BACKGROUND: Arm wrestling has recently become one of the most popular sports among young people, mainly due to its simplicity and spectacularity. Yet, unfortunately it is also injury prone. The aim of the study was to perform a biomechanical analysis of the forces which act during arm wrestling, as well as to explain the mechanism of the occurrence of humeral fractures of a similar topology as observed on X-rays.
MATERIAL/METHODS: During the period 2001 to 2008 nine cases of humeral fractures resulting from arm wrestling were consulted and treated at the Clinic. The assessment of the limb con

Nitric oxide (NO) is a small free radical that is generated by a family of enzymes called the nitric oxide synthases (NOS). There are 3 isoforms of NOS: endothelial NOS (eNOS), brain or neuronal NOS (bNOS), and inducible NOS (iNOS). In experiments performed during the last 20 years, we have shown that NO is induced by all 3 isoforms of NOS after tendon injury and that NOS activity is upregulated in tendinopathy. In normal uninjured tendons, there is very little NOS activity. In injured rat and human tendons, NOS activity was found in healing fibroblasts in a temporal fashion. In animal models,

CASE HISTORY: An 8-week-old, 16-kg alpaca cria was presented with a 2-week history of acute onset of left forelimb lameness.
CLINICAL FINDINGS: The cria was weight-bearing but lame at the walk in the left forelimb. There was swelling, heat and pain on palpation of the soft tissues, extending from the mid-shaft of the humerus to the elbow. There was no evidence of a traumatic injury. Radiologically, there was evidence of osteomyelitis and a sequestrum involving the distal metaphysis of the humerus. The sequestrum was removed surgically. No bacteria were cultured from samples taken during surge

INTRODUCTION: A Monteggia lesion is a dislocation of the radial head associated with a fracture with the proximal third of the ulna. It is rare in children and the dislocation of the radial head is often missed at the time of injury. There are a lot of described treatment methods: open reduction of the radial head and reconstruction of the annular ligament combined with ulnar osteotomy, the same method without reconstruction of the annular ligament, gradual lengthening and angulation of the ulna by Ilizarov method without the opening of radiocapitelar joint.

Manual wheelchair propulsion has been linked to a high incidence of overuse injury and pain in the upper extremity, which may be caused by the high load requirements and low mechanical efficiency of the task. Previous studies have suggested that poor mechanical efficiency may be due to a low effective handrim force (i.e. applied force that is not directed tangential to the handrim). As a result, studies attempting to reduce upper extremity demand have used various measures of force effectiveness (e.g., fraction effective force, FEF) as a guide for modifying propulsion technique, developing reh

This is a prospective study of 107 repairs of obstetric brachial plexus palsy carried out between January 1990 and December 1999. The results in 100 children are presented. In partial lesions operation was advised when paralysis of abduction of the shoulder and of flexion of the elbow persisted after the age of three months and neurophysiological investigations predicted a poor prognosis. Operation was carried out earlier at about two months in complete lesions showing no sign of clinical recovery and with unfavourable neurophysiological investigations. Twelve children presented at the age of

This study explores the effects of functional electrical stimulation (FES) of the lumbar trunk extensors on the seated posture and bimanual workspace of subjects with spinal cord injury (SCI). Four subjects with motor complete SCI with implanted intramuscular stimulating electrodes to activate the lumbar erector spinae were studied. The positions of markers on the pelvis, trunk, and hands were monitored by a motion capture system during bimanual reaching maneuvers. To define three-dimensional functional workspace boundaries, subjects swept their hands through the extremes of their range of mot

OBJECTIVE: To evaluate whether ulnar neuropathy could induce changes in spinal cord and motor cortex excitability and therefore predispose to development of focal dystonia.
BACKGROUND: A high incidence of ulnar neuropathy has been observed in patients with musician's cramp. Polygraphic electromyograph recordings in patients with entrapment of the ulnar nerve at the elbow have demonstrated long-duration bursts of co-contraction in antagonistic muscles, similar to those observed in focal dystonia.
METHODS: All control subjects and 12 patients with ulnar neuropathy underwent an electrophysiolog

The purpose of this study was to show that independence of self care activities of motor complete quadriplegic patients related better to the force of contraction of the wrist extensor (WE) muscles measured by a hand held myometer than to the manual muscle test (MMT) for grades 3 to 5. The patients (n = 24), identified by having unilaterally or bilaterally functional WE muscle (MMT > or = 3/5) with bilateral elbow extensor (EE) muscles < 3/5, were evaluated at 2 to 6 months after injury for four feeding activities utilizing the Quadriplegia Index of Function (QIF). We found 54% were independen

BACKGROUND: Osteochondral autologous transplantation surgery (OATS) has been advocated for unstable osetochondritis dissecans (OCD) lesions of the adolescent capitellum, though limited information is available regarding clinical and radiographic results in North American patients. We hypothesize that single-plug OATS is safe and effective in alleviating pain and restoring function in unstable OCD.
METHODS: Twenty-eight patients with unstable OCD treated with single-plug OATS were evaluated. Mean age at surgery was 14.2 years; there were 14 males. Etiology of OCD was presumed to be sports part

PURPOSE: The posterior interosseous nerve (PIN) is at risk during the posterior and lateral approaches to the proximal radius. We aimed to define a safe zone for these approaches to avoid injury of the PIN and to evaluate their close and changing relationship to the nerve during forearm rotation.
METHODS: The study collective consisted of 50 upper limbs. After performance of the lateral approach, the distance between the tip of the radial head and the PIN's exit point from the supinator (= distance 1) and the shortest interval between the nerve's exit to the radial margin of the ulna (= dista

PURPOSE: We hypothesized that health of the donor nerve and corresponding muscle, as assessed by electromyography (EMG), could predict the outcome of nerve transfer surgery.
METHODS: A retrospective review was performed to investigate outcomes of nerve transfers for elbow flexion and shoulder abduction. Motor strength was graded preoperatively and after a minimum 1-year follow-up. Preoperative EMG results were classified as functionally normal or affected based on motor unit recruitment pattern and correlated with follow-up motor strength and range of motion.
RESULTS: Forty nerve transfers w

BACKGROUND: In patients managed with plate fixation for the treatment of an olecranon fracture, the placement of an axial intramedullary screw may obstruct the placement of bicortical screws in the ulnar shaft. To overcome this problem, unicortical screws can be applied with use of a contoured locking compression plate. The present study was designed to assess the effectiveness of this fixation method.
METHODS: Nineteen consecutive patients with an acute comminuted olecranon fracture were managed with a contoured locking compression plate and intramedullary screw fixation. Sixteen patients we
