Benefit-risk profile comparison between dupilumab and upadacitinib: a structured benefit-risk assessment of the Heads Up trial.

OBJECTIVES: Structured benefit-risk assessments evaluate multiple patient-relevant endpoints simultaneously, weighing therapeutic benefits against risks, enabling healthcare professionals and patients to select the most suitable treatment. Such assessments evaluating therapeutic options for atopic dermatitis (AD) have not yet been published. We performed a structured benefit-risk assessment based on multicriteria decision analysis, using data from the 24-week Heads Up trial comparing dupilumab versus upadacitinib in adults with moderate-to-severe AD. METHODS: A benefit-risk score (0-100) meas

Pharmacokinetic Optimization of Radiocopper-Based Theranostic Pretargeting.

In vivo pretargeting offers a strategy to improve nuclear imaging and radiopharmaceutical therapy by increasing tumor-to-background activity concentration ratios and decreasing radiation burden to healthy tissues. One particularly promising approach to in vivo pretargeting is predicated on the inverse electron-demand Diels-Alder (IEDDA) ligation between tetrazine (Tz)-based radioligands and trans-cyclooctene (TCO)-bearing immunoconjugates. Not surprisingly, the performance of such systems is highly dependent upon the pharmacokinetic profiles of the small molecule radioligands. Herein, we repor

Nintedanib increases BCL-2 in fibrotic fibroblasts, enhancing ABT-199 apoptosis and fibrosis resolution.

RATIONALE: Progressive fibrosing interstitial lung diseases (PF-ILDs) have a large global impact and there is a pressing need to develop new therapies. OBJECTIVES: We investigate if nintedanib augments apoptosis of profibrotic fibroblasts induced by BCL-2 inhibition with ABT-199, and if combination therapy reverses pulmonary fibrosis. METHODS: Healthy and PF-ILD fibroblasts were treated with nintedanib, and BCL-2 family member expression was measured. Fibroblasts and precision-cut lung slices were analyzed for apoptosis after ABT-199 and nintedanib treatment. Therapeutic interventions with A

Liposomal-Mitoxantrone Overcomes Acquired Resistance and Enhances Venetoclax Synergy in AML-Mechanistic Insights and Preliminary Clinical Evidence.

INTRODUCTION: Liposomal drug delivery has emerged as a promising strategy to improve the efficacy and safety of chemotherapeutic agents, yet whether and how liposomal reformulation fundamentally reshapes therapeutic response and resistance evolution remains unclear. This study compared liposomal mitoxantrone with free mitoxantrone and examined their effects on antileukemic activity, hematologic toxicity, resistance development, and cell-death pathways. METHODS: Liposomal and free mitoxantrone were evaluated in an MLL-AF9-driven acute myeloid leukemia mouse model by survival analysis and perip

Virtual screening and cellular validation of dolutegravir as a BRD9 inhibitor for attenuating pyroptosis in peritoneal mesothelial cells.

BACKGROUND: Peritoneal dialysis is an essential therapy for end-stage renal disease; however, long-term exposure to high-glucose dialysis solutions induces pyroptosis of peritoneal mesothelial cells, promoting peritoneal fibrosis and ultimately leading to technique failure. The involvement of the epigenetic regulator bromodomain-containing protein 9 (BRD9) in this process remains unclear. METHODS: Structure-based virtual screening of 3,447 FDA-approved drugs from the ZINC database identified dolutegravir as a candidate BRD9 inhibitor. Direct binding and inhibitory activity were validated usin

Oral CXCR4 inhibition with mavorixafor: Emerging therapeutic applications in WHIM syndrome, chronic neutropenia, oncology, and stem cell mobilization.

The CXCR4/CXCL12 signaling axis plays a central role in regulating immune cell trafficking, hematopoietic homeostasis, and organogenesis. However, dysregulation of this axis contributes to the pathogenesis of numerous disorders, highlighting CXCR4 inhibition as a promising therapeutic strategy. Mavorixafor, the first orally available small-molecule CXCR4 antagonist, recently received FDA approval for WHIM syndrome (Warts, Hypogammaglobulinemia, Infections, and Myelokathexis) and is currently being developed for additional indications. Despite extensive research on CXCR4 biology, a comprehensiv

Lingering sex and age disparities in dolutegravir uptake among adults with HIV: a multicountry observational cohort study.

INTRODUCTION: Since July 2019, the WHO has recommended dolutegravir (DTG)-based regimens as preferred first-line antiretroviral therapy (ART) for adults and adolescents living with HIV (DTG-for-All), a reversal of a 2018 safety alert on use of DTG-based regimens by women of reproductive age (WRA). We examined sex and age disparities in DTG uptake before and after DTG-for-All in the International epidemiology Databases to Evaluate AIDS. METHODS: We included patients ≥16 years on or initiating treatment between January 2017 and July 2021 in 14 low- and middle-income countries where initial guid

CXCR4 Blockade Alleviates Pulmonary and Cardiac Outcomes in Early Chronic Obstructive Pulmonary Disease.

Chronic obstructive pulmonary disease (COPD) is a prevalent respiratory disease lacking effective treatment. Focusing on early COPD should help to discover disease modifying therapies. We examined the role of the CXCL12/CXCR4 axis in early COPD using human samples and murine models. Blood samples and lung tissues from both individuals with early COPD and controls were analyzed for CXCL12 and CXCR4 levels. To generate an early-like COPD model, 10-week-old male C57BL/6J mice were exposed to cigarette smoke for 10 weeks and intranasal instillations of polyinosinic-polycytidylic acid (poly(I:C)) f
