Knowledge localization is associated with higher performance of domestic large language models in a Chinese radiation oncology examination.

INTRODUCTION: The rapid evolution of large language models has demonstrated human-level proficiency in general English-language medical examinations, yet their applicability in highly specialized, non-English clinical domains remains insufficiently explored. METHODS: To bridge this gap, this study benchmarked leading domestic (Qwen 3 Max, DeepSeek V3.2) and international (GPT-5, Gemini 2.5 Flash) models against a board-certified Chinese radiation oncologist, using the National Intermediate Professional Title Examination for Radiation Oncology in China. Statistical comparisons were conducted u…

Structured clinical approach to enable large language models to be used for improved clinical diagnosis and explainable reasoning.

BACKGROUND: Applying large language models to medicine faces critical trust challenges in diagnostic reasoning. Existing approaches often fail to generalize across different models and datasets, particularly those covering a wide range of diseases and diverse patient records. This study aims to develop a universal model-based clinical framework that improves diagnostic performance while providing explainable reasoning. METHODS: We introduce a structured clinical approach that replicates real-world diagnostic workflows. Patient narratives are first transformed into labeled clinical components.…

Speech and swallow outcome measures for ALS and perspectives on remote monitoring: an international survey of speech & language therapists.

OBJECTIVE: Dysarthria and dysphagia occur frequently in Amyotrophic Lateral Sclerosis (ALS). To manage these symptoms, speech & language therapists (SLTs) must identify relevant speech and swallow outcomes and select suitable outcome measurement instruments. Remote monitoring is an evolving mode of health status tracking. This survey aimed to establish SLT perspectives on ALS assessment regarding 1) the clinical meaningfulness of existing outcome measurement instruments 2) remote monitoring 3) usefulness of assessment devices for patient care and 4) bulbar function outcomes and measurement ins…

Difference in Language Profiles of Children With Autism Spectrum Disorder and Down Syndrome Is Not Driven by Non-Verbal Cognition.

BACKGROUND: Autism Spectrum Disorder (ASD) and Down syndrome (DS) are among the most common types of neurodevelopmental conditions that have co-occurring language impairments. Usually, non-verbal IQ has been reported as one of the main predictors of language functioning in children with these conditions. Although language abilities of children with ASD and DS have been described in the previous studies, there is still a lack of direct comparisons of language profiles in the non-verbal IQ-matched groups of children with these disorders, and, therefore, it is largely unexplored whether language …

Leveraging simulation to provide a practical framework for estimating the novel scope of risk of large language models in healthcare.

BACKGROUND: Large language models (LLMs) are rapidly entering clinical and consumer use, yet their probabilistic outputs have delivered a variety of unsafe user responses. Difficulties in quantifying and mitigating risks posed by LLMs threaten to stall regulatory evaluation and clinical deployment of LLM-based software as a medical device (LLM-SaMD). Practical approaches are needed to extend existing medical-device regulations to LLM-SaMDs. OBJECTIVE: To demonstrate how simulation can extend existing medical-device risk management frameworks for addressing LLM-SaMD-specific risks. METHODS: W…

Validity and reliability of the 8-item version of the jaw functional limitation scale (JFLS-8) in the Arabic language.

BACKGROUND: The Jaw Function Limitation Scale (JFLS) is a recently developed instrument that assesses specific jaw function limitations and objective restrictions in daily activities such as chewing, jaw mobility, and verbal communication in a valid and reliable manner. The scale has been translated and validated in several languages and contexts, but no Arabic version is currently available. To address the absence of a validated tool for assessing jaw dysfunction in Arabic-speaking populations, this study aimed to evaluate the validity, reliability, and factor structure of the 8-item version …

Evaluation of the performance of large language models in the management of hip and knee osteoarthritis: analysis of EULAR 2023 recommendations.

BACKGROUND: Clinical practice guidelines play a pivotal role in the management of complex and chronic conditions such as osteoarthritis (OA). However, the systematic evaluation of large language models (LLMs) against established guidelines-particularly in the context of knee and hip OA-remains limited. This study aimed to compare the responses generated by multiple LLMs to open-ended questions derived from the 2023 European League Against Rheumatism (EULAR) recommendations. METHODS: This study transformed eight recommendations from the 2023 EULAR guideline on non-pharmacological management of…

Understanding participation of persons with disabilities in clinical trial research in India-a retrospective analysis of inclusive language of protocols.

BACKGROUND AND OBJECTIVES: The under-representation of vulnerable groups in clinical trials, including persons with disabilities, could affect the applicability of trial findings to real-world public health interventions. We undertook this study to assess the extent to which persons with disabilities are excluded due to non-inclusionary language use in protocols from clinical trial research in India. METHODS: We conducted a retrospective mixed-methods analysis of fully published clinical trial protocols relevant to India (January 2017 to December 2024), sourced from national and international…

Spinal cord stimulation as a novel neuromodulation strategy for enhancing language recovery in post-stroke aphasia: a hypothesis.

Post-stroke aphasia is a debilitating language disorder that significantly impairs communication and quality of life. Although speech therapy and non-invasive brain stimulation techniques such as repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) have shown clinical benefit, treatment responses remain variable and incomplete. We hypothesize that spinal cord stimulation (SCS), encompassing invasive epidural SCS (eSCS) and two mechanistically distinct non-invasive transcutaneous modalities, transcutaneous spinal direct current stimulation (tsDC…

Performance of leading large language models in adhering to clinical guidelines for anaplastic thyroid cancer: a comparative study.

Anaplastic thyroid cancer (ATC) is a rare, aggressive malignancy with poor prognosis. Adherence to guidelines from the National Comprehensive Cancer Network (NCCN), American Thyroid Association (ATA), and European Society for Medical Oncology (ESMO) is critical for optimal patient outcomes. As large language models (LLMs) increasingly enter clinical workflows, rigorous evaluation of their alignment with established guidelines is essential. We evaluated five leading LLMs for their ability to generate guideline-concordant responses to clinical questions about ATC. We conducted a comparative stud…

Right Hemisphere Functional Connectivity Increases in Proportion to Left Hemisphere Lesion Size, but Not Within the Language Network.

Left hemisphere (LH) strokes often disrupt the language network (LN), causing aphasia. The right hemisphere (RH) has been proposed to contribute to post-stroke aphasia outcomes, but existing findings on RH functional connectivity (FC) changes after LH stroke are inconsistent. This study systematically investigates whether and how the RH FC differs in chronic stroke survivors compared to healthy controls, and whether these differences relate to lesion characteristics and language outcomes. Using a naturalistic movie-watching fMRI paradigm, we compared the RH FC in a large sample of chronic LH s…

Written Languaging with Direct Feedback in Beginner CSL Writing: A Crossover Study of Metacognitive Engagement and Transfer Effects.

Understanding the cognitive architectures that enable learners to process feedback and self-regulate is fundamental to fostering intelligent second language (L2) learning, particularly as AI-based pedagogies become prevalent. Direct written corrective feedback, while common in beginner L2 writing, often triggers only surface-level processing. This study investigates whether written languaging (WL), a metacognitive activity where learners explain language problems in writing, can deepen feedback processing, thereby activating a more robust cognitive architecture for error correction. Using a wi…

Using the DNA language model, GROVER, to parse effects of sequence, chromatin and regulatory features on genome stability.

MOTIVATION: Genome stability is shaped by DNA sequence and chromatin context, but their relative contributions to double-strand break (DSB) sensitivity remain unclear. RESULTS: We show that the DNA language model, GROVER, can infer DSB location based on sequence. DSB hotspots tend to contain GC-rich sequences that belong to promoters, genes and short interspersed nuclear elements (SINEs). Additionally, we identified several specific short sequences (tokens) that are associated with modulating DSB sensitivity. Another model using chromatin and genome regulatory features outperforms the sequenc…
