Association of Sleepwalking and REM Sleep Behavior Disorder With Parkinson Disease in Men.

IMPORTANCE: Previous studies conducted among patients with Parkinson disease (PD) reported that parasomnias other than rapid eye movement (REM) sleep behavior disorder (RBD), particularly sleepwalking (SW), are associated with PD severity. However, it remains unclear whether the presence of SW is associated with altered odds of having PD in a population-based study. OBJECTIVE: To evaluate whether probable SW, either alone or co-occurring with probable RBD, is associated with higher odds of PD in men. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study included 25 694 men from the H

Bias in case-crossover studies of medications due to persistent use: A simulation study.

PURPOSE: The case-crossover design is increasingly used to evaluate the effects of chronic medications; however, as traditionally implemented in pharmacoepidemiology, with referent period preceding the outcome, it may lead to bias in the presence of persistent exposures. We aimed to evaluate the extent and magnitude of bias in case-crossover analyses of chronic and persistent exposures, using simulations. METHODS: We simulated cohorts with either 30-day, 180-day, or 2-year exposure duration; and with varying degrees of persistence (10%, 30%, 50%, 70%, or 90% of patients not stopping exposure)

Optimising the case-crossover design for use in shared exposure settings.

With a case-crossover design, a case's exposure during a risk period is compared to the case's exposures at referent periods. The selection of referents for this self-controlled design is determined by the referent selection strategy (RSS). Previous research mainly focused on systematic bias associated with the RSS. We additionally focused on how RSS determines the number of referents per risk, sensitivity to overdispersion and time-varying confounding. We illustrated the consequences of different RSS using a simulation study informed by data on meteorological variables and Legionnaires' diseas

In ineffective esophageal motility, failed swallows are more functionally relevant than weak swallows.

BACKGROUND: Esophageal pressure topography (EPT) diagnosis of ineffective esophageal motility (IEM) can be non-specific with unclear clinical significance. AIMS: To determine whether peristaltic vigor or lower esophageal sphincter (LES) integrity is associated with poor clearance and acid reflux in IEM. METHODS: Bolus clearance on high-resolution impedance manometry (HRIM) and available reflux studies in patients with IEM were retrospectively reviewed. Bolus clearance was assessed using both line tracing and colored contour methods on HRIM. EPT parameters, bolus clearance, and acid reflux va

Executive function changes before memory in preclinical Alzheimer's pathology: a prospective, cross-sectional, case control study.

BACKGROUND: Early treatment of Alzheimer's disease may reduce its devastating effects. By focusing research on asymptomatic individuals with Alzheimer's disease pathology (the preclinical stage), earlier indicators of disease may be discovered. Decreasing cerebrospinal fluid beta-amyloid42 is the first indicator of preclinical disorder, but it is not known which pathology causes the first clinical effects. Our hypothesis is that neuropsychological changes within the normal range will help to predict preclinical disease and locate early pathology. METHODS AND FINDINGS: We recruited adults with

Acupressure for chemotherapy-associated nausea and vomiting in children.

The objective of this study was to determine the feasibility and effectiveness of acupressure therapy in preventing chemotherapy-associated nausea in children. A prospective, randomized, crossover trial was conducted among pediatric oncology patients at Brenner Children's Hospital (Winston-Salem, NC). Patients were randomized to one of two treatment sequences involving acupressure wrist bands and placebo bands, separated by a standard care treatment with no bands. All patients received standard antiemetic therapy for each treatment. Expectations and outcomes of nausea and vomiting were assesse

A prospective, single-blind, randomized crossover trial of sacral vs pudendal nerve stimulation for interstitial cystitis.

OBJECTIVE: To compare sacral nerve stimulation (SNS) with pudendal nerve stimulation (PNS) for interstitial cystitis (IC). PATIENTS AND METHODS: Twenty-two patients with well-documented, refractory IC had a tined lead placed at S3 and a second electrode implanted at the pudendal nerve via a posterior approach. In a blinded, randomized design, each lead was tested for 7 days. The best lead was implanted to a pulse generator and patients were followed at 1, 3 and 6 months. RESULTS: The time required to place a sacral lead was 27.4 min, and a pudendal lead 19.6 min (P = 0.039). Of the 22 patien

Positional therapy for obstructive sleep apnoea.

BACKGROUND: The modalities of therapy for obstructive sleep apnoea (OSA) include behavioural and lifestyle modifications, positional therapy, oral appliances, surgery and continuous positive airway pressure therapy (CPAP). Though CPAP has proven efficacy in treating OSA, adherence with CPAP therapy is suboptimal. Positional therapy (to keep people sleeping on their side) is less invasive and therefore expected to have better adherence. This review considered the efficacy of positional therapy compared to CPAP as well as positional therapy against no positional therapy. Devices designed for pos

Reconstruction of a cross-family chitinase cocktail for efficient chitobiose production guided by the secretome of Streptomyces sp. F-3.

The enzymatic conversion of chitin to chitobiose is of significant interest due to its broad biological applications. However, the limited functional diversity and insufficient synergism of chitinase systems constrain catalytic efficiency and industrial applicability. In this study, we rationally reconstructed a cross-family chitinase cocktail by elucidating the synergistic mechanisms between GH18 and GH19 enzymes from Streptomyces sp. F-3. The three GH18 chitinases and one GH19 chitinase displayed complementary substrate specificity toward insoluble polysaccharides and oligosaccharides, sugge

Multimodal gene prioritization reveals nonlinear regulatory architecture in childhood-onset asthma.

UNLABELLED: Asthma is a heritable complex disease that disproportionately burdens minority and admixed populations in the US. However, the causal genes and regulatory mechanisms governing inherited risk remain largely unresolved. We performed a European-ancestry meta-analysis of 141,894 cases and 1,361,846 controls drawn from the Trans-national Asthma Genetic Consortium (TAGC) and Global Biobank Meta-analysis Initiative (GBMI), yielding an estimated h 2 SNP of 0.056 (SE = 0.0038) and 275 independently associated loci. To enhance mechanistic inference beyond variant-level associations, we devel

Evaluation of factors influencing the reduction of home mechanical ventilation dependency in patients planned to receive home health care.

BACKGROUND: With increasing life expectancy and the growing burden of chronic diseases, healthcare systems are shifting from hospital-centered models to community- and home-based care. The use of home mechanical ventilators (HMVs) facilitates early discharge from hospital settings yet creates significant physical, psychological, and social burdens for caregivers. Identifying strategies to reduce ventilator dependency during palliative inpatient care may alleviate these challenges. METHODS: This retrospective, cross-sectional, descriptive study employed a quantitative approach with purposive s
