Psychological Profile and Distinct Salivary Cortisol Awake Response (CAR) in Two Different Study Populations with Obstructive Sleep Apnea (OSA) and Central Serous Chorioretinopathy (CSC).

Obstructive sleep apnea (OSA) and central serous chorioretinopathy (CSC) are in terms of nosography different pathologies, however they share a stress-related physio-pathogenetic component, not yet explored in depth. Therefore, the aim of the present study was to ascertain whether OSA and CSC share a common profile, specifically in cortisol production focusing on the cortisol awake response (CAR), the area under curve (AUCCAR) and the SLOPECAR compared with healthy matched controls. Furthermore, standardized self-administered questionnaires were used to identify mental health status related to

Diagnosing obstructive sleep apnea patients with isolated nocturnal hypoventilation and defining obesity hypoventilation syndrome using new European Respiratory Society classification criteria: an Indian perspective.

BACKGROUND: Recently, new classification criteria for obesity hypoventilation syndrome (OHS) have been described. OHS prevalence according to new criteria has not been reported in obstructive sleep apnea (OSA). Prevalence of OHS has not been previously reported from India. METHODS: Retrospective study was carried out in OSA patients for whom both arterial blood gases (ABG) and end-tidal CO2 (etCO2) records were available. OHS was defined according to old and new criteria. Analysis was carried out among various groups: patients without OHS (Group A), patients with sleep hypoventilation but wit

The relation between retrobulbar blood flow and posterior ocular changes measured using spectral-domain optical coherence tomography in patients with obstructive sleep apnea syndrome.

PURPOSE: To investigate the effect of posterior ocular hemodynamics on the retinal nerve fiber layer (RNFL), choroid thickness (CT) and central macular thickness (CMT) in patients with obstructive sleep apnea syndrome (OSAS) and to reveal the association with glaucomatous optic neuropathy. METHODS: The research was planned as a prospective, randomized study. The ophthalmic, retinal and posterior ciliary artery pulsatile index (PI) and resistive index (RI) were measured by colored Doppler sonography. RNFL thickness, CMT and CT were then measured by spectral-domain optical coherence tomography.

Combined adaptive servo-ventilation and automatic positive airway pressure (anticyclic modulated ventilation) in co-existing obstructive and central sleep apnea syndrome and periodic breathing.

BACKGROUND: The co-existence of obstructive and central sleep apnea/hypopnea syndrome (OSAS) and periodic breathing is common in patients with and without underlying heart diseases. While automatic continuous positive airway pressure (APAP) has proven to effectively treat OSAS, the adaptive servo-ventilation (ASV) sufficiently improves periodic breathing. This is the first trial on a device which combines both treatment modes. METHODS: Pilot study on a two-week treatment in patients with co-existing obstructive and central and periodic breathing disturbances during sleep. Twelve consecutive p

Monitored anesthesia care with a combination of ketamine and dexmedetomidine during magnetic resonance imaging in three children with trisomy 21 and obstructive sleep apnea.

We present a series of three children with trisomy 21 and obstructive sleep apnea who required sedation during magnetic resonance imaging of the upper airway. In an effort to provide effective sedation with limited effects on cardiovascular and ventilatory function, sedation was provided by a combination of ketamine and dexmedetomidine. Sedation was initiated with a bolus dose of ketamine (1 mg x kg(-1)) and dexmedetomidine (1 microg x kg(-1)) and maintained by a continuous infusion of dexmedetomidine (1 microg x kg(-1) x h(-1)). One patient required a repeat of the bolus doses of ketamine and

Analysis of error profiles occurring during the OSLER test: a sensitive mean of detecting fluctuations in vigilance in patients with obstructive sleep apnea syndrome.

The OSLER test represents a simple alternative to the maintenance of wakefulness test. Standard analysis of OSLER test results yields a mean sleep latency (MSL). The aim of this study was to use both MSL and errors (nonresponses to stimulations) to characterize daytime sleepiness in apneic patients. OSLER test results at 9:00 A.M., 11:00 A.M. and 1:30 P.M. were compared in 27 obstructive sleep apnea syndrome patients (50.4 +/- 10.4 years; apnea-hypopnea index: 43.05 +/- 25.08) and 20 control subjects (C). Not only did obstructive sleep apnea syndrome patients demonstrate earlier sleep onset th

Comment on "Association of peripheral NLRP3 inflammasome activation with prevalent and incident chronic kidney disease in patients with obstructive sleep apnea: a prospective study".

This correspondence comments on the study by Cai et al. examining circulating NLRP3 and prevalent and incident chronic kidney disease (CKD) in patients with obstructive sleep apnea (OSA). The inclusion of 648 participants, complementary cross-sectional and prospective analyses, and evaluation of estimated glomerular filtration rate (eGFR) and albuminuria are important strengths. Nevertheless, several methodological considerations affect interpretation. Prevalent CKD appears to have been classified using baseline eGFR or urinary albumin-to-creatinine ratio without consistent confirmation of chr

Remimazolam ED95 for Preoperative Sedation in Children with Obstructive Sleep Apnea, with Obesity or a Reference-Range BMI: A Biased-Coin Dose-Finding Study.

BACKGROUND: Obesity may alter total-body-weight dosing of remimazolam, whereas obstructive sleep apnea narrows the respiratory safety margin. We estimated the ED95 for protocol-defined preoperative sedation in children with obesity or a reference-range body mass index (BMI). METHODS: In this prospective, single-center sequential dose-finding study, 80 children aged 3-17 years with a clinical OSA diagnosis undergoing adenoidectomy and/or tonsillectomy entered study-defined obesity (BMI-for-age >95th percentile) or reference-BMI (25th-84th percentile) groups (40 each). Artificial intelligence-based prediction of metabolic dysfunction-associated steatotic liver disease (MASLD) and liver fibrosis in patients with obstructive sleep apnea: An innovative approach.

BACKGROUND: Obstructive sleep apnea (OSA) is strongly associated with metabolic dysfunction and may contribute to the development of hepatic disorders such as metabolic dysfunction-associated steatotic liver disease (MASLD) and liver fibrosis. Early identification of hepatic involvement in OSA is important for preventing long-term complications. This study aimed to develop and compare artificial intelligence (AI)-based models using non-invasive clinical parameters to predict steatosis, MASLD, and liver fibrosis in patients with OSA. METHODS: This prospective study included patients diagnosed

Differential effects of adenotonsillectomy on growth and asthma control in children with obstructive sleep apnea and varying body mass index: A prospective cohort study.

BACKGROUND: Adenotonsillectomy (AT) is the primary treatment for pediatric obstructive sleep apnea (OSAS). However, its effectiveness on somatic growth and comorbid conditions like asthma may vary depending on the child's body mass index (BMI). The interplay between OSAS, obesity, and asthma is complex, and prospective data guiding expectations after AT is needed. OBJECTIVE: To investigate the differential effects of AT on growth parameters and asthma control over 12 months in children with OSAS, stratified by baseline BMI categories. METHODS: We conducted a prospective cohort study between Ma

Apnea-Hypopnea Index Versus Hypoxic Burden as Predictors of Blood Pressure Response to Continuous Positive Airway Pressure Treatment in Patients With Obstructive Sleep Apnoe.

Obstructive sleep apnea (OSA) is associated with increased cardiovascular risk and hypertension. While continuous positive airway pressure (CPAP) therapy reduces blood pressure (BP), its effect shows substantial interindividual variability. Hypoxic burden (HB) has been proposed as a potentially superior predictor of cardiovascular outcomes compared to the apnea-hypopnea index (AHI). In this retrospective study, 141 patients with OSA underwent in-laboratory polysomnography and initiated CPAP therapy. Office BP and clinical parameters were assessed at baseline and follow-up (median 200 days). Re
