The Use of Engineered Bilayered Skin (MyDermTM) in the Management of Massive Skin Defect in Grade III Gustilo-Anderson Open Fracture.

Open fracture Gustilo-Anderson grade IIIC is associated with higher risk of infection and problems with soft tissue coverage. Various methods have been used for soft tissue coverage in open fractures with large skin defect. We report a case of a patient who had grade IIIC open fracture of the tibia with posterior tibial artery injury. The patient underwent external fixation and reduction. Because of potential compartment syndrome after vascular repair, fasciotomy of the posterior compartment was performed. This wound, however, became infected and because of further debridement, gave rise to a

ORIF with percutaneous cross pinning via the posterior approach for paediatric widely displaced supracondylar femoral fractures.

BACKGROUND: Supracondylar femoral fractures are considered uncommon in children; however, they are frequently associated with complications. To date, the optimal surgical approach for these fractures remains unclear. OBJECTIVE: The aim of the present study was to determine the outcomes of open reduction with percutaneous cross pinning via the posterior approach for paediatric widely displaced, pulseless supracondylar femoral fractures refractory to closed reduction. METHODS: Between March 2007 and March 2014, 18 patients (11 boys and seven girls; average age, 6.6 years; range, 3.5-8.5 years)

Femoral lengthening with a motorized intramedullary nail.

PURPOSE: We assessed whether an intramedullary lengthening device would reduce the problems normally associated with the external fixation technique. We also wanted to determine whether it is a reliable construct for limb lengthening and deformity correction in the femur. PATIENTS AND METHODS: We conducted a matched-pair comparison of 30 femoral lengthenings, 15 with a motorized intramedullary nail (the nail group) and 15 lengthenings with an external ring fixator (the fixator group). The patients were matched based on age, sex, amount of lengthening, and the etiology of leg length discrepanc

Open reduction and annular ligament reconstruction with fascia of the forearm in chronic monteggia lesions in children.

Fifteen children with chronic Monteggia lesions were treated with open reduction, annular ligament reconstruction with fascia of the forearm, and ulnar osteotomy. Mean age was 8 years 3 months (range 3-16 years). The chronicity of missed Monteggia was from 6 weeks to 2 years (mean 12 weeks). All patients were classified as Bado type I, except one with Bado type III. Mean follow-up was 4 years 3 months after surgery. There was a loss of pronation in six cases with the mean of 16 degrees. However, only four cases had rotational loss; the others had a mean rotational arch increase of 5 degrees. T

Percutaneous pin fixation of intercondylar fractures in young children.

T-condylar fractures of the distal humerus are very rare in children. When they do occur they tend to affect those approaching skeletal maturity. Most agree that the undisplaced fracture can be managed conservatively. With increasing displacement and comminution the opinions tend to differ. As an alternative to open reduction and internal fixation we report the use of closed reduction and percutaneous pin fixation for displaced and comminuted T-condylar fractures in a series of three young children. In addition to the classical medial and lateral column fixation we report the use of a transcon

Distal metaphyseal fractures of the tibia with minimal involvement of the ankle. Classification and treatment by locked intramedullary nailing.

We reviewed 63 patients with fractures of the distal tibial metaphysis, with or without minimally displaced extension into the ankle joint. The fractures had been caused by two distinct mechanisms, either a direct bending force or a twisting injury. This influenced the pattern of the fracture and its time to union. All fractures were managed by statically locked intramedullary nailing, with some modifications of the procedure used for diaphyseal fractures. There were few intra-operative complications. At a mean of 46 months, all but five patients had a satisfactory functional outcome. The poor

Synovial tissue features associated with poor prognosis in inflammatory arthritis.

BACKGROUND: Inflammatory arthritis encompasses a group of immune-mediated diseases characterized by chronic joint inflammation. Despite having pathogenic mechanisms in common, the prognosis of rheumatoid arthritis (RA), psoriatic arthritis (PsA), and undifferentiated arthritis (UA) could be different regarding progression to chronic, to erosive, or to self-limited disease. Our aim was to evaluate the potential association of synovial tissue (ST) inflammatory cell infiltrate, the presence of ectopic lymphoid neogenesis (LN +) structures, and poor prognosis factors (PPF) in patients with RA, PsA

Pediatric tibia and femur fractures in patients weighing more than 50 kg (110 lb): mini-review on current treatment options and outcome.

The main objective of this paper is to review the current literature on treatment of tibial and femur fractures in children and adolescents guided by body weight in patients weighing 50 kg (110 lb) or more. A secondary aim of this mini-review was to determine, as per literature review, whether weight  > 50 kg (110 lb) is an identifiable factor associated with increased complication rate. A search of the PubMed/MEDLINE, EMBASE and Cochrane Database of Systematic Reviews databases from 1954 to September 2017 was performed to identify papers related to pediatric tibia and femur fractures in child

Isolated posterior cruciate ligament injuries associated with closed tibial shaft fractures: a report of two cases.

INTRODUCTION: Knee ligament injuries associated with tibia shaft fractures are usually neglected and treatment is delayed. To our knowledge, no case presentation discusses the clinical result of closed tibial shaft fracture with concomitant ipsilateral isolated PCL injury. In this literature, we report the clinical result of two cases that sustained closed tibial shaft fracture with concomitant PCL injury and discuss the treatment options. MATERIALS AND METHODS: We report the clinical result of two cases that sustained closed tibial shaft fracture with concomitant posterior cruciate ligament

Perioperative Selective Serotonin Reuptake Inhibitor (SSRI) Use and Postoperative Fracture Fixation Outcomes: A Multi-institutional Retrospective Cohort Study.

INTRODUCTION: Selective serotonin reuptake inhibitors (SSRIs) have been implicated in impaired bone metabolism and delayed fracture healing. This study evaluates the association between perioperative SSRI use and complications following open reduction and internal fixation (ORIF) and intramedullary nailing (IMN) of various long bone shaft fractures. The study hypothesis is that perioperative SSRI use negatively affects fracture healing and increases adverse postoperative outcomes across fixation constructs. METHODS: The TriNetX database was used to identify patients (≥18 years) undergoing hum

Skeletal Related Events are Rare After Radiation Treatment for Metastatic Disease of the Femur.

BACKGROUND: Pain is a common presenting symptom in patients with metastatic disease to the femur (MDF), and it is often difficult to differentiate pain from the tumor itself versus pain from an impending pathologic fracture. Radiation therapy (RT) is commonly used in the management of pain secondary to MDF but is not adequate in isolation when the underlying bone is structurally compromised. QUESTIONS/ PURPOSES: The purposes of this study were to determine 1) the incidence of skeletal related events (SREs) following RT to the femur, 2) the frequency and implications of orthopedic evaluation p

Can the TrueLok Hexapod System™ be used to accurately correct lower limb deformity in children?

INTRODUCTION: Progressive bone lengthening in children can be done using an external fixator, a lengthening nail, or plate with screws. The TrueLok Hexapod System™ (TL-HEX™) is the newest hexapod external fixator on the market. We hypothesized that the TL-HEX™ can accurately correct lower limb deformities in children. The goal of this study was to evaluate the clinical and radiographic outcomes after correcting lower limb deformities in children using the TL-HEX™ system. MATERIAL AND METHODS: Data from 58 limbs that underwent bone lengthening with the TL-HEX™ were analyzed for this retrospect
