A 24-year-old man with tuberculosis meningitis developed acute paraplegia and sensory disturbances 5 weeks after receiving conventional antituberculous therapy. Magnetic resonance imaging revealed an intradural extramedullary long segmental mass mimicking en plaque meningioma at the T2-T6 vertebrae levels. Prompt surgical decompression was performed. A histology examination of the mass revealed a tuberculoma. After surgery, the patient showed improved motor power and a normal bladder function. Intradural extramedullary tuberculoma of the spinal cord is rare complication of tuberculosis meningi

PURPOSE OF REVIEW: Traumatic optic neuropathy is common clinical problem for neuro-ophthalmologists, without evidence-based guidelines regarding evaluation and therapy. Although no large natural history or randomized controlled trial has been published, corticosteroid therapy, surgical decompression or observation remains the mainstays of therapy.
RECENT FINDINGS: Additional research is ongoing, concerning medications and therapies to enhance visual recovery following trauma; future treatments may concern neuroprotective factors and retinal ganglion cell regeneration.
SUMMARY: A randomized c

Stonefish envenomation is a common marine sting. Although stonefish injuries are commonly sustained during maritime activities, this local delicacy can also be considered a regional occupational hazard for chefs. The availability and consumption of stonefish in local restaurants has increased the risk of acute carpal tunnel syndrome after a stonefish injury. This case report describes acute carpal tunnel syndrome following stonefish envenomation. An excellent recovery was achieved after surgical decompression of the carpal tunnel syndrome. Standard management of stonefish injuries should there

INTRODUCTION: We describe a case of a 9-year-old girl who developed progressive severe retro-orbital pain and partial visual loss in association with left optic-nerve compression due to polyostotic fibrous dysplasia of the skull.
MATERIALS AND METHODS: Intradural decompression of the optic nerve resulted in immediate and complete resolution of the pain as well as a vast visual improvement.
CONCLUSION: In cases of fibrous dysplasia of the skull with evidence of optic-nerve compression, relief of retro-orbital pain should be an additional consideration when deciding to proceed with surgical ma

We report the spontaneous regression of an Epstein-Barr virus-associated monoclonal lymphoid proliferation in an immunocompetent child. A 2-year-old male with acute otitis media presented with a right-sided facial palsy secondary to acute mastoiditis. During mastoid decompression a polypoid mass, a histologically diffuse large B cell lymphoma, was found. Staging revealed localized disease. At surgical re-exploration 5 weeks later the disease had resolved. Retrospective serological testing was consistent with an acute Epstein-Barr viral infection and in situ hybridization of the tumour tissue w

A case is presented with secondary trigeminal neuralgia (TN) caused by an arteriovenous malformation (AVM) of the cerebellopontine cistern, which was detected by radiological work-up for planned microvascular decompression. An AVM surrounding the trigeminal nerve was demonstrated on thin-slice heavily T (2)-weighted 3D-sequence on magnetic resonance imaging (MRI) and confirmed by angiography. The first therapeutic step was endovascular embolization with complete obliteration of the AVM and cessation of pain. Nevertheless surgical excision was performed in order to remove compressive vessels an

Carpal tunnel syndrome of mild to moderate severity can often be effectively treated in a primary care environment. Workplace task modification and wrist splints can reduce or defer referral to hospital for surgical decompression. Nerve and tendon gliding exercises may also be of benefit. Steroid injections to the mouth of the carpal tunnel are particularly useful for symptomatic women in the third trimester of pregnancy. However inadvertent neural injection may cause disabling chronic pain. Referral to a minority of practitioners trained in the technique would ensure sufficient patient number

The author describes a woman in whom an S1-2 fracture developed after L4-S1 decompression and fusion. Osteoporosis was not present, but the lesion failed to respond to conservative therapy, necessitating surgical extension of the spinal fusion. Although biomechanical complications of lumbosacral fusion are uncommon, pseudarthrosis, degenerative spondylolisthesis, and stress fractures have been reported. To date, only four cases of sacral stress fracture appear in the literature, all involving female patients and reportedly associated with osteoporosis. Unlike the present case, the fracture res

A 54-year-old man developed progressive spastic paraparesis beginning 2 weeks after a back injury caused by a subacute compressive thoracic myelopathy attributable to a post-traumatic arachnoid cyst. Three to 18 months after surgical decompression of the thoracic arachnoid cyst, the patient developed a diffuse predominantly upper motor neuron syndrome characterized by spastic quadriparesis, pseudobulbar paresis, and pseudobulbar affect. Retrograde corticospinal tract degeneration and upper motor neuron death after spinal cord injury is recognized. This case suggests that focal upper motor neur

BACKGROUND: Hyperaesthesia involving the infra-orbital nerve is rarely associated with orbital floor fracture.
METHODS: An 11-year-old boy and a 34-year-old man had persistent and severe hyperaesthesia in the distribution of the infra-orbital nerve 12 and 26 months, respectively, after blunt orbital trauma.
RESULTS: Surgical decompression of the infra-orbital nerve resulted in rapid and complete resolution of the hyperaesthesia in both cases.
CONCLUSION: Persistent hyperaesthesia of the infra-orbital nerve is another indication for exploration of the orbital floor following blunt orbital tr

During routine lower extremity laboratory dissection at the Ohio College of Podiatric Medicine, an anomalous muscle, the flexor digitorum accessorius longus was uncovered on one male and one female cadaver. The muscles were located in the deep posterior compartment of the leg and were of differing sizes and origins. This anomalous muscle has been implicated as an etiology for tarsal tunnel syndrome. In cases of tarsal tunnel syndrome that undergo surgical decompression, the flexor digitorum accessorius longus muscle may be present and may be the cause of the tibial nerve compression.

20% of all spine injuries are cervical spine injuries. Surgical treatment of these injuries must take into account the great mobility of this part of the spinal column. Therefore, biomechanical aspects must be considered, especially in the upper cervical spine-C1 and C2. Describing our own therapeutical regimen in 35 patients with unstable upper cervical spine injuries we explain the biomechanical back-ground and review the literature. It becomes evident that ventral approaches are superior to dorsal techniques for decompression, reposition, and stabilization with minimal loss of mobility.

Anterior tarsal tunnel syndrome is a rare condition in which the deep peroneal nerve is trapped below the extensor retinaculum at the ankle. The authors report a variation of this condition: a 67-year-old woman who had anterior tarsal tunnel syndrome caused by entrapment of the deep peroneal nerve by the extensor hallucis brevis muscle. Conservative treatment was unsuccessful, but surgical decompression of the nerve provided immediate improvement, and by 2 weeks postoperatively she had no residual pain or paresthesia, although there was some numbness in the first web space.

Carpal tunnel syndrome is the most common entrapment neuropathy in the United States. The diagnosis should be considered for any patient with hand or wrist pain, weakness, or numbness. Changes in sensory conduction velocities across the wrist appear to be the most sensitive electrophysiologic indicators of the condition. Treatment of mild cases involves avoiding the precipitating activity, wrist splinting, and local steroid injections; surgical decompression is often required for moderate and severe cases. Overall, prognosis relates directly to the severity of the median nerve injury at diagno

Superficial sensory communication between the ulnar and median nerves is well recognized. In the vast majority of cases this communication is from the ulnar nerve to the median nerve. We report a case in which a communicating branch passed from the median nerve to the ulnar nerve immediately proximal to the wrist to supply sensation to the little finger. The presence of this branch correlated with the presence of symptoms which had persisted in spite of conventional open carpal release 7 years earlier. Surgical decompression of this branch led to complete resolution of those symptoms.

Sixty-five patients with symptomatic pancreas divisum were treated by endostents, surgery, or observation. In 35 patients, endoscopic stenting either alone (20) or followed by surgery (15) was the primary therapy. Of 30 patients not stented, 10 underwent elective surgery and 20 were followed. Treatment was based on symptoms and biochemical and radiologic tests. The results of surgical decompression correlated favorably with endoscopic drainage. In untreated patients, the natural history of pancreas divisum was benign. Surgery is safe and effective in symptomatic patients, although multiple ope

The 'failed back' represents a major clinical problem. Among the causes recognized for the 'failed back' is lateral root stenosis which has led to the use of electromyography (EMG) in the diagnosis of root compression. One hundred and thirty-two patients selected for surgical decompression were compared with 25 undergoing spinal fusion. EMG was found to be diagnostic in 95% of patients with bony canal stenosis, but only gave positive results in 62.5% of prolapsed disc patients. EMG improved in those patients who made a good recovery after surgery.

Serial measurements of auditory brainstem-evoked responses (BERs) were conducted in 15 patients with supratentorial mass lesions. Significant prolongation of the latency of wave V BERs, which originates in the inferior colliculus, occurred when the intracranial pressure (ICP) approached 30 mmHg. In four of five patients whose BERs were measured before pupillary changes, a significant lengthening of wave V latency was observed prior to clinical manifestation of uncal herniation. These results suggest that immediate medical or surgical decompression of ICP should be performed when ICP approaches

Unreduced lunate, perilunate, and transscaphoid perilunate dislocations present a difficult and challenging surgical problem. The authors feel that all efforts should be made to obtain a reduction of the old dislocation. They support a combined dorsal and palmar approach, open reduction, and internal fixation of scaphoid with a Herbert screw to obtain midcarpal stability. A carpal tunnel decompression should be performed if symptoms suggest median nerve entrapment. When the dislocation is unable to be reduced, the authors favor limited wrist arthrodesis or a proximal row carpectomy in the abse
