Topical Calcineurin Inhibitors in Rosacea: Potential Benefit in Selected Clinical Settings and Risk of Rosaceiform Dermatitis.

Rosacea is a chronic inflammatory skin disorder characterized by persistent erythema, flushing, papules, pustules, and telangiectasia, predominantly affecting the central face. Topical calcineurin inhibitors (TCIs), including tacrolimus and pimecrolimus, are approved for atopic dermatitis(AD) but have also been used off-label in rosacea and other facial inflammatory dermatoses. Evidence for their use in rosacea is limited and mixed. Some small uncontrolled studies have reported clinical improvement, particularly with pimecrolimus. In contrast, rosaceiform dermatitis has been reported during tr

Effect of topical application of lipopolysaccharide on contact hypersensitivity.

Lipopolysaccharide (LPS) is the principal component of the outer membrane of gram-negative bacteria. The prior oral administration of LPS attenuates inflammatory responses, such as intestinal injury and atopic dermatitis, in mouse models; however, the underlying mechanism remains unclear. Here, we examined the effect of topical LPS application on allergic contact dermatitis and its mechanism of action using a murine contact hypersensitivity (CHS) model. Prolonged LPS application to the skin significantly suppressed 2,4-dinitrofluorobenzene (DNFB)-induced CHS. LPS application to the skin also r

Allergic contact dermatitis in children and proposal for an Australian Paediatric Baseline Series.

BACKGROUND: Allergic contact dermatitis (ACD) is an increasingly common diagnosis in children. The objectives of this study were to review our experience with ACD in children in tertiary settings, to ascertain the spectrum of allergens in this population and to subsequently propose the first Australian Paediatric Baseline Series for patch testing. METHODS: We conducted a retrospective analysis of patch test data from 1993 to 2017 from two tertiary referral patch-testing centres in Melbourne, Victoria. RESULTS: A total of 511 children were patch tested during the study period. Of these, 58.3%

Lymphatic Function Regulates Contact Hypersensitivity Dermatitis in Obesity.

Obesity is a major risk factor for inflammatory dermatologic diseases, including atopic dermatitis and psoriasis. In addition, recent studies have shown that obesity impairs lymphatic function. As the lymphatic system is a critical regulator of inflammatory reactions, we tested the hypothesis that obesity-induced lymphatic dysfunction is a key regulator of cutaneous hypersensitivity reactions in obese mice. We found that obese mice have impaired lymphatic function, characterized by leaky capillary lymphatics and decreased collecting vessel pumping capacity. In addition, obese mice displayed he

Prevalence of methylchloroisothiazolinone and methylisothiazolinone contact allergy in facial dermatitis: a single centre Irish study.

BACKGROUND: There has been an increase in methylchloroisothiazolinone (MCI) and especially methylisothiazolinone (MI) contact allergy in recent years. OBJECTIVES: We examined the prevalence and demography of MCI/MI and MI contact allergy in patients presenting with facial dermatitis. METHODS: Eighty patients with facial dermatitis were patch tested to the British Society for Cutaneous Allergy standard series, preservatives and own products between January 2012 and March 2014. RESULTS: A total of 14 (17.5%) patients had positive reactions to this preservative. Five (6.3%) patients tested pos

Atopic diseases in pediatric population: prematurity and small for gestational age.

BACKGROUND: Atopic diseases, including asthma, atopic dermatitis (AD), allergic rhinitis (AR), and food allergy, are significant chronic conditions in the pediatric population. Prematurity and small-for-gestational-age (SGA) status are critical factors influencing long-term health outcomes. This study investigated the associations between prematurity, SGA, and the development of atopic diseases in children using a nationwide longitudinal cohort. METHODS: We analyzed data from Taiwan's National Health Insurance Research Database (NHIRD), which includes nearly all residents. The cohort comprise

Intraosseous epidermoid cyst in the digit of an atopic dog with chronic pododermatitis: case report.

Epidermoid cysts are non-neoplastic lesions characterized by a cavity lined by stratified squamous keratinized epithelium and filled with lamellar keratin. While dermal epidermoid cysts are commonly observed in dogs, the intraosseous form is rarely reported. These cysts must be differentiated from other digital lesions involving bone, particularly malignant tumors, as the latter often require more extensive surgical intervention for definitive management. This report describes a case of an intraosseous epidermoid cyst in a dog diagnosed with atopic dermatitis and chronic pododermatitis.

Racial and Ethnic Health Disparities in Dermatology.

Health disparities are differences in health or disease incidence, prevalence, severity, or disease burden that are experienced by disadvantaged populations. Their root causes are attributed in large part to socially determined factors, including educational level of attainment, socioeconomic status, and physical and social environments. There is an expanding body of evidence documenting differences in dermatologic health status among underserved populations. In this review, the authors highlight inequities in outcomes across 5 dermatologic conditions, including psoriasis, acne, cutaneous mela

Non-psoriatic uses of calcipotriol: a concise updated review.

Calcipotriol (calcipotriene) is a synthetic vitamin D3 derivative that is a standard treatment option for psoriasis. It is generally well tolerated with minimal side effects. Due to its ability to reduce keratinocyte proliferation and induce keratinocyte differentiation as well as its immunomodulatory effects, calcipotriol has been used to treat a variety of skin disorders such as atopic dermatitis, actinic keratoses, lichen planus, seborrheic keratoses, and vitiligo [1]. We surveyed the literature examining the use of calcipotriol for non-psoriatic dermatologic disease.

The role of innate lymphoid cells in healthy and inflamed skin.

The skin constitutes the interface between the organism and the environment, and it protects the body from harmful substances in the environment via physical, chemical and immunological barriers. The immunological barrier of the skin comprises both cells from the innate and the adaptive immune system. During the last years, it has become clear that innate lymphoid cells play a role in homeostasis and inflammation of the skin in humans and mice. In this review, we will discuss the role of innate lymphoid cells in healthy and inflamed skin with special focus on their role in atopic dermatitis.

The use of cyclosporine in dermatology.

Cyclosporine is an immunosuppressive drug that acts selectively on T-cells by inhibiting calcineurin phosphorylase. It has been used in dermatology since its approval for US Food and Drug Administration in 1997 for the use in psoriasis. While indicated only for the treatment of moderate to severe psoriasis, cyclosporine has also been used as an off-label drug for the treatment of various inflammatory skin conditions, including atopic dermatitis, blistering disorders, and connective tissue diseases. In this article, we review the use of cyclosporine in dermatology.

Condyloma acuminatum in a 3-year old girl.

We report the case of 3-year-old girl with condyloma acuminatum. She was brought to our clinic with pruritus, painful urination and four discharge and treated with electrocautery. Histological examination revealed acanthosis, nuclear basophilic inclusions and vacuolar degeneration. In this case, transmission of the human papilloma virus might have occurred during close, non-sexual contact with the infected mother, and the depressed immune response associated with atopic dermatitis appears to have played an important role in the development of the condyloma acuminatum infection.

Skin Manifestations and Coeliac Disease in Paediatric Population.

Celiac disease (CD) is an immune-mediated enteropathy caused by gluten ingestion, affecting approximately 1% of the worldwide population. Extraintestinal symptoms may be present as the first signs of CD, years before the CD diagnosis is made. A great variety of extraintestinal manifestations may be associated with CD. Cutaneous manifestations represent the main extraintestinal manifestations, with dermatitis herpetiformis being the most common in patients with CD. In adults, it has been demonstrated that the role of a gluten-free diet is crucial not only for the recovery of signs and symptoms

Diagnostic procedures for eczema patients.

Of 7887 patients with eczema seen in a private dermatological practice over a period of 2 years and 4 months, a specific final diagnosis was found in 5376 (68%), while a final diagnosis of non-specific eczema was made in 2511 cases (32%). The most common final diagnoses were seborrhoeic dermatitis (1351), contact dermatitis (1317), atopic dermatitis (1009) and nummular eczema (822). Battery patch testing was the initial diagnostic procedure performed, and 897 of the 3164 patients who were patch tested had one or more positive patch tests. In 545 cases, the reactions were considered of relevanc

Pneumocystis jirovecii pneumonia in a human caused by long-term use of veterinary drug oclacitinib: A case report.

Pneumocystis jirovecii pneumonia (PJP), an opportunistic fungal infection, typically occurs in immunocompromised patients, such as those with human immunodeficiency virus (HIV) infection or those receiving prolonged immunosuppressive therapy. Recently, PJP in non-HIV patients treated with novel immunomodulatory agents, including Janus kinase (JAK) inhibitors, has been increasingly reported. Here, we report a case of PJP in a 53-year-old HIV-negative Japanese man with no recognized immunosuppressive comorbidities. The patient, a veterinarian, had been self-administering oclacitinib (16-64 mg/da

Clinical Benefits of Basic Emollient Therapy for the Management of Patients With Xerosis Cutis.

Xerosis cutis refers to dry skin that may be caused or exacerbated by external triggers (e.g., cold weather), endogenous factors (e.g., aging), or present as a symptom of other diseases (e.g., atopic dermatitis). Emollients are topical products formulated to rehydrate and restore the barrier function of the skin and are consistently recommended for the management of xerosis cutis and related diseases. Following the previous articles in this supplement, the present article aims to review the wider spectrum of clinical benefits associated with emollient therapy for xerosis cutis and diseases ass

The regulatory T cell-selective interleukin-2 receptor agonist rezpegaldesleukin in the treatment of inflammatory skin diseases: two randomized, double-blind, placebo-controlled phase 1b trials.

Regulatory T cell (Treg) impairment is implicated in the pathogenesis of chronic inflammatory diseases, but relatively little is known about the therapeutic potential of Treg restoration. Here we present clinical evidence for the Treg-selective interleukin-2 receptor agonist rezpegaldesleukin (REZPEG) in two randomized, double-blind, placebo-controlled Phase 1b trials in patients with moderate-to-severe atopic dermatitis (AD) (NCT04081350) or chronic plaque psoriasis (PsO) (NCT04119557). Key inclusion criteria for AD included an Eczema Area and Severity Index (EASI) score ≥ 16 and a validated
