Sudden-onset tinnitus associated with arterial dissection of the vertebrobasilar system.

Magnetic resonance (MR) images of 62 patients with tinnitus were reviewed. These included T1-weighted and T2-weighted axial images and three-dimensional spoiled gradient-recalled acquisition in steady-state (3D-SPGR) imaging with gadopentetate dimeglumine. In 49 cases with tinnitus occurring gradually, the MR images of 2 cases (4.1%) suggested arterial dissection of the vertebrobasilar system. One of these patients underwent vertebral angiography (VAG) and no dissection was confirmed. In 13 cases of sudden-onset tinnitus, MR images of 3 cases (23.1%) suggested arterial dissection. Of these thr

A relationship between autoimmune thyroiditis and benign paroxysmal positional vertigo?

Benign paroxysmal positional vertigo (BPPV) is a labyrinthine disorder with a typical behavior: intense crises of rotational vertigo induced by postural changes of the head, with short duration and usually good responsiveness to rehabilitative maneuvers. This phenomenon is thought to be subsequent to the movement of floating particles in the labyrinthine fluids, which can provoke gravitational stimulations. In order to conduct a metabolic and autoimmune examination, 70 patients affected by BPPV were examined. In 34 cases (48.5%) autoimmune alterations were found: in 19 cases (27.1%) the level

Linkage of infantile Bartter syndrome with sensorineural deafness to chromosome 1p.

Bartter syndrome (BS) is a family of disorders manifested by hypokalemic hypochloremic metabolic alkalosis with normotensive hyperreninemic hyperaldosteronism. We evaluated a unique, inbred Bedouin kindred in which sensorineural deafness (SND) cosegregates with an infantile variant of the BS phenotype. Using a DNA-pooling strategy, we screened the human genome and successfully demonstrated linkage of this unique syndrome to chromosome 1p31. The genes for two kidney-specific chloride channels and a sodium/hydrogen antiporter, located near this region, were excluded as candidate genes. Although

3D analysis of nystagmus during peripheral vertiginous attacks.

In order to localize the site of lesion of peripheral vertigo, 3D analysis of nystagmus during peripheral vertiginous attacks was carried out. In comparison between the three components, the horizontal component had the largest ratio in each disease. Spontaneous nystagmus was directed toward the affected side in 3 cases and to the opposite side in 15. In patients with Menière's disease (MD), all subjects had horizontal and torsional components and had almost the same slow phase velocity in these two components. In patients with vestibular neuritis (VN) and Hunt's syndrome (HS), nystagmus was d

Deafness and vertigo.

This review follows closely on the publication of significant handbooks and symposia concerned with neuro-otology, pharmacology of emesis, imaging, cochlear prostheses and aspects of vertigo which reflect the considerable advances that have been made in clinical and basic neuroscience in these areas in recent years. The value of the cochlear prosthesis has been demonstrated convincingly and may well be a model for the future of brain implantations in diverse disorders of the central nervous system. Imaging of the inner ear has made spectacular advances to provide invaluable aids to diagnosis.

Fluctuating hearing loss following immune reaction in the endolymphatic sac of guinea pigs.

This study has investigated immune injuries to the inner ear auditory system of guinea pigs. Following secondary antigen challenge to the endolymphatic sac, the mean hearing threshold significantly increased in the early phase from day 1 to day 3 and thereafter recovered. In the early phase, hearing threshold significantly increased simultaneously to the elevation of perilymph antibody levels. The size of hydrops was not the only factor that causes an increase in hearing loss as well as in AP/SP ratio. Scale-out hearing loss was seen in 2 animals with severe degeneration of the stria vasculari

Raynaud's phenomenon.

Raynaud's phenomenon is episodic digital ischemia provoked by cold and emotion; it is associated with other disorders, such as Raynaud's syndrome, especially the connective tissue diseases. Recent information suggests links with Sjögren's syndrome, malignancy, and obstructive vascular disease. The vasospasm can affect areas outside the periphery such as the inner ear in vibration white finger syndrome and the placental vessels in pregnancy. The initial presence of autoantibodies increases the chance by 63-fold of developing connective tissue diseases, and such immunologic testing can be useful

Treatment of noise induced hearing loss.

In most textbooks, it is stated that noise-induced hearing loss (NIHL) is an irreversible impairment of hearing. This is certainly true for the slowly, over time acquired NIHL. Working for years in a very noisy environment is causing a gradual killing of hair cells in the inner ear, with no hope for recovery. The other way of acquiring NIHL is by means of exposure to impact noise, and may cause various degree of impairment of the hair cell cilia, depending on the energy in the impulsive noise. If the cilia are completely broken, the chance for recovery is probably zero. In many instances, the

Positional nystagmus of benign paroxysmal type (BPPN) due to cerebellar vermis lesions. Pseudo-BPPN.

Positioning nystagmus accompanied by severe vertigo had been reported in patients with partial lesions of the inner ear, especially otolith lesions. Typically this type of nystagmus represents a latent period and subsequent fatiguability. We concur with this finding and have constantly emphasized the significance of this phenomenon in clinical diagnosis. Since we started to use CT-scanning, this type of nystagmus has been noted in 20 patients, all of whom had cerebellar vermis lesions. Attention should be focused on this association. A simple coincidence could not be excluded if such a combina

Otocerebral mucormycosis--a case report.

Mucormycosis is an often fatal infection caused by ubiquitous organisms of the order Mucorales. Infection is most commonly seen in the immunocompromised host, particularly in the setting of diabetic ketoacidosis. The most common presentation is with rhinocerebral involvement. We here report a case of otocerebral mucormycosis occurring in an elderly man with maturity onset diabetes who was not acidotic. The unusual site of infection delayed diagnosis until the pons had been invaded by the infecting organism which was demonstrated as a hypodense area on CT scan. Consequently radical excision of

[Lesions of round window membrane].

In 15 cases, a rupture of the round window membrane could be found via exploratory tympanotomy. The main sign in these cases was an inner ear deafness; vertigo and tinnitus were less frequent. These findings correspond well with reports in the literature. Since there is no specific clinical sign of a ruptured membrane, tympanotomy is necessary in all suspicious cases. Chances of restoration or maintenance of hearing are high in cases of early repair of the fistula, but prospects diminish as the time interval between injury and repair increases. Vertigo and tinnitus disappear even by a delayed

A distributed computer system for hierarchical control of a clinical vestibular laboratory.

A vestibular testing laboratory with five test stations was implemented as a clinical research and testing facility, automating and adapting tests for patients with inner-ear balance disorders. These were designed to quantitate postural control, and eye movement response to rotational, caloric, and visual (optokinetic) stimuli. Microprocessors were used as satellite processors in a star network under hierarchical control of a host minicomputer. All disk storage was controlled by the host, with data transfer from satellites via parallel interfaces. Network software was designed and implemented

Hair cell condition and auditory nerve response in normal and noise-damaged cochleas.

Histological and physiological data are presented from cats born and raised in a low-noise environment and from cats with long-standing, noise-induced threshold shifts. Even after a 1 1/2-year survival from acoustic trauma, there were threshold elevations of roughly 40 dB in the response of single auditory-nerve fibers which could not be correlated with significant loss of hair cells. An attempt was made to correlate these threshold differences with differences in the condition of the sensory cells as seen in a light-microscopic examination of epon-embedded surface preparations. Of all the his

Harmful effects of "aspirin compounds".

"Aspirin" is the most widely used medication which is considered to be safe and effective, and which can be obtained universally without prescription. A review of the literature and clinical experience reveal that many complications can be attributed to the prolonged use of it. Some of these complications involve damage to various tissues and organs (particularly the gastric mucosa, the renal papilla, red blood cells, and the inner ear) and to coagulation and body temperature control. Hemorrhagic gastritis may result with as little as 600 mg. of salicylate four times a day for 5 days. Papillit

Hypoplastic vestibular aqueduct in congenital temporal bone anomalies - implications for subtype diagnosis of Meniere's disease.

BACKGROUND AND PURPOSE: As the endolymphatic (ES) undergoes normal postnatal maturation, the surrounding vestibular aqueduct (VA) undergoes a corresponding change in morphology, quantified by its angular trajectory (ATVA). In adult temporal bones with Meniere's disease (MD), a fetal orientation ATVA (≥140°) indicates underlying ES hypoplasia and defines the so called hypoplastic disease endotype (MD-hp). However, ES hypoplasia has also been described histologically in other inner ear syndromes and congenital conditions, independent of MD. We aimed to investigate whether ATVA ≥140° occurs in ma

Value of CT and three-dimensional reconstruction revealing specific radiological signs for screening causative high jugular bulb in patients with Meniere's disease.

BACKGROUND: The aim of the present study was to investigate the pathological features of vestibular aqueduct (VA) related high jugular bulb (HJB) and explore the possible cause-consequence relation between HJB and endolymphatic hydrops (EH), and the potential specific radiological signs for screening causative HJB in Meniere's disease (MD). METHODS: High-resolution computed tomography (HRCT) and three-dimensional reconstruction (3DRC) were used to detect the anatomical variables associated with VA and jugular bulb (JB) in hydropic and non-hydropic ears. The presence or absence of EH in the in

Novel Use of Portable Audiometry to Track Hearing Fluctuations in Menière's Disease: A Pilot Study.

INTRODUCTION: Menière's Disease (MD) is a disorder of the inner ear consisting of episodic attacks of vertigo associated with aural fullness, tinnitus, and fluctuating hearing loss. Hearing levels in MD can often fluctuate over time, and may eventually decline permanently in a step-wise fashion. There are no current studies examining daily hearing fluctuations for prolonged periods in patients with MD. Portable audiometry has the potential to allow the patient to monitor their hearing on a daily basis without attending a center for formal audiology. The objective of this pilot study was to ass

Three-Dimensional Histopathologic Correlates of Cochlear Hypoplasia Type II Versus Incomplete Partition Type II.

HYPOTHESIS: Cochlear hypoplasia type-II (CH-II) and incomplete partition type-II (IP-II) may display measurable differences in modiolar: cochlear area ratios and interscalar ridge angles correlating with spiral ganglion neuron (SGN) counts. BACKGROUND: Inner ear malformations (IEM) can cause variable sensorineural hearing loss and cochlear implant outcomes. CH-II and IP-II share overlapping imaging features. Subtle differences in bony cochlear architecture may underlie distinct SGN patterns. METHODS: IP-II (n=11), CH-II (n=5), and normal controls (n=16) were identified from a digitised human

Is Vestibular Meniere's Disease Associated With Endolymphatic Hydrops?

Background: Vestibular Meniere's disease (American Academy of Ophthalmology and Otolaryngology, 1972) also known as possible Meniere's disease (American Academy of Otolaryngology Head and Neck Surgery, 1995) or vestibular type of atypical Meniere's disease (V-AMD) (Japan Society for Equilibrium Research, 2017) is characterized by an episodic vertigo without hearing loss. Though named as Meniere's disease (MD), this entity may not be caused solely by endolymphatic hydrops (EH). Objective: To estimate the role of EH in vestibular Meniere's disease in comparison with definite Meniere's disease. M

Involvement of Cholesterol Metabolic Pathways in Recovery from Noise-Induced Hearing Loss.

The objective of this study was to explore the molecular mechanisms of acute noise-induced hearing loss and recovery of steady-state noise-induced hearing loss using miniature pigs. We used miniature pigs exposed to white noise at 120 dB (A) as a model. Auditory brainstem response (ABR) measurements were made before noise exposure, 1 day and 7 days after noise exposure. Proteomic Isobaric Tags for Relative and Absolute Quantification (iTRAQ) was used to observe changes in proteins of the miniature pig inner ear following noise exposure. Western blot and immunofluorescence were performed for fu
