologically, the quality of the effected healing being stable. In more than two-thirds of the cases the indication for performing the fusion had been a fracture of the talus of the Weber/Marti IV or Hawkins III type with its associated complications. Hence, bony fusion between tibia and calcaneus is a commendably reliable treatment possibility in severe diseases of the talus and its ad

Seidel intramedullary nailing of humeral diaphyseal fractures: a preliminary report.

Forty-two humeral diaphyseal fractures in 41 patients were treated at three centers between April 1988 and November 1989. There were 28 acute fractures; four were open. Average time to union was 8 weeks. There were no infections. Six patients with seven pathologic fractures due to metastatic disease died during the course of this study, but the nail had allowed them to be functional with minimal surgical dissection. Five of six nonunions united with one procedure. There was one residual nonunion in a patient with a wide canal and an arthrodesed shoulder above the nonunion. There were three pre

Crossed pins vs parallel pins in the treatment of femoral neck fractures.

Twenty femoral neck fractures treated with three crossed modified Knowles pins (Gouffon pins) were compared to 20 fractures treated with three parallel pins. All patients had survived for more than 2 years after operation. The groups were comparable concerning Garden stage, reduction, and age. No significant difference concerning the failure rate was found.

Treatment of open fractures of the tibial shaft: Ender nailing versus external fixation. A randomized, prospective comparison.

A randomized, prospective study comparing Ender nailing with external fixation for open fractures of sixty-three tibiae (sixty patients) was undertaken. Ender nailing proved to be at least as effective as external fixation with respect to seven parameters: time to union, tibial alignment, total number of operations, ranges of motion of the knee and ankle, pain, presence of infection, and complications. Ender nailing is a safe alternative to external fixation for grade-I and grade-II open fractures. It should not be used for comminuted fractures, which might shorten about the nails.

Closed intramedullary fixation of humeral shaft fractures.

During a seven-year prospective study, 158 humeral shaft fractures in 157 patients were treated with closed intramedullary nailing. Only humeral fractures not responsive to closed reduction and immobilization and those in noncomplaint patients were eligible. No immobilization was used postoperatively. Seven patients were lost to follow-up. A total of 148 fractures healed; there were three nonunions. The average time to clinical union was 7.2 weeks. There were no infections or malunions. Eight of 11 preoperative and three postoperative radial nerve palsies were lesions in continuity and healed

Zickel supracondylar nails for fractures of the distal end of the femur.

Between 1974 and 1984 the Zickel supracondylar nail system was used to fix 82 femora. Sixty-nine femora had supracondylar fractures and 13 supracondylar osteotomies. A high rate of union (98%) was achieved in the fracture group, which included a variety of fracture types, but five supracondylar osteotomies failed to unite initially and required additional surgery. Knee motion exceeded 90 degrees in 70% of the femora that healed but was poorest in the T or Y intracondylar fracture group, in which only 46% exceeded 90 degrees. The operation may be performed as either open or closed nailing. Clos

Replacement of the femoral head by open operation in severe adolescent slipping of the upper femoral epiphysis.

The stages in adolescent slipping of the upper femoral epiphysis are classified in relation to treatment. The operation of open replacement of the displaced femoral head is described, and the results of a personal series of seventy-three such operations are presented. Open replacement is excellent treatment for severe chronic slipping so long as the growth plate is still open. The greater incidence of avascular necrosis in acute-on-chronic cases is probably due to damage to the blood supply of the head at the time of the acute slip or kinking of the vessels before replacement. Prolonged tracti

Real-world prevalence of adverse events with first-line systemic therapies among patients with metastatic castration-sensitive prostate cancer.

BACKGROUND: The current guidelines for treating metastatic castration-sensitive prostate cancer (mCSPC) recommend treatment intensification of androgen deprivation therapy (ADT) with the addition of an androgen receptor pathway inhibitor (ARPI), with or without docetaxel. However, the adoption of these treatment options has been slow, leading to therapeutic inertia. This real-world study was conducted to investigate the occurrence of adverse events (AEs) among treated patients diagnosed with mCSPC in the United States. METHODS: This retrospective claim review estimated the occurrence of AEs a

Comparison of intramedullary and extramedullary fixation of stable intertrochanteric fractures in the elderly: a prospective randomised controlled trial exploring hidden perioperative blood loss.

BACKGROUND: Hip fracture is a severe and common injury that occurs predominantly in the elderly. Blood loss in the perioperative period is associated with a greater risk of dying in anaemic patients. The aim of the study was to explore the best way to treat stable intertrochanteric fractures, taking hidden blood loss into account. METHODS: This prospective, randomised blinded study included patients aged over 65 years with stable intertrochanteric fractures (Evans grades I and II). The patients were allocated to one of two groups treated via extramedullary or intramedullary fixation. Patient

Immobilization contributes to exaggerated neuropeptide signaling, inflammatory changes, and nociceptive sensitization after fracture in rats.

UNLABELLED: A tibia fracture cast immobilized for 4 weeks can induce exaggerated substance P and calcitonin gene-related peptide signaling and neuropeptide-dependent nociceptive and inflammatory changes in the hind limbs of rats similar to those seen in complex regional pain syndrome (CRPS). Four weeks of hind limb cast immobilization can also induce nociceptive and vascular changes resembling CRPS. To test our hypothesis that immobilization alone could cause exaggerated neuropeptide signaling and inflammatory changes, we tested 5 cohorts of rats: 1) controls; 2) tibia fracture and hind limb c

Standardised cement augmentation of the PFNA using a perforated blade: A new technique and preliminary clinical results. A prospective multicentre trial.

Pertrochanteric fractures are a rising major health-care problem in the elderly and their operative stabilisation techniques are still under discussion. Furthermore, complications like cut-out are reported to be high and implant failure often is associated with poor bone quality. The PFNA(®) with perforated blade offers a possibility for standardised cement augmentation using a polymethylmethacrylate (PMMA) cement which is injected through the perforated blade to enlarge the load-bearing surface and to diminish the stresses on the trabecular bone. The current prospective multicentre study was

Tibiotalocalcaneal arthrodesis for salvage of severe ankle degeneration.

Tibiotalocalcaneal arthrodesis is a successful and proven surgical procedure for patients who have significant arthritic changes, deformity, and failed previous operations. Surgical technique varies depending on the type of fixation. Basic surgical principles should not be violated. Correction of the deformity with appropriate joint preparation and stable fixation is important for a good outcome. Other adjunctive materials, such as bone growth stimulators and orthobiologics, should be used appropriately to ensure adequate primary arthrodesis.

Dyskeratosis congenita is a chromosomal instability disorder.

Dyskeratosis congenita (DC) is a rare inherited disorder characterized by dystrophic changes in the skin and mucous membranes, bone marrow failure, and a predisposition to malignancy. In the majority of families the pattern of inheritance of DC has been compatible with X-linkage, the most likely location being Xq28. The primary defect responsible for this disease remains unknown. As DC shares many features (congenital abnormalities, bone marrow failure) with the chromosomal instability disorder. Fanconi's anaemia (FA), several studies have focused on cytogenetic features in DC. Unlike in FA, c

Unstable distal radius fractures treated by modified Kirschner wire pinning: anatomic considerations, technique, and results.

A modified technique of percutaneous Kirschner-wire pinning for unstable distal radius fractures is presented. Three to four pins are placed from the radial styloid process toward and through the dorsal, volar, and ulnar cortices of the proximal fracture fragment, respectively. Rotational stability is achieved by bending and counterblowing of the wire ends in the styloid process. Anatomic studies of cadaver wrists revealed possible lesions of the superficial radial nerve, the cephalic vein, and the tendons of the brachioradialis muscle or the thumb and wrist extensors, respectively. Neverthele

A biomechanical comparison of intramedullary nailing systems for the humerus.

This study evaluated the rotational and bending stability of three interlocking nail systems in paired cadaveric humeri. The Russell-Taylor Humeral Interlocking Nail, the Seidel Humeral Locking Nail, and the True-Flex Humeral Nail were mechanically tested in torsion and four-point bending. The Russell-Taylor and the Seidel interlocking nails are reamed systems that rely on proximal interlocking screws and distal screws or phalanges respectively for rotational stability. However, axillary nerve damage may result during proximal screw placement, and these systems exhibit low resistance to rotati

External Fixation and Adjuvant Pins Versus Volar Locking Plates in Unstable Distal Radius Fractures: A Randomized Controlled Study with a 10-Year Follow-Up.

PURPOSE: To investigate if volar locking plates (VLPs) are superior to external fixation (EF) with adjuvant pins in unstable distal radius fractures after 10 years of follow-up. METHODS: From 2007 to 2009, 111 unstable distal radius fractures were randomized to either EF or VLP. The patients' mean age was 54 years (20-84). At 10 years, 86 patients (77%) were assessed (VLP 43, EF 43). Eighty-one patients (VLP 42, EF 39) had a clinical and radiological examination. Five patients (EF 4, VLP 1) refused to attend the examination, but returned the Disabilities of the Arm, Shoulder, and Hand (QuickD
