Invasive Fusarium oxysporum infection mimicking recalcitrant bacterial cellulitis in a diabetic foot ulcer.

Diabetic foot ulcers are predominantly attributed to bacterial aetiologies, but invasive fungal infections are an increasingly recognised opportunistic threat in immunocompromised patients. We present the case of a male farmer in his 30s with uncontrolled diabetes who developed a worsening necrotic ulcer on the left tibia following agricultural trauma. Despite treatment with broad-spectrum antibiotics, the wound deteriorated, exhibiting a central black eschar. Deep tissue biopsy revealed hyaline septate hyphae and fungal culture confirmed Fusarium oxysporum, identified by characteristic banana…

Mechanical testing methods for determining stiffness of ankle-foot orthoses: A scoping review.

Stiffness of ankle-foot orthoses (AFOs) has been assessed using many methods; however, results are unstandardized due to varied testing conditions such as range of motion and speed. Therefore, the aim of this scoping review was to provide a state-of-the-space review of the studies that investigate AFO ankle stiffness in the sagittal plane and then provide insight into the details of the developed devices and protocols. Six electronic databases (MEDLINE, SCOPUS, Web of Science, CINAHL, EMBASE, and IEEE Xplore) were searched for studies from when indexing began to April 2024. Articles of any des…

Fungating Hemangioma of the Foot Following Traumatic Laceration Mimicking Sarcoma: A Case Report.

CASE: A 64-year-old man developed a rapidly enlarging, painless exophytic mass on the plantar medial aspect of the right foot following a traumatic laceration. Magnetic resonance imaging revealed a 5.5-cm soft-tissue lesion. Core biopsy revealed a hemangioma with papillary endothelial hyperplasia. The lesion was treated with wide excision followed by acellular dermal matrix reconstruction. At 1-year follow-up, the wound was fully healed with no recurrence. CONCLUSION: This represents the first reported plantar hemangioma with temporal association to trauma. The exact mechanism remains uncerta…

Prevalence of Large Fiber Neuropathy in Nondiabetic Older Adults Seeking Routine Foot Care.

BACKGROUND: Older adults qualifying for routine foot care (RFC) under the Medicare program are often diagnosed with diabetes, peripheral artery disease (PAD), or neuropathy. Specifically, large fiber neuropathy (LFN) has been shown to increase during the aging process, rendering patients more susceptible to unperceived trauma because of loss of protective sensation and an increased fall risk because of balance deficits. This study assessed the prevalence of LFN as diagnosed by the timed vibration test (TVT) in the nondiabetic segment of the Medicare population seeking RFC. A comparison was mad…

SwinDFU-Net: Deep learning transformer network for infection identification in diabetic foot ulcer.

BACKGROUND: The identification of infection in diabetic foot ulcers (DFUs) is challenging due to variability within classes, visual similarity between classes, reduced contrast with healthy skin, and presence of artifacts. Existing studies focus on visual characteristics and tissue classification rather than infection detection, critical for assessing DFUs and predicting amputation risk. OBJECTIVE: To address these challenges, this study proposes a deep learning model using a hybrid CNN and Swin Transformer architecture for infection classification in DFU images. The aim is to leverage end-to…

Ultrasound-Triggered Oxygen Release System for Accelerating Wound Healing of Diabetic Foot Ulcers.

Diabetic foot ulcer (DFU) is a common complication of chronic diabetes mellitus. Oxygen plays a critical role in the healing process of DFU wounds by promoting cell migration and neovascularization. However, clinical hyperbaric oxygen (HBO) therapy predominantly uses systemic oxygen administration, posing challenges in inadequate DFU local oxygen penetration and potential ectopic organs oxygen toxicity. To address these challenges, a strategy to encapsulate oxygen with lipid microbubbles (OMBs) and incorporate them into a body temperature-sensitive heparin-pluronic copolymer hydrogel (HP/OMBs)…

Comparative Efficacy of Regenerative Therapies for Diabetic Foot Ulcers: A Network Meta-analysis.

BACKGROUND: Diabetic foot ulcer (DFU) is one of the most dreaded complications of diabetes mellitus and is associated with a high risk of lower limb amputation and mortality. Because the efficacy of conventional treatments for DFUs is often limited, regenerative medicine therapy, such as growth factors and stem cells, may provide a solution to chronic wound healing. In this study, we compared several kinds of regenerative therapy for treating DFUs in terms of efficacy. METHODS: We systemically searched relevant studies on PubMed, Cochrane Library, and EMBASE. Randomized clinical trials on con…

Analysis of ferroptosis-related key genes and regulatory networks in diabetic foot ulcers.

BACKGROUND: Diabetic foot ulcers (DFUs) is a severe complication of diabetes. Recent evidence suggests that ferroptosis, a form of regulated necrosis, may play a significant role in the progression of DFU. However, the precise molecular mechanisms remain elusive. OBJECTIVE: This study aims to identify ferroptosis-related genes (FRGs) and the signaling pathways involved in DFU progression by analyzing the gene expression profiles of DFU. METHODS: Differentially expressed genes (DEGs) were identified by analyzing gene expression data from two DFU-related datasets (GSE38396, and GSE143735). FRG…

Distinctive Immunological Signatures Define Foot-and-Mouth Disease Virus Persistence in Vaccinated Cattle.

Persistent infection with foot-and-mouth disease virus (FMDV) develops in over 50% of infected ruminants, presenting major obstacles to disease control and eradication. To clarify host immune correlates of FMDV persistence, we characterized systemic T- and B-cell responses, as well as mucosal responses in 15 vaccinated cattle following experimental FMDV challenge. The prevalence of FMDV persistence was 53.3%. While peripheral CD4+, CD8+, and γδ T-cell populations and their respective naïve/memory/effector subpopulations showed comparable frequencies between carriers and noncarriers, carriers e…
