The effect of increasing temperature on skin blood flow and red cell deformability.

Using laser Doppler techniques in nine healthy volunteers, we contrasted the effect of increasing local skin temperature at the elbow, a skin site with nutritive microvasculature, and the finger pulp, with predominantly arteriovenous anastomic (AVA) perfusion). We also assessed flow at the finger dorsum, with contributions of both types of microvasculature. In parallel with the laser Doppler studies, we determined the effect of increasing temperature on the red cell deformability of our subjects, using the new technique of Cell Transit Time Analysis (CTTA). Thermal stimulation produced very la…

An Atypical Presentation of Terrible Triad Injury in a Healthy 51-Year-Old Female Patient.

A left elbow fracture-dislocation in a 51-year-old female patient following a backward fall at home presented with a comminuted radial head fracture, coronoid fracture, and ruptured collateral ligaments. The patient underwent open reduction and internal fixation (ORIF) with a long-arm splint. Post surgery, aggressive rehabilitation was initiated to restore the range of motion (ROM). Compared to previous reports, this case demonstrates successful management of a severe elbow injury with excellent short-term outcomes. This case highlights the occurrence of the terrible triad in a patient without…

Coronal Shear Fractures of the Distal Humerus: A Review of Diagnosis, Treatment, and Outcomes.

Fractures of the capitellum and trochlea are uncommon fractures of the elbow and can be challenging to treat due to their size, location, and articular nature. Because of their intra-articular nature and predilection toward displacement, these fractures are typically treated operatively. Furthermore, capitellum fractures have high rates of associated injuries, including radial head fractures or lateral collateral ligament injury in ~30% to 60% of patients. In addition to open reduction internal fixation, operative options include fragment excision, arthroscopic assisted reduction and fixation,…

[Isolated fractures of the olecranon].

Fractures of the olecranon account for 7% of fractures in adult patients. Of all elbow fractures, 38% are isolated fractures of the olecranon. Falling on the 90 degrees flexed elbow is the most common cause of isolated olecranon fractures. Reconstitution of the joint surface is the main treatment goal, as well as stability of the joint and full range of motion and muscular strength. This can only be achieved in most cases by open reduction and precision osteosynthesis. Tension band wiring and plate osteosynthesis are the most commonly used techniques. Operative therapy is the therapy of choice…

Arthroscopy and microfracture technique in the treatment of osteochondritis dissecans of the humeral capitellum: report of three adolescent gymnasts.

The aim of this paper is to report on three cases of symptomatic osteochondritis dissecans of the humeral capitellum in adolescent gymnasts, two females and one male. In all the cases arthroscopic surgery was performed. During arthroscopy, loose osteochondral fragments were removed, the defect was debrided and microfractures were performed. All the three patients regained the full range of motion of the affected elbow, and returned to the high-level gymnastics within a period of 5 months. At 12 months follow-up, all the three patients remained symptomless and were participating in high-level g…

Krukenberg kineplasty: a case study.

Functional independence has always been one of the most important goals in hand therapy. Following below-elbow amputation, this goal is often achieved by means of a prosthetic device. Unfortunately, there are rare instances in which an individual possesses an additional debilitating injury, such as blindness, that may preclude the use of a prosthetic device. Few hand therapists or surgeons actually have the opportunity to treat this type of patient. The purpose of this case study is to report the surgical methods, assessment techniques, and therapy guidelines for a 13-year-old male patient who…

Diagnosis of ulnar neuropathy: a new approach.

Conventional electrodiagnosis used to detect an ulnar neuropathy at the elbow depends on accurate determination of ulnar nerve length across this segment. We present a new approach, using the difference in latency of the compound nerve action potentials (CNAPs) of the ulnar and median nerves elicited by stimulation at the wrist and recorded 10 cm above the elbow. Sixty normal controls were examined in order to determine the normal upper limit (1.4 ms) of the difference in CNAP latency of the ulnar and the median nerves (Dlat index). Values obtained in 10 patients with ulnar nerve lesions are d…

Factors associated with removal of a radial head prosthesis placed for acute trauma.

PURPOSE: This study tests the hypothesis that there are no factors associated with removal or revision of a radial head prosthesis. A secondary analysis addressed the time to removal or revision. METHODS: We reviewed the database of two large hospitals from 2000 to 2014 and identified 278 patients that had radial head replacement after an acute fracture or fracture dislocation of the elbow: 19 had removal and 3 had revision of the radial head implant within the study period. Explanatory variables including demographics, the type of injury, prosthesis type, surgeon, medical centre, and associa…

En-Bloc separation of the distal humerus articular appendage.

The unique anatomy of the distal humerus predisposes it to coronal shear injuries of its articular appendages. These injuries often require … fracture of the distal humerus, fixed using combined lateral and medial surgical approaches to the elbow. Postoperatively, a favourable functional outcome was observed at the 13 month follow-up mark, emphasising the importance of early fixation and rehabilitation. Our case can be categorised as a rare variant of the Bryan and Morrey Type IV fracture. T …

Medial Epicondyle Fracture in Children and Its Association with Increased Carrying Angle.

BACKGROUND: This study aimed to evaluate the injury mechanism of medial epicondylar fractures in children and adolescents and its association with increased carrying angle (CA) as a predisposing factor. MATERIALS AND METHODS: We evaluated 37 patients with medial epicondylar fractures who were surgically treated at our institution. Medical records and plain radiographs were reviewed to determine the mechanism of injury and the humerus-elbow-wrist angle (HEWA) and CA of the uninjured arm. To evaluate the effect of coronal alignment on specific fracture type, we compared the CA and HEWA of the 2…

Chronic Posttraumatic Dislocation of Radial Head With Ulnar Nerve Entrapment in a Child: A Case Report and Literature Review.

We present an unusual case of chronic posttraumatic anteromedial dislocation of radial head with direct ulnar nerve entrapment in a child. Ulnar nerve decompression, open reduction of the radial head, and annular ligament reconstruction using a palmaris longus tendon graft were performed, and a satisfactory functional outcome was achieved at the 15-month follow-up. Through a review of literature, we conclude that early diagnosis and management for radial head dislocation are recommended to avoid nerve symptoms. Besides, open reduction and annular ligament reconstruction with a palmaris longus

Isolated shear fracture of the humeral trochlea in an adolescent: a case report and literature review.

This report documents a rare case of isolated trochlear shear fracture in an adolescent patient and provides a literature review. Radiographic analysis of the distal humerus revealed an intra-articular fracture of the anterior trochlea, evidenced by a full moon-shaped osteochondral fragment. Open reduction and internal fixation with two headless Herbert screws secured the fragment. Because of the paucity of information on this injury, standard treatment and management have not been established. Prospective studies with longer follow-up are needed before a consensus with regard to these fractur

Evaluation and treatment of medial ulnar collateral ligament injuries in the throwing athlete.

Medial ulnar collateral ligament (UCL) injuries are common and are seen most frequently in baseball pitchers. Appropriate recognition, treatment, and rehabilitation are necessary to ensure the best chance for return to preinjury levels of participation. Participation in competitive sports may be disrupted for 6 months to 1 year when treated optimally. Abstinence from play may be prolonged when treatment is delayed or if conservative treatment fails; this delay carries significant consequences to the professional, collegiate, and high school athlete. The orthopedic literature is replete with re

Closed-Wedge osteotomy for osteochondritis dissecans of the capitellum. A 7- to 12-year follow-up.

This article details a 7- to 12-year follow-up of seven young male baseball players with osteochondritis dissecans of the capitellum that we treated using closed-wedge osteotomy. This procedure was established by Yoshizu in 1986 for the treatment of "Little League elbow." The bone of the capitellum was revascularized and remodeled within 6 months in all seven patients. Six of the patients were able to return to full athletic activity and continued to play baseball. Radiographic assessment during the follow-up study revealed minimal osteoarthritic change and suggests that the treatment is usefu…

Supracondylar fractures of the humerus. Assessment of cubitus varus by the Baumann angle.

A simple method of radiological assessment has been used to measure the angle between the long axis of the humeral shaft and the growth plate of the capitellum (the Baumann angle). In the normal arm a significant relationship was found between the Baumann angle and the carrying angle. The Baumann angle was also measured after reduction of supracondylar fractures of the humerus and was found to correlate well with the final carrying angle measured at follow-up. There was no significant difference between the Baumann angle after reduction and that measured at follow-up; and it is suggested that
