Dislocation and Iatrogenic Dissociation of Bipolar Hip Hemiarthroplasty: A Case Series and Literature Review.

INTRODUCTION: Bipolar hemiarthroplasty (BHA) is widely used for femoral neck fractures, particularly in elderly patients. However, complications such as the rare dissociation of the bipolar cup can severely impact outcomes. This study aims to investigate iatrogenic BHA dissociation following closed reduction, identify risk factors, and discuss preventive strategies. MATERIAL AND METHODS: A retrospective study was conducted on patients who underwent BHA for femoral neck fractures at our institution between 2019 and 2024. Included patients had a minimum post-operative survival of 3 months. Para

Systematic review of radiocarpal dislocations: epidemiology, treatment modalities, and outcomes.

PURPOSE: Radiocarpal dislocation (RCD) is a rare but severe injury, often resulting from high-energy trauma such as motor vehicle accidents and falls. This study aims to synthesize existing case reports and series to provide insights into the epidemiology, associated injuries, and management strategies for RCD. METHODS: A systematic search of PubMed, Scopus, and Web of Science was conducted, including English-language case reports and series up to August 5, 2023. Data extraction followed PRISMA guidelines, with bias assessment using Joanna Briggs Institute criteria. Descriptive statistics and

Bony hip reconstruction for displaced hips in patients with cerebral palsy: Is postoperative immobilization indicated?

PURPOSE: Decisions for postoperative immobilization after bony hip reconstructive surgery in cerebral palsy are controversial in current practice. The aim of this study was to check if choosing not to use any kind of postoperative immobilization is a safe practice. METHODS: A retrospective cohort study was conducted in a pediatric orthopedic tertiary referral center. The study included 148 patients (228 hips) with cerebral palsy, who had bony hip surgery. Medical records were reviewed for incidence of complications, methods of pain control, and length of hospital stay. Three radiographic meas

Pseudarthrosis of Pelvic Fracture With Charcot Arthropathy Successfully Treated With Constrained Total Hip Arthroplasty.

Charcot arthropathy is a rapidly progressive and destructive form of arthropathy caused by various neurological diseases. Total hip arthroplasty (THA) is usually contraindicated in patients with Charcot arthropathy; however, recent studies have reported good results following THA in this patient population. Herein, we report a case of Charcot arthropathy secondary to syphilis in a patient who was successfully treated with constrained THA, a new type of THA. A 56-year-old man was injured in a car accident, and a displaced acetabular fracture was revealed three weeks later. He was treated conser

Neocortex Formation in a Tapered Wedge Stem is Not Indicative of Complications or Worse Outcomes.

BACKGROUND: The formation of sclerotic bone, a neocortex, distally surrounding total hip arthroplasty (THA) stems may commonly be seen on radiographs around proximally porous coated stems but can be confused with loosening. The goal of this study was to determine the prevalence of the neocortex finding and whether it associated with worse outcomes after THA. METHODS: A retrospective review of 825 patients with a single tapered wedge stem was performed. Radiographs at 1-year, as well as final follow-up were reviewed for evidence of sclerotic bone (neocortex) surrounding the stem in all 14 Grue

A novel technique for stabilising sacroiliac joint dislocation using spinal instrumentation: technical notes and clinical outcomes.

PURPOSE: Currently, sacroiliac joint dislocations, including crescent fracture-dislocations, are treated using several techniques that have certain issues. We present the technical details and clinical outcomes of a new technique, anterior sacroiliac stabilisation (ASIS), performed using spinal instrumentation. METHODS: ASIS is performed with the patient in a supine position via the ilioinguinal approach. The displacements are reduced and fixed by inserting cancellous screws from the sacral ala and iliac brim; the screw heads are bridged using a rod and locked. We performed a retrospective re

Outpatient total hip and knee arthroplasty exhibit similar early complication rates to inpatient procedures.

PURPOSE: The purpose of this study was to compare early post-operative complications and readmissions between total joint arthroplasty (TJA) patients completed as outpatient versus inpatient procedures. METHODS: A review of 2648 primary total hip arthroplasty (THA) and total knee arthroplasty (TKA) patients between 2015 and 2019 was performed. Patients who underwent same-day discharge were matched to the inpatient cohort using age, gender, body mass index (BMI), and smoking status. American Society of Anesthesiologists (ASA) Scores were calculated for the two cohorts. A total of 234 matched p

Primary total hip arthroplasty using modified posterior approach through quadratus femoris flap osteotomy; comparison with conventional posterior approach.

INTRODUCTION: The quadratus femoris muscle has not attracted attention as a structure for surgical exposure during posterior hip approaches. We sought to introduce a modified posterior approach through the quadratus femoris muscle area only, by flap osteotomy, which we have named the quadratus femoris osteotomy (QFO) approach. We compare this with the conventional posterior approach to determine the effectiveness of the new technique. METHODS: We retrospectively reviewed the medical records of 329 patients (383 hips) who had undergone primary total hip arthroplasty (THA) between March 2006 an

Posterior sternoclavicular joint injuries in skeletally immature patients.

The management of sternoclavicular injuries in skeletally immature patients has not been well described. The purpose of this study was to describe the authors' experience treating this rare and potentially life-threatening injury. All skeletally immature patients who underwent treatment for a medial clavicular physeal fracture or sternoclavicular dislocation between 2003 and 2011 were identified using ICD-9 diagnostic codes. Sternoclavicular injuries with posterior displacement were isolated from this cohort for a thorough chart review. Patients were contacted to complete brief phone surveys a

Mechanical stability analysis of reference clamp fixation in computer-assisted spine surgery.

BACKGROUND: Pedicle screw misplacement is a common complication, while 7% may result in neurological complications. Computer-assisted navigation improves the rate of ideally placed screws. Inappropriate reference marker attachment can cause major problems in the outcome and duration of surgery. OBJECTIVE: To improve fixation of reference bases by comparing different designs of spine clamps and measuring their stability against the relevant thoracic and lumbar anatomy. METHODS: Force needed to dislocate the clamp from the processus spinosus using defined fixation of 0.79, 0.90 and 1.02 Nm tor

Surgical management of iliac wing fractures: Proposal of a new classification system.

INTRODUCTION: Isolated fractures of the iliac wing are a rare injury, usually occurring in high-energy trauma, and are associated with other non-musculoskeletal and soft tissue injuries that could compromise the patient's life. Surgical indications are unclear, and there is limited information on the most frequent fracture patterns. METHODS: A descriptive multicenter case series study of isolated fractures of the iliac wing treated surgically in three referral trauma hospitals in Latin America. The different fracture patterns are described, the "iliac ring" concept is proposed, and a classifi

A three-dimensional computed tomography study of the palmar ulnar corner fragment in distal radial fractures.

Fixing palmar ulnar corner fragments of distal radial fractures can be challenging. We described the palmar ulnar corner fragment morphology in a retrospective cohort study of 40 patients who underwent preoperative wrist computed tomography scans. Palmar ulnar corner fractures were categorized based on articular cross-sectional area, sagittal angulation relative to the radius long axis, palmar cortical length, radioulnar width and associated palmar radiocarpal subluxation. Three types emerged: type 1 fragments involved 37% (SD 10) of the radiocarpal articular surface and were extended in the s

Low-Energy Lisfranc Injuries: When to Fix and When to Fuse.

Lisfranc injuries can be devastating to the athlete and nonathlete. In the athletic population, minor loss of midfoot stability compromises the high level of function demanded of the lower extremity. The most critical aspect of treatment is identifying the injury and severity of the ligamentous/articular damage. Not all athletes are able to return to their previous level of function. With appropriate treatment, a Lisfranc injury does not mandate the cessation of an athletic career. We focus on the diagnosis and an algorithmic approach to treatment in the athlete discussion the controversy of o

Pediatric posterior monteggia lesion: a greenstick fracture of the proximal ulnar metaphysis with radial neck fracture. A case report.

We report a case of posterior Monteggia lesion in a 12-year-old girl consisting of a greenstick fracture of the proximal ulnar metaphysis and a radial neck fracture with a proximal and posterior 90-degree rotational displacement of the radial head. Closed reduction of the radial head resulted in complete reversal of the radial head, which was corrected by open reduction and internal fixation. Cast immobilization without correction of the plastic deformation of the proximal ulna caused posterior bowing of the ulna and posterior displacement of the fractured radial neck. Corrective osteotomy of

Tibial plateau fractures. Management and expected results.

The objective of treatment of tibial plateau fractures is precise reconstruction of the articular surfaces, stable fragment fixation allowing early motion, and repair of all concomitant lesions. A classification scheme is employed to include the high incidence of concomitant lesions in specific fracture types. The term "complex trauma" is used for extensive injuries involving multiple structural elements of the knee joint. Exact grading of the soft-tissue injury is crucial for the treatment plan. The authors suggest a four-grade classification system of closed and open soft-tissue injury. Pref
