Pyodermatitis-pyostomatitis vegetans is a rare mucocutaneous dermatosis characterized by pustular and vegetating lesions of the skin and oral mucosa. It is considered a highly specific marker for inflammatory bowel diseases. The authors describe a case of pyodermatitis-pyostomatitis vegetans in a pediatric patient who presented marked clinical improvement after beginning treatment with oral corticosteroids, azathioprine, and dapsone. Bowel surveillance is mandatory, since the dermatosis is associated with inflammatory bowel diseases in more than 70% of patients, especially ulcerative colitis.

Fidaxomicin (FDX) is approved to treat Clostridium difficile-associated diarrhea and is superior to vancomycin in providing a sustained clinical response (cure without recurrence in the subsequent 25 days). The mechanism(s) behind the low recurrence rate of FDX-treated patients could be multifactorial. Here, we tested effects of FDX, its metabolite OP-1118, and vancomycin on spore germination and determined that none affected the initiation of spore germination but all inhibited outgrowth of vegetative cells from germinated spores.

Pyogenic granuloma (PG) is a small, almost always solitary, sessile or pedunculated, raspberry-like vegetation of exuberant granulation tissue. Conservative treatment by techniques such as cryosurgery, laser surgery, and electrodesiccation are usually adequate, whereas excisional treatment can often result in noticeable scars. We attempted a different approach using an injection of absolute ethanol in five patients with recurrence due to inadequate cryosurgery. This therapy is less invasive than surgical excision and appears to be an alternative therapy for PG.

A 72-year-old female was examined because of acute congestive heart failure. Echocardiogram revealed vegetations on aortic and pulmonary valve, regurgitation of four valves, and perimembraneous ventricular septal defect. Aortic valve replacement, mitral valve replacement, repair of tricuspid valve using a Carpentier ring, partial resection of pulmonary valve, and direct closure of VSD was performed. Pathological finding of resected valves showed acute endocarditis. A patient resumed social activities after surgery.

A case of right-sided endocarditis due to Salmonella typhi is described involving a native tricuspid valve in a child who was human immunodeficiency virus negative with no evidence of intravenous drug addiction. The patient had classic features of typhoid and tricuspid regurgitation without clinical evidence of bacterial endocarditis. Transthoracic echocardiography confirmed the tricuspid regurgitation. Eight cases of poisoning in workers cleaning silo are presented. Silo gas, produced during fermentation of vegetable material, contains very toxic nitrogen oxides. In this group three workers died within silo, four patients were hospitalized (one of them with acute toxic pulmonary oedema, two with sings of pneumonia, one had only transient decrease of consciousness) and recovered without detectable sequelae. One patient, in general good condition, refused hospitalization and recovered.

We report the case of a 26-year old alcoholic woman who abruptly presented with confusion associated with a major hypertonia. She evolved into a chronically vegetative state. Magnetic resonance imaging (MRI) was consistent with the diagnosis of Marchiafava Bignami disease. The corpus callosum first presented an oedematous aspect, then a central atrophy with an axial band. The clinical presentation and the functional imaging strongly suggest an association between the Marchiafava Bignami disease and diffuse cortical lesions, such as the laminar sclerosis of Morel.

We report a case of myocardial infarction with normal coronary arteriography in a 39-year-old woman presenting with tight mitral stenosis complicated by atrial extrasystoles and spontaneous atrial contrast at echocardiography. The diagnosis of coronary embolism was most probable. Coronary embolism is a rare disease, usually due to a blood clot or, less frequently, to a vegetation of endocarditis starting in a heart valve. The left network is usually involved, with typical myocardial necrosis. Coronary arteriography is sufficient to make the diagnosis. Preventive treatment is essential.

Disposable diapers for incontinent elderly patients are expensive. Cost-effectiveness analysis in this prospective study showed that diapers reduced prevalence of urinary tract infections and of infected pressure sores. The saving resulting from this reduction in morbidity lowers the cost of diapers by 40% per month. This advantage, and especially improvement in the patients' health, hygiene and quality of life, justify the use of disposable diapers. Catheters were used mostly in patients in a vegetative state, while diapers were used in the preceding stages.

From 1986 to 1988, 170 patients with asthmatic state were observed. Of them in 13, the X-ray study revealed hiatal hernia (HH) and reflux-esophagitis. The analysis of observations of 1173 patients with HH was carried out. In a number of patients together with the symptoms of pathology of the respiratory system, the vegetative, cardial and other disorders were found. All these disorders the authors consider a consequence of irritation of the vagus and celiac plexus occurring in HH. A complex of medicamentous therapy of the asthmatic state caused by HH is suggested.

The frequent occurrence of the oral lesions of histoplasmosis in combination with disseminated infection necessitates prompt recognition and diagnosis. The possibility of such an infection occurring in an immunocompromised patient should be considered high on the list of differential diagnoses, and treatment should be planned accordingly. A patient with a ruptured sinus of Valsalva aneurysm (RSVA) demonstrated an unusual two-dimensional echocardiographic manifestation of this entity. Cassava leaves, which usually contain large quantities of cyanogenic glycosides, were processed into a Zairian traditional vegetable sauce "Mpondu" by simple methods which included blanching (10 min), mashing and then boiling for 20-80 min. These methods enhanced the detoxification of the leaves, with blanching alone resulting in the loss of 57% of the free (non-glycosidic) cyanide content and of 60% of the bound (glycosidic) cyanide. It is presumed that losses of cyanide during these processes would be accounted for in volatile HCN, its derivatives and in the boiling water.

Microscopic examination of horse caecum contents revealed vegetative growth of phycomycete fungi on particles of digesta, and uniflagellated cells similar to fungal zoospores in the liquid phase. Three morphologically distinct isolates of strictly anaerobic phycomycete fungi were obtained from the caecum contents and cultured in vitro. Two of the isolates were able to utilize a wide range of plant carbohydrates for growth, including alpha-cellulose, xylan and particulate starch, and extensively digested water-insoluble plant tissues.

Laboratory measurements of foliar uptake of sulfur dioxide and ozone by red kidney beans demonstrated a strong effect of relative humidity on internal pollutant dose. Foliar uptake was enhanced two- to threefold for sulfur dioxide and three- to fourfold for ozone by an increase in relative humidity from 35 to 75 percent. For the same exposure concentration, vegetation growing in humid areas (such as the eastern United States) may experience a significantly greater internal flux of pollutants than that in more arid regions.
