Chronic convection-enhanced delivery of topotecan for patients with recurrent glioblastoma: a first-in-patient, single-centre, single-arm, phase 1b trial.

BACKGROUND: Topotecan is cytotoxic to glioma cells but is clinically ineffective because of drug delivery limitations. Systemic delivery is limited by toxicity and insufficient brain penetrance, and, to date, convection-enhanced delivery (CED) has been restricted to a single treatment of restricted duration. To address this problem, we engineered a subcutaneously implanted catheter-pump system capable of repeated, chronic (prolonged, pulsatile) CED of topotecan into the brain and tested its safety and biological effects in patients with recurrent glioblastoma. METHODS: We did a single-centre,

A new, practical, low-dose 14C-urea breath test for the diagnosis of Helicobacter pylori infection: clinical validation and comparison with the standard method.

The carbon-14 urea breath test (UBT) is a reliable and non-invasive technique for the diagnosis of Helicobacter pylori (HP) infection. In this study we evaluated the diagnostic performance of a new, practical and low-dose (14)C-UBT system for the diagnosis of HP and compared the results with those obtained using the standard method. Seventy-five patients (56 female, 19 male) with dyspepsia underwent (14)C-UBT and endoscopy with antral biopsies for histological analysis. The rapid urease test (CLO test) was applied to 50 of these patients. After a 6-h fasting period, a 37-kBq (14)C-urea capsule

Prophylactic platelet transfusion for haemorrhage after chemotherapy and stem cell transplantation.

BACKGROUND: Platelet transfusions are used in modern clinical practice to prevent and treat bleeding in thrombocytopenic patients with bone marrow failure. Although considerable advances have been made in platelet transfusion therapy in the last 30 years, some areas continue to provoke debate, especially the use of prophylactic platelet transfusions for the prevention of thrombocytopenic bleeding. OBJECTIVES: To determine the optimal use of platelet transfusion for the prevention of haemorrhage (prophylactic platelet transfusion) in patients with haematological malignancies undergoing chemoth

Obstructive sleep apnea and rising cerebrovascular disease mortality in the United States, 1999-2020: A retrospective analysis.

BACKGROUND: The prevalence of obstructive sleep apnea (OSA) in the general population has been rising in recent years. OSA has not only been identified as an independent risk factor for cerebrovascular disease (CVD) but is also associated with major CVD risk factors such as hypertension, obesity, and diabetes. CVD is the second leading cause of death globally, with approximately 165,000 deaths occurring annually in the United States. This study aimed to conduct a retrospective comparative analysis of CVD mortality among adults with OSA in the United States from 1999 to 2020, stratified by gend

Plasma miR-34a-5p outperforms miR-126-3p in predicting cognitive decline in cerebral small vessel disease patients with impaired glucose regulation.

INTRODUCTION: MicroRNAs (miRNAs) are promising biomarkers for the diagnosis and prognosis of neurodegenerative diseases. Over the past few years, miR-34a-5p and miR-126-3p have become some of the most characterized miRNA, the former being associated with cellular senescence and apoptosis and the latter with the maintenance of vascular endothelial function. The present study aimed to evaluate diagnostic performance of miR-34a-5p and miR-126-3p for cognitive dysfunction in CSVD patients. In addition, we investigated whether these miR-34a-5p and miR-126-3p have mediating effect of impaired glucos

Interventions from pregnancy to two years after birth for parents experiencing complex post-traumatic stress disorder and/or with childhood experience of maltreatment.

BACKGROUND: Acceptable, effective and feasible support strategies (interventions) for parents experiencing complex post-traumatic stress disorder (CPTSD) symptoms or with a history of childhood maltreatment may offer an opportunity to support parental recovery, reduce the risk of intergenerational transmission of trauma and improve life-course trajectories for children and future generations. However, evidence relating to the effect of interventions has not been synthesised to provide a comprehensive review of available support strategies. This evidence synthesis is critical to inform further

Economic Evaluations of Speech and Language Therapy Interventions: A Scoping Review.

BACKGROUND: With constrained funding and increasing demand, the pressures on health and care services globally continue to grow. Given the challenges faced, it is imperative that services and interventions demonstrate cost-effectiveness, as well as patient/service-user and societal benefits, to maximize impact. Research has started to explore the cost-effectiveness of speech and language therapy (SLT) interventions, but little is known about this body of evidence overall. Given the increasing demand for economic information by different decision-makers, an overall understanding of the current

Analysis of CT and MRI imaging features of ectopic pancreas.

BACKGROUND: Ectopic pancreas (EP) is a rare congenital anomaly that is often difficult to diagnose preoperatively due to its nonspecific clinical manifestations and imaging overlap with other gastrointestinal subepithelial lesions. Several characteristic imaging findings have been reported; however, the quantitative imaging features of EP remain insufficiently investigated. Therefore, this study aimed to characterize the clinical and imaging manifestations of EP, summarize its computed tomography (CT) features, quantitatively compare CT attenuation values between EP and native pancreas across
