Technique of aortic valve replacement with the Edwards stentless aortic bioprosthesis 2500.

Aortic valve replacement with a stentless device ought to result in superior hemodynamic function, because obstructing stents and sewing rims are eliminated. From 15 June 1991 to 15 October 1991, 15 patients underwent aortic valve replacement with the newly designed Edwards stentless aortic bioprosthesis 2500. Patients' ages ranged from 51 to 70 years (mean 61 years). Preoperatively 4 patients presented with aortic regurgitation, 7 with aortic stenosis and 4 with combined lesions; 7 patients were male and 8 female. No additional cardiac or noncardiac diseases were encountered. The operations w

Fracture of the Duromedics mitral valve housing with leaflet escape.

We report a case of leaflet embolisation consequent upon a hemicircumferential fracture of the pyrolytic valve housing of a 29 mm mitral Duromedics Valve implanted for seven months. The clinical presentation is discussed and the literature on mechanical valve failure compared. Unlike pivoting disc valves where absent valve clicks equate with serious occluder dysfunction, valve sounds may persist with bileaflet valve malfunction due to the unimpeded movement of the normally functioning second leaflet. Whilst intrinsic structural failure is increasingly uncommon constant clinical awareness of it

Tricuspid valve replacement using an unstented pulmonary homograft.

In consideration of the limitations of mechanical and stent-mounted tissue valves, a technique was developed for tricuspid valve replacement using an inverted pulmonary homograft. It was used successfully in a 4-year-old patient operated on for pulmonary atresia with intact ventricular septum and in a 3 1/2-year-old girl with Ebstein's anomaly.

A comparison of repair and replacement for mitral valve incompetence.

A total of 101 reparative and 389 valve replacement operations, isolated or combined with tricuspid annuloplasty or operations for coronary artery disease, were done for mitral incompetence (1975 to July 1, 1983). The patients undergoing repair as a group were younger and had less hemodynamic and functional derangement than those undergoing replacement. The prevalence of repair was less (p less than 0.001) for two surgeons than for the other four, even when possible differences in patient populations were taken into account by multivariate analysis. Five-year survival rate, including hospital

Imaging and guiding intervention for tricuspid valve disorders using 3-dimensional transesophageal echocardiography in pediatric and congenital heart disease.

In the pediatric and congenital heart disease (CHD) population, tricuspid valve (TV) disorders are complex due to the variable TV morphology, its sophisticated interaction with the right ventricle as well as associated congenital and acquired lesions. While surgery is the standard of care for TV dysfunction in this patient population, transcatheter treatment for bioprosthetic TV dysfunction has been performed successfully. Detailed and accurate anatomic assessment of the abnormal TV is essential in the preoperative/preprocedural planning. Three-dimensional transthoracic and 3D transesophageal

Early clinical outcomes of right ventricular outflow tract reconstruction with small caliber bovine jugular vein conduit (Contegra®) in small children.

This study reviewed early clinical outcomes of right ventricular outflow tract reconstruction with Contegra® valved conduits in pediatric patients. Between April 2013 and July 2014, thirteen pediatric patients underwent right ventricular outflow tract reconstruction with Contegra valved conduits. The size of the implanted conduits were 12 mm in 5 patients, 14 mm in 3, 16 mm in 3, and then 2 patients were implanted with bicuspidized conduits for downsizing the conduit to 9 and 10 mm in each. Follow-ups were completed in all patients. One conduit was explanted 7 days after a neonatal biventricul

Successful double bioprosthetic valve replacement in a 64-year-old man with carcinoid syndrome and bipolar disorder: case report.

The case is described of a 64-year-old man with bipolar disorder and severe carcinoid heart disease who required a double valve replacement. Multidisciplinary team involvement and extensive preoperative investigations resulted in a successful regimen which prevented reactivation of carcinoid syndrome and avoided the serious side effects of lantreotide and lithium therapy. In addition, two bioprosthetic valves were used, thereby avoiding the potential complications of anticoagulation in a patient with known hepatic metastases.

[Heart autotransplantation for the treatment of a rhabdomyosarcoma of the left ventricle. Report of one case].

Heart autotransplantation is an exceptional surgical technique used in the treatment of uncontrolled cardiac arrhythmias and primary unresectable cardiac tumors. We report a 28-year-old male with a rhabdomyosarcoma of the left ventricle, localized in the lateral and posterior wall, which involved the mitral valve and circumflex artery. After a complete study ruling out dissemination of the tumor, the patient was operated. Surgical exploration determined the unresectability of the tumor with the heart in situ. Therefore, the heart was explanted, preserving the right atrium and coronary sinus fo

[Association of an interauricular communication of the sinus venosus type with obstructive myocardiopathy. Surgical treatment].

The authors report the case of a 46 year old man in congestive cardiac failure for 4 years due to the association of a sinus venosus atrial septal defect and hypertrophic obstructive cardiomyopathy. Surgery was attempted because of the poor response to medical therapy. The operation consisted in a trans aortic left ventricular myotomy with mitral valve replacement, enlargement of the atrial septum by a patch and correction of the sinus venosus also by a patch. The result was excellent, with regression of the clinical signs of cardiac failure, maintained after 6 months' follow-up. This is certa

Origin and clinical relevance of musical murmurs.

Although our observations are limited to studies performed on a degenerated bioprosthetic valve that produced a musical murmur, we believe that they can be applied to musical murmurs caused by abnormal natural valves. Several points regarding the characteristics of musical murmurs have been clarified. A musical murmur results from a uniform periodic vibration of a cardiac structure. A non-musical murmur results from turbulent blood flow which initiates random vibrations of adjacent structures. The broad spectrum of frequency of a non-musical murmur reflects the broad range of random fluctuatio

Infective endocarditis following transcatheter edge-to-edge mitral valve repair: A systematic review.

OBJECTIVES: To assess the clinical characteristics, management, and outcomes of patients diagnosed with infective endocarditis (IE) after edge-to-edge mitral valve repair with the MitraClip device. BACKGROUND: Transcatheter edge-to-edge mitral valve repair has emerged as an alternative to surgery in high-risk patients. However, few data exist on IE following transcatheter mitral procedures. METHODS: Four electronic databases (PubMed, Google Scholar, Embase, and Cochrane Library) were searched for original published studies on IE after edge-to-edge transcatheter mitral valve repair from 2003

Hemodynamic and clinical impact of prosthesis-patient mismatch after transcatheter aortic valve implantation.

OBJECTIVES: This study examined the mid-term hemodynamic and clinical impact of prosthesis-patient mismatch (PPM) in patients undergoing transcatheter aortic valve implantation (TAVI) with balloon-expandable valves. BACKGROUND: PPM can be observed after aortic valve surgery. However, little is known about the incidence of PPM in patients undergoing TAVI. METHODS: Echocardiography and clinical assessment were performed in 165 patients at baseline, before hospital discharge, and at 6 months after TAVI. PPM was defined as an indexed effective orifice area ≤0.85 cm(2)/m(2). RESULTS: Thirty pati

Serial Changes in Right Ventricular Outflow Tract Gradient and Endocarditis Risk After Transcatheter Pulmonary Valve Replacement.

BACKGROUND: Infective endocarditis (IE) after transcatheter pulmonary valve replacement (TPVR) remains a significant concern. Although there are overall good short-term and long-term hemodynamic outcomes, limited data exist comparing postimplantation echocardiographic findings leading to IE. OBJECTIVES: To compare the progressive echocardiographic performance of percutaneous implanted pulmonary valves before the development of IE. METHODS: This is a single-center retrospective cohort study of all patients who underwent successful TPVR using either Medtronic Melody or Edwards Sapien valves fr

Type and Size of Implanted Bioprosthetic Valve Rather Than Intraoperative Peak Transprosthetic Valvular Velocity Predict Postoperative Midterm Prosthesis-Patient Mismatch in Patients Undergoing Surgical Aortic Valve Replacement.

OBJECTIVES: High transprosthetic valvular peak velocity (PV) is indicative of prosthesis-patient mismatch (PPM), which exacerbates mortality and morbidity after surgical aortic valve replacement (AVR). During surgical AVR, a high intraoperative PV sometimes is detected, but whether it affects mortality and morbidity is unknown. The aims of this study were to determine whether intraoperative and postoperative PV were correlated and what factors predicted postoperative PPM. DESIGN: Retrospective, observational, cohort study. SETTING: Tertiary medical center. INTERVENTIONS: None. MEASUREMENTS

Early experience with Shelhigh stentless composite valve. A complete biological replacement of ascending aorta.

BACKGROUND: Currently, Shelhigh Stentless Composite Valve (SSCV) is the only complete biological conduit available on the market for this purpose. SSCV has been used to avoid anticoagulation with the advantage of established haemodynamic benefits of stentless valves as well as for its surgeon-friendly surgical implantation. METHODS: Between August 1999 and January 2001, 11 patients (8 women, 3 men, mean age 71.5+/-9.3 years) underwent first time aortic root and ascending aorta replacement with SSCV in Leicester. Aortic regurgitation was found in seven patients and aortic stenosis was predomin

Impact of additional tricuspid valve annuloplasty in TOF patients undergoing pulmonary valve replacement.

BACKGROUND: Many patients with tetralogy of Fallot (TOF) who underwent surgical correction of their congenital cardiac malformation during infancy develop right ventricular dysfunction and exercise intolerance in the long term. The right ventricle (RV) dilates due to the development of severe pulmonary regurgitation (and secondary tricuspid insufficiency). To reduce RV dilation and improve exercise tolerance pulmonary valve replacement (PVR) is the common therapeutic strategy. Whether concomitant tricuspid valve repair (TVR) is beneficial in these pure volume-overload conditions is still unkno

Mitral Valve Procedures and Multivessel CABG through a Single Left Anterior Minithoracotomy.

We describe a technique for concomitant coronary artery bypass grafting (CABG) and mitral valve (MV) replacement or annuloplasty with ring implantation performed through a single left anterior minithoracotomy (LAmT). Four patients underwent combined MV and CABG surgery using peripheral cardiopulmonary bypass, a transseptal approach to the MV, and complete coronary revascularization. MV exposure was successfully achieved in all cases without conversion to sternotomy. No major complications such as stroke, reoperation for bleeding, or early mortality occurred. Our initial results demonstrate tha
