Drug-Induced Sleep Endoscopy in Children Predicts OSA Severity.

BACKGROUND: While adenotonsillectomy (AT) is first line for pediatric obstructive sleep apnea (OSA), treatment failure is common. Drug-induced sleep endoscopy (DISE) provides dynamic airway assessment, but its role in predicting OSA severity requires clarification. This study correlates DISE findings with polysomnographic severity. OBJECTIVE: To determine the association between DISE-based obstruction patterns and OSA severity in children. METHODS: This prospective study included 95 children with adenotonsillar hypertrophy. All underwent preoperative Level III polysomnography followed by DIS…

Obstructive sleep apnea in people with epilepsy: Modifying risk.

Obstructive sleep apnea (OSA) is a common but underdiagnosed and undertreated sleep disorder among people with epilepsy (PWE). In PWE, this sleep disorder is often managed as a comorbid condition rather than a contributor to epilepsy outcomes. For many years, OSA has been associated with higher seizure burden and interictal epileptiform discharges. Emerging evidence links OSA to late onset epilepsy (LOE) and increased risk markers for sudden unexpected death in epilepsy (SUDEP). This evidence also suggests that treating OSA with continuous positive airway pressure may improve seizure control. …

Longitudinal associations between sleepiness and sleep disturbances across pregnancy.

OBJECTIVE: Although sleep disturbances increase during pregnancy, sleepiness does not appear to follow a similar trajectory. Given that the relationship between sleep disturbances and sleepiness during pregnancy is understudied, we examined their associations to determine whether these relationships strengthen with advancing gestation. METHODS: For this prospective longitudinal survey, we enrolled 3808 women, of whom 3286 responded. Sleep disturbances were assessed in early pregnancy, midpregnancy, and late pregnancy and at delivery. Mixed-effects models were constructed with adjustments for …

Differential expression of spatiotemporal sleep spindle clusters in aging.

STUDY OBJECTIVES: Sleep spindles are potential biomarkers for memory decline in aging. However, significant within-person variability in spindle attributes complicates their utility in predicting cognitive deterioration. This study aimed to uncover distinct spindle types and their relevance to memory decline using exploratory, data-driven clustering. METHODS: Polysomnography was collected from younger (n = 43, ages 20-45 years) and older cognitively healthy adults (n = 34, ages 60-81 years). Clustering analysis was performed using multiple features and spatiotemporal context, irrespective of …

Can sleep affect destination memory? A prospective narrative review.

The destination memory is the ability of individuals to remember to whom the information has been delivered. The memory system is an essential and critical piece of social communication and human social cognition. Previously, research has been done on the relationship between source memory and sleep which are critical and fundamental processes influencing our daily lives. However, this present prospective narrative review sheds light on the idea of beginning new research to understand the relationship between destination memory and sleep. Although no such literature exists that can explain thi…

Gradients in signal complexity of sleep-wake intracerebral EEG.

Spatial variation in the morphology of the electroencephalogram (EEG) over the head is classically described. Ultimately, location-dependent … y, mapped to continuous color scale and overlaid on segmented cortical surface models, one for each sleep-wake state. Wake SE followed a rostro-caudal gradient (RCG), with high values anteriorly and a global minimum in the posterior cortex. Superimposed on the RCG were other gradients radiating away from primary somatic sensorimotor, visual and auditory …

Non-motor Subtypes of Parkinson's Disease and Sleep Dysfunction.

Parkinson's disease is a syndrome with many clinical presentations. It is often dominated by visible motor symptoms; however, specific nonmotor features, such as cognitive dysfunction, sleep dysfunction, pain, apathy, dysautonomia, depression, and anxiety, enrich the clinical picture significantly. Proposed non-motor phenotypes segregate to central cholinergic, serotonergic, or noradrenergic dysfunctions, and clinical and biomarker-driven studies support these subtypes. Specific sleep problems appear to be associated with specific nonmotor subtypes of Parkinson's disease.

Association of ethylene oxide exposure and obstructive sleep apnea.

BACKGROUND: Ethylene oxide (EO) is a widely utilized industrial compound known to pose health hazards. Although its carcinogenic characteristics have been thoroughly investigated, recent findings indicate possible links to respiratory disease. The correlation between EO exposure and the likelihood of developing obstructive sleep apnea (OSA) in individuals remains unclear. The study aimed to explore the association between EO exposure and OSA within the broader US population. METHODS: From 2015 to 2020, 4355 participants were analyzed cross-sectionally in the National Health and Nutrition Exam…

Examining Sleep-Disordered Breathing Events Using Latent Profile Analysis.

The clinical utility of the ratio of the apnea-hypopnea index (AHI) occurring during rapid eye movement (REM) and non-REM (NREM) sleep (AHIREM/AHINREM ratio) has been debated. We investigated the heterogeneity of REM and NREM sleep behaviors to identify unobserved distinct subtypes of sleep-disordered breathing (SDB) and examine their demographic and clinical features. The present study used a sample of 3626 adult patients who underwent diagnostic polysomnography evaluations at the Sleep Disorders Center of King Abdulaziz Medical City in Riyadh, Saudi Arabia. Latent profile analysis was perfor…

Similarities and differences between natural sleep and urethane anesthesia.

Slow oscillations dominate the EEG or local field potential (LFP) of mammals during specific periods within natural sleep and anesthesia. Such similarities have led to the use of anesthesia as a model to study sleep and state-dependent changes of consciousness. Previous research has documented the similarities between the activated state of urethane anesthesia and natural REM sleep, particularly with respect to network oscillations in the theta (θ) frequency domain. Likewise, the deactivated states, characterized by large amplitude slow waves in both urethane anesthesia and non-REM sleep, have…

Sleep disorders and mental health: Understanding the cognitive connection.

Sleep disorders, particularly insomnia, have emerged as a critical public health challenge, with the situation worsened by the coronavirus disease-2019 pandemic. Insomnia symptoms, which affected up to 45% of the population during this period, highlight the urgent need to understand the mechanisms linking sleep disturbances to mental health outcomes. Recent findings suggest that cognitive failures, such as memory lapses and attentional deficits, mediate the relationship between insomnia and emotional disorders such as anxiety and depression. The role of personality traits, particularly neuroti…

Improving Nocturia Management Through Sleep Apnea Diagnosis and Treatment.

INTRODUCTION/BACKGROUND: Obstructive sleep apnea (OSA) has an established association with nocturia, but even when referred, patients presenting with nocturia may not undergo a full evaluation or begin treatment for underlying OSA. At our institution, patients with nocturia (≥ 2 episodes per night) are often referred for at-home sleep studies. This study aims to assess if patients presenting with nocturia undergo evaluation for OSA and the impact of continuous or automatic positive airway pressure (CPAP/APAP) on nocturia severity. METHODS: We conducted a retrospective chart review of patients…

Sleep patterns predicting stress resilience are dependent on sex.

Sleep disturbances and stress have a well-established link with neuropsychiatric illness; however, the nature of this relationship remains unclear. Recently, studies using the mouse social-defeat stress model revealed a causal role for non-rapid eye movement (NREM) sleep in the maladaptive behavioral responses to stress. These results suggest a novel function for NREM sleep; as a response by cortical neurons to mitigate the maladaptive effects of stress. A major limitation in many social defeat studies has been the exclusion of females. Women exhibit a greater prevalence of both affective diso…
