Appropriate use of fluoroquinolones in children.

With the increasing resistance to antibiotics among common bacterial pathogens, challenges associated with the use of fluoroquinolones (FQs) in paediatrics have emerged. The majority of FQs have favourable pharmacokinetic properties, although these properties can differ in children compared with adults. Moreover, all FQs have broad antimicrobial activity both against Gram-positive and Gram-negative bacteria. However, only some FQs for which adequate studies are available have been approved for use in children in a limited number of clinical situations owing to the supposed risk of development

Whole exome sequencing for the identification of a new mutation in TGFB2 involved in a familial case of non-syndromic aortic disease.

BACKGROUND: Non-syndromic aortic disease (NSAD) is a frequently asymptomatic but potentially lethal disease characterised by familial cases of thoracic aortic aneurysms and dissections. This monogenic but genetically heterogeneous condition is primarily inherited as an autosomal dominant disorder with low penetrance and variable expression. Mutations in ACTA2, TGFBR1, TGFBR2, MYH11, SMAD3, MYLK, and FBN1 genes have been described but still, there are many unresolved familial cases. METHODS: The whole exome of two distantly related and affected members of a Spanish family with multiple cases o

Epidemiology of partial urorectal septum malformation sequence (or 'persistent cloaca'): a population-based study in seven regions of England and Wales, 1985-2010.

BACKGROUND: Partial urorectal septum malformation (pURSM) sequence (or 'persistent cloaca') is a rare congenital anomaly characterised by a joining of the urethral, anal, and genital openings into a single common channel. This study describes the epidemiology of pURSM sequence in England and Wales including prevalence, additional anomalies, and pregnancy outcomes. METHODS: All cases of pURSM sequence prospectively notified to seven congenital anomaly registers in England and Wales during 1985-2010, whether delivered as live births, spontaneous fetal deaths (≥20 weeks' gestation), or elective

Characteristics of selected parameters of body posture in patients with hip osteoarthritis.

BACKGROUND: Osteoarthritis is the most common disorder of the musculoskeletal system. A unilateral change in the hip joint changes the position of the pelvis and spine, and therefore the patient's body posture. Due to the uneven loading of the lower limbs asymmetry occurs in the trunk. Detailed dependencies of that have not been thorougly described in the available literature. The aim of the study was to characterize the posture parameters in patients with unilateral osteoarthritis of the hip. MATERIAL AND METHODS: The study population consisted of 40 patients with unilateral hip osteoarthrit…

Aging, the central nervous system, and mobility.

BACKGROUND: Mobility limitations are common and hazardous in community-dwelling older adults but are largely understudied, particularly regarding the role of the central nervous system (CNS). This has limited development of clearly defined pathophysiology, clinical terminology, and effective treatments. Understanding how changes in the CNS contribute to mobility limitations has the potential to inform future intervention studies. METHODS: A conference series was launched at the 2012 conference of the Gerontological Society of America in collaboration with the National Institute on Aging and t

Reduced satellite cell population may lead to contractures in children with cerebral palsy.

AIM: Satellite cells are the stem cells residing in muscle responsible for skeletal muscle growth and repair. Skeletal muscle in cerebral palsy (CP) has impaired longitudinal growth that results in muscle contractures. We hypothesized that the satellite cell population would be reduced in contractured muscle. METHOD: We compared the satellite cell populations in hamstring muscles from participants with CP contracture (n=8; six males, two females; age range 6-15y; Gross Motor Function Classification System [GMFCS] levels II-V; 4 with hemiplegia, 4 with diplegia) and from typically developing p

Vacuum phenomenon: prevalence and appearance in the knee with 3 T magnetic resonance imaging.

OBJECTIVES: To determine the prevalence of vacuum phenomenon (VP) in the knee on magnetic resonance (MR) images, describe the imaging features that characterize VP, and assess how often VP mimics pathological knee lesions. MATERIALS AND METHODS: Consecutive knee MR studies performed on a 3 T MR system over a 9-month period were retrospectively reviewed by one radiologist who then selected studies with findings potentially indicating VP. Three experienced musculoskeletal radiologists reviewed these cases in consensus to confirm the presence of VP and to assess the shape, size, and signal of VP

Case mix in paediatric rheumatology: implications for training in Australia.

BACKGROUND: Despite a move towards the provision of specialist training in Australia in settings that extend beyond the public hospital system, formal comparisons of case mix between public and private specialty clinics have rarely been performed. It is therefore unclear for many specialties how well training in one setting prepares trainees for practice in the other. AIMS: This study aims to compare the case mix of paediatric rheumatology patients seen in public and private settings and the referral sources of patients in each. METHODS: An audit of all new patients seen in the public and pr
