Solitary bone cyst: controversies and treatment.

The solitary bone cyst (SBC) has not yet revealed all its secrets. The pathogeny of the SBC is thus considered and also its evolutivity. Conservative treatment has been used in attempts to heal this tumor-like bone. In the case of surgery, an original technique is described. The SBC still remains mysterious in many of its aspects. At the time of this writing, nobody can predict the occurrence modalities of this benign bone tumor. In a similar way, the reality of this tumor-like lesion cannot be precisely described. This emphasizes the first controversial point about this lesion. Indeed, must w

A digital workflow for a custom osteotomy guide for alveolar ridge splitting using an ultrasonic osteotome.

A digital method of designing a patient-specific osteotomy guide for an ultrasonic osteotome used for alveolar ridge splitting is described. The cutting paths are planned based on the ideal restoration and cone beam computed tomography (CBCT) and then transferred to surgery by the osteotomy guide. Detachable lateral guides and longitudinal guide planes enable the required bone plate formation. This approach enhances accuracy, safety, and customization in alveolar ridge splitting.

Sternal Resection and Reconstruction for Solitary Plasmacytoma of the Sternum: Case Report.

A 63-year-old patient was admitted with a sternal fracture and mass. On evaluation, most of the body of the sternum had been destroyed by a tumor. Radical resection of the sternum was performed and part of the major pectoral muscles adherent to the sternal tumor was also resected. The chest wall defect was reconstructed with mesh, bone cement, and a titanium rib plate system. Reconstruction with this method seemed to be an appropriate procedure to prevent instability of the chest wall.

Enchondroma of the skull base secondary to generalized enchondromatosis: a case report and review of the literature.

Generalized enchondromatosis, of which Ollier disease is the most common type, is a rare bone dysplasia characterized by multiple intraosseous tumors near growth-plate cartilage. These cartilaginous tumors have a propensity to transform into chondrosarcomas. Enchondromas of the skull base are exceedingly rare. We present the case of a patient with generalized enchondromatosis who developed a large enchondroma of the clivus, and we discuss the clinical presentation and potential treatments for this entity.

Reversible visual loss due to impacted lateral orbital wall fractures.

Impaction of the sharp medial edge of the orbital plate of the greater sphenoid wing into the orbital apex is a unique type of lateral orbital wall fracture that can produce a potentially reversible optic neuropathy. Two patients in whom the lateral or temporal approach to the orbit was used to reduce this type of fracture will be presented. In both patients, improvement in vision appeared to be related to removal of a bone fragment compressing the optic nerve in the orbital apex.

IoMT-Based Osteosarcoma Cancer Detection in Histopathology Images Using Transfer Learning Empowered with Blockchain, Fog Computing, and Edge Computing.

Bone tumors, such as osteosarcomas, can occur anywhere in the bones, though they usually occur in the extremities of long bones near metaphyseal growth plates. Osteosarcoma is a malignant lesion caused by a malignant osteoid growing from primitive mesenchymal cells. In most cases, osteosarcoma develops as a solitary lesion within the most rapidly growing areas of the long bones in children. The distal femur, proximal tibia, and proximal humerus are the most frequently affected bones, but virtually any bone can be affected. Early detection can reduce mortality rates. Osteosarcoma's manual detec

Decreased bone formation and increased osteoclastogenesis cause bone loss in mucolipidosis II.

Mucolipidosis type II (MLII) is a severe multi-systemic genetic disorder caused by missorting of lysosomal proteins and the subsequent lysosomal storage of undegraded macromolecules. Although affected children develop disabling skeletal abnormalities, their pathogenesis is not understood. Here we report that MLII knock-in mice, recapitulating the human storage disease, are runted with accompanying growth plate widening, low trabecular bone mass and cortical porosity. Intralysosomal deficiency of numerous acid hydrolases results in accumulation of storage material in chondrocytes and osteoblast

Salvage reconstruction of congenital pseudarthrosis of the clavicle with vascularized fibular graft after failed operative treatment: a case report.

UNLABELLED: Congenital pseudarthrosis of the clavicle is a rare condition present at birth but often diagnosed later in childhood. Indications for surgical treatment include deformity, pain, or neurovascular compromise. Reconstruction usually involves resection of the pseudarthrosis, placement of iliac crest bone graft, and internal fixation. We report a case of congenital pseudarthrosis of the clavicle that failed initial surgical management at another institution using bone marrow aspirate, bone graft substitute, and internal fixation with plate and screws. Treatment failure was associated w

Role of calcitriol in prevention of osteoporosis: Part I.

Prevention of postmenopausal osteoporosis is an alternative to the currently problematic goal of reversing the trabecular plate thinning and perforation that constitute the major pathologic defects in patients with established osteoporosis. 1,25(OH)2D3 (calcitriol) has been suggested as a drug that may decrease bone resorption sufficiently to preserve trabecular structure in perimenopausal women. In the present study, we compared the effects of calcitriol and an investigational analog, 1,25,26-(OH)3delta22-D3 (Ro-23-8525), to those of an inert vehicle in maintaining the bone mass of oophorecto

Establishment of Subtrochanteric Fracture in Pre-Clinical Animal Model.

OBJECTIVE: About 7%-34% of the femur fracture contributes to subtrochanteric fracture, and only very little research is available about these fractures when compared to common hip fractures. Hence, the aim of this study was to develop a clinically relevant and reproducible open fracture model in rabbits at the subtrochanteric region to understand the fracture healing mechanism at this site and to explore treatment effects of biophysical intervention in future studies. METHODS: An open osteotomy was created in 32 adult New Zealand white rabbits at the subtrochanteric region, followed by custom

Nonunion of a Third Metatarsal Base Stress Fracture Injured During Cross-Country Skiing Treated by Osteosynthesis: A Case Report.

Stress fractures at the base of the metatarsal are classified as high-risk stress fractures. The second and third metatarsal are likely to occur in classical ballet and jumping sports. Among them, there are few reports of nonunion cases after fractures at the base of the third metatarsal. We present a rare case of nonunion of a stress fracture at the base of the third metatarsal injured during practicing cross-country skiing. A 15-year-old male high school top-level cross-country skier couldn't stand the pain on the dorsum of his left forefoot during training for six months. Treatment of Bilateral Distal Femoral Shaft Fractures in a Paraplegic Patient with Intramedullary Nailing: A Case Report.

INTRODUCTION: Lower extremity fractures are a frequent complication in patients with chronic spinal cord injury due to significant bone mineral density loss and altered bone metabolism. While non-operative management has traditionally been favored, advances in surgical techniques have expanded treatment options, including intramedullary nailing and plate fixation. However, the optimal approach remains controversial, especially in paraplegic patients with distal femoral fractures. This case report highlights the challenges and complications associated with surgical management in this unique pat

What Are the Long-Term Clinical and Functional Outcomes After Hemicortical Allograft Reconstruction for Primary Bone Tumors?

BACKGROUND: Hemicortical allograft reconstruction is associated with low short-term complication rates and excellent functional outcomes. However, data on their long-term complication profile and patient-reported outcomes is scarce to non-existent. METHODS: Patients with hemicortical resection and allograft reconstruction for primary bone tumors were included. The Henderson classification for biologic reconstruction was used to classify allograft failures. Functional outcomes at a minimum of 10 years postoperatively were assessed. RESULTS: Twenty-four patients with femur hemicortical allogra

Role of extracellular vesicles from adipose tissue- and bone marrow-mesenchymal stromal cells in endothelial proliferation and chondrogenesis.

The secretome of mesenchymal stromal cells (MSCs) derived from different tissue sources is considered an innovative therapeutic tool for regenerative medicine. Although adipose tissue-and bone marrow-derived MSCs (ADSCs and BMSCs, respectively) share many biological features, the different tissue origins can be mirrored by variations in their secretory profile, and in particular in the secreted extracellular vesicles (EVs). In this study, we carried out a detailed and comparative characterization of middle- and small-sized EVs (mEVs and sEVs, respectively) released by either ADSCs or BMSCs. Th
