Fifteen patients suffering from visual disturbance of varying degrees caused by a mucocele of the posterior ethmoid and/or sphenoid sinuses … tively. During operation partial bony defect was found in the optic canal in 12 patients and in the skull base in 12 patients. A good understanding of this disease by ophthalmologists and otolaryngologists is essential for early diagnosis and prompt surgical treatment to avoid permanent visual dysfunction and operative sequelae.

Osteopetrosis is a rare congenital disorder of bone resorption, which causes generalized hyperostosis. One of its many complications involves the skull base and progressive encroachment of the optic nerves within their canals, resulting in optic atrophy and blindness. Management of patients with deteriorating vision may include surgical decompression of the optic canals; their selection requires careful assessment by radiology, computed tomography, visual evoked potentials, electroretinography and possibly haemodynamic studies of the ophthalmic arteries. Postoperative changes can create diffic…

Vertebrobasilar-distribution stroke is a rare but sometimes severe complication of chiropractic neck manipulation. We report two patients with dissections of the vertebral arteries authenticated two and six days after the cervical manipulation. In the first case, a Wallenberg's syndrome occurred due to a dissection of the right intracranial vertebral artery; the patient was treated with anticoagulant therapy but little improvement of the disorder was noted. The second patient had transitory neurologic manifestations which led to the discovery of an intimal tear of the ostium of the right verte

Craniometaphyseal dysplasia (CMD) is a very rare disorder of bone remodelling characterised by sclerosis of the skull base, vault and facial bones and metaphyseal splaying of tubular bones. The recessive form appears to be more severe than the dominant. Cranial nerve deficits have been reported in infancy and early childhood in a few patients, but the long-term history of recessive CMD is not well documented. We report cerebellomedullary compression in a girl with recessive CMD recognised at 14 years because of progressive truncal ataxia. MRI revealed backward angulation of the thickened clivu…

We report on a case of achondrogenesis type IA (Houston-Harris) with an occipital encephalocele. Prenatal sonograms revealed polyhydramnios, subgaleal edema, microcephaly, a narrow thorax, pericardial effusion, and a severe short-limbed dwarfism with unossified tubular bones and vertebral bodies. Postmortem examination demonstrated additional findings of hydrops fetalis, a membranous calvarium with a defect, an occipital encephalocele, hypoplastic lungs, and wedge-like tubular bones. Whole body radiography revealed no ossification of the bones except some small identified foci of calcification

Hypoglossal nerve (cranial nerve XII) palsy is uncommon. Damage to this nerve produces characteristic clinical manifestations, of which unilateral atrophy of the tongue musculature is the most important. When these features are recognized, the radiologist, armed with knowledge of the normal anatomy of the area, can focus on each segment of the nerve in search of a cause. The hypoglossal nerve is divided into five segments: the medullary, cisternal, skull base, nasopharyngeal/oropharyngeal carotid space, and sublingual segments. Because each segment is usually affected by different disorders, l…

A 57-year-old male with clival chordoma developed severe hoarseness, dysphagia, and dysphonia 1 month after a second removal of the tumor. Magnetic resonance imaging demonstrated a mass 10 cm in diameter in the region of the middle clivus enhanced inhomogeneously by gadolinium-diethylenetriaminepentaacetic acid, and a defect in the skull base. There was evidence of compression of the anterior surface of the pons. He received proton irradiation employing a pair of parallel opposed lateral proton beams. The dose aimed at the tumor mass was 75.5 Gy, to the pharyngeal wall less than 38 Gy, and to …

Bony abnormalities caused by thanatophoric dysplasia affect the base of the skull and the vertebrae as well as the ribs and appendicular long bones. We present our findings in a full-term infant with thanatophoric dysplasia in whom the posterior fossa, the rostral vertebral column, and the neuraxis at and adjoining the craniovertebral junction were studied by dissection, roentgenography, and histologic examination. In this infant, malformations of the vertebral laminae, most prominent in the basiocciput and atlas vertebra, led to compression of the rostral cervical spinal cord, causing gliosis

Chordomas are bone tumors of the axial skeleton. They arise from notochordal remnants. In children these tumors are extremely rare and are predominantly located in the skull base. The authors report on a clivus chordoma in a 9 7/12-year-old girl. It presented as a nasopharyngeal mass with destruction of the clivus and paralyses of the ninth, tenth, and eleventh cranial nerves on the right side. After incomplete resection by a transoral transclival route, high-dose radiotherapy was added. This treatment was effective as demonstrated by follow-up CAT scans. A short review of the current literatu…

Magnetic resonance imaging (MR) has already gained wide acceptance in the evaluation of intracranial and spinal canal abnormalities. MR also provides excellent resolution of certain tumors of the head and neck and is particularly useful for the evaluation of neoplasms in the vicinity of the skull base. The absence of a bone signal prevents the streaking artifact so troublesome with computerized axial tomography (CT) and allows better definition of tumor. MR does not use ionizing radiation and appears to be an innocuous imaging mode--thus multiple examinations in young patients are not objectio…

Thirty-six patients with different clinical and topographic forms of cerebromeningeal tuberculosis: meningitis, meningo-encephalitis, tuberculoma, abscess and their complications, were investigated by CT scans. Whereas standard films allow diagnosis of intracranial hypertension, other neurological examinations are very insufficient when compared with CT imaging, a non-invasive procedure which provides presumptive signs of a tuberculous lesion and thus assists diagnosis, and assessment and follow up of results of treatment. Meningitis and meningo-encephalitis are seen as isodense images of cist

The development of new non-irritating thermoplastic endotracheal tubes together with improved nursing techniques have resulted in a considerable reduction in the number of tracheotomies. The choice of method to keep the airways patient depends on various factors and must be made individually. The advantages and disadvantages of naso-tracheal and oro-tracheal intubation and of tracheotomy are reviewed. Some of the risks and complications attendant on long-term naso-tracheal intubation are discussed. Maxillary sinusitis due to obstructed drainage should have X-ray examination without delay and,

OBJECTIVE: One consideration in pediatric stereoencephalography (SEEG) is decreased skull thicknesses compared with adults, which may limit traditional bolt-based anchoring of electrodes. The authors aimed to investigate the safety profile, complication rates, and technical adaptations of placing SEEG electrodes in pediatric patients.
METHODS: The authors retrospectively reviewed all patients aged 12 years or younger at the time of SEEG implantation at their institution. Postimplantation CT scans were used to measure skull thickness at the entry point of each SEEG lead. Postimplantation lead

The unsolved problem in three-dimensional surgical planning for patients with facial deformity, dysgnathia, or asymmetry is the lack of a normative database of "norm skulls" that can be used as treatment objectives. A study was conducted on 90 Eurasian persons (46 male and 44 female adults) for whom cone beam-computed tomography images were available. Inclusion criteria were adult patients with a skeletal Class I pattern, proper interincisal relationship with normal occlusion, the absence of an open bite both in the anterior and posterior region, and a normal and balanced facial appearance; pa

Reliable computer models are needed for a better understanding of the physical mechanisms of skull fracture in accidental hits, falls, bicycle - motor vehicle & car accidents and assaults. The performance and biofidelity of these models depend on the correct anatomical representation and material description of these structures. In literature, a strain energy criterion has been proposed to predict skull fractures. However, a broad range of values for this criterion has been reported. This study investigates if the impactor orientation, scalp thickness and material model of the skull could prov

PURPOSE: Manual segmentation of CT datasets for preoperative planning and intraoperative navigation is a time-consuming procedure. The purpose of this study was to develop an automated segmentation procedure for the facial skeleton based on a virtual anatomic atlas of the skull, to test its practicability, and to evaluate the accuracy of the segmented objects.
MATERIALS AND METHODS: The atlas skull was created by manually segmenting an unaffected skull CT dataset. For automated segmentation of cases via IPlan cranial (BrainLAB, Germany), the atlas skull underwent projection, controlled deform

INTRODUCTION: Correlations between cephalometric measurements are frequently assumed to represent biologic associations. However, a significant portion of such correlations might arise from purely geometric dependencies, when measurements share common landmarks. Analytic calculation of this topographic component is difficult. The purpose of this study was to propose a permutation method for evaluating the topographic component of cephalometric correlations.
METHODS: The method consisted of creating a virtual sample of cephalometric tracings (landmark configurations) from the original biologic

The aim of the present study was to examine the craniofacial morphologic characteristics of different vertical dysplasias in a population of Chinese adults with normal occlusion. Sixty-nine subjects (39 males and 30 females) were selected from 800 healthy students between 18 and 24 years of age. Lateral cephalograms were obtained and 27 hard and 10 soft tissue measurements were analysed. The subjects were then divided into three groups: high angle, low angle, or control according to the value of FH-MP. Intraclass correlation coefficient was determined for the repeated measurements. One-way ana

INTRODUCTION: Fronto-orbital advancement (FOA) is the standard treatment for craniosynostosis patients with fronto-orbital dysmorphology. Few studies examine the incidence of secondary cranioplasty operations after FOA. This retrospective analysis utilizes multicenter billing data to identify risk factors and the incidence of secondary reconstructive cranioplasty operations after FOA.
METHODS: A retrospective review was performed on all patients aged 0 to 16 years who underwent primary FOA between 2007 and 2015 in the Pearldriver database. The rates of secondary cranioplasty were analyzed thr
