[Neurobiological correlates of EMDR therapy].

The EEGs in a group of ten subjects with major psychological trauma treated with EMDR and in ten controls have been registered both during the listening of the autobiographical narrative of the index trauma (script) and during a whole EMDR session. The EEGs have been performed again during the last EMDR session when patients were free of symptoms. During script listening a prevalent activation of the limbic regions corresponding to prefrontal and orbitofrontal cortex has been registered, being explained as the emotional arousal during trauma reliving at the symptomatic phase. The significant d

The functional anatomy of Gilles de la Tourette syndrome.

Gilles de la Tourette syndrome (GTS) holds a prime position as a disorder transgressing the brittle boundaries of neurology and psychiatry with an entangling web of motor and behavioral problems. With tics as the disorder's hallmark and myriads of related signs such as echo-, pali- and coprophenomena, paralleled by a broad neuropsychiatric spectrum of comorbidities encompassing attention deficit hyperactivity disorder, obsessive-compulsive disorder and self-injurious behavior and depression, GTS pathophysiology remains enigmatic. In this review, in the light of GTS phenomenology, we will focus

Subclinical cerebellar impairment in the common types of migraine: a three-dimensional analysis of reaching movements.

Mutations in the CACNA1A gene can cause familial hemiplegic migraine (FHM) and/or cerebellar ataxia CACNA1A codes for the alpha1 subunit of P/Q-Ca2+ channels and is highly expressed in the cerebellum. Using a pointing paradigm and infrared optoelectronic tracking system, we found subclinical hypermetria and other subtle cerebellar signs in the common forms of migraine. These were more pronounced in migraine with than without aura. Whether this reflects involvement of Ca2+ channel genes in the common types of migraine needs to be investigated by genetic analyses.

[A case of Guillain-Barré syndrome associated with bilateral ballism: an overlap between Fisher's syndrome and Guillain-Barré syndrome].

A 29-year-old woman developed diplopia and ataxic gait. Neurological examination revealed total ophthalmoplegia, cerebellar ataxia, and areflexia. Muscle weakness in four limbs was also noted. We diagnosed the patient as having an overlap between Fisher's syndrome and Guillain-Barré syndrome. On day 5, she suddenly had flinging involuntary movements affecting the face and four extremities. Surface EMG recording revealed 1.5- 2 Hz rhythmic grouping discharges. The involuntary movements were considered as ballism. Brain CT and MRI could not detect any abnormal findings in subthalamic nucleus, bu

Clinicophysiological features of akinesia.

Among the motor tetrad of Parkinson's disease, akinesia is not easy to define and controversies still exist on the nature of akinesia. In 1972 Barbeau first described pure akinesia without rigidity or tremor responsive to L-dopa therapy, that is, akinesia due to striatal dopamine deficiency. Since 1974, Imai and Narabayashi have described a different type of pure akinesia, unresponsive to L-dopa treatment. This new condition exhibits only the freezing symptom, which is a breakdown of repetitive voluntary movements emerging through festination or suddenly, e.g., freezing of gait, micrographia a

Common gait abnormalities of the knee in cerebral palsy.

Gait abnormalities in children with cerebral palsy are the consequence of contractures across joints, muscle spasticity, and phasically inappropriate muscle action. Though abnormalities involving one of the major joints of the lower extremity will usually have consequences on the function of the other joints, it is possible to recognize certain primary disorders at each joint. The most common gait abnormalities of the knee in patients with cerebral palsy occur in the sagittal plane. Based on the experience gained from performing gait analysis on more than 588 patients with cerebral palsy, four

[Palato-pharyngo-laryngeal myoclonus].

Palato-pharyngo-laryngeal myoclonus is well-known clinical criterion characterized by persistent, rapid, repetitive, rhythmic muscle twitch the rate of which ranges 50-200 Hz. It appears in the palate, pharynx, larynx, diaphragma, and the ocular or limb muscles, but the most common combination of the distribution is palate-pharyngo-laryngeal type or only palatal type. In most instances the movement is synchronous, but others show asynchronous myoclonus at different rates. There was a case report in which an electrical stimulation given at the ulnar nerve produced reset of myoclonus indicating

[A case of hemiballismus during cryptococcal meningitis].

We present a case of hemiballismus related to cryptococcal meningitis. A 23 year-old man was hospitalized because of involuntary movements of his left side, confusion, hyperpyrexia, neck stiffness, bilateral papilledema, right hemiparesis and bilateral pyramidal signs. Diagnosis was made by CSF examination demonstrating cryptococci by india ink. CT with contrast showed hyperdense lesions in the head of the right caudate nucleus, in the left internal capsule and in the frontal and occipital lobes. After treatment with amphotericin B, 5-fluorocytosine and haloperidol, he experienced rapid recove

Occurrence of sequelae in Bell's palsy.

The occurrence of sequelae in 330 patients with Bell's palsy followed up for more than 6 months was investigated. At least one of five kinds of sequelae (synkinesis, crocodile tears syndrome, spasm, contracture, tinnitus and/or hearing loss during facial movement) was observed in 30 of the 330 cases (9.1%). Sequelae occurred at 23.9-39.0 weeks (mean) after onset of palsy and at scores of 30-34 points (mean; full score 40 points). The incidence of sequelae was correlated to the degree of facial nerve dysfunction.

Camptocormia, a new side effect of sodium valproate.

A mentally retarded 23-year-old woman with myoclonic astatic epilepsy developed an abnormal posture of extreme forward flexion, called camptocormia, during valproate monotherapy. Camptocormia occurred concomitantly with an increase of the plasma valproate level from about 330 mumol/l up to 530 mumol/l. Four weeks earlier she had developed hepatopathy during a regimen of carbamazepine, lynestrenol and sodium valproate. Hepatopathy subsided after the cessation of carbamazepine and lynestrenol. Camptocormia improved within a week after the reduction of the plasma valproate level to about 300 mumo

Cervical radiculopathy. A review.

This paper is a review of current information on cervical radiculopathy. The focus is on the natural history of the process, the accuracy of diagnostic tests to determine etiology, the differential diagnosis involved, and the surgical tactics available. The most important conclusions are the criteria for patient selection for surgery and a statistical demonstration of the adequacy of both the anterior and the posterior approaches for decompression of the cervical nerve root.

Lagophthalmos in acquired oculomotor synkinesis.

Two consecutive patients who exhibited classic signs of unilateral acquired oculomotor synkinesis demonstrated an inability to appose eyelid margins on the affected side, when attempting gentle closure of the eyes. In one patient, this occurred in conjunction with a unilateral upper eyelid crease on the affected side, and in the other patient it was associated with accentuation of the crease in comparison with that of the opposite side. Bilateral eyelid closure normally induces a contraction of the superior recti muscles. The lagophthalmos and accentuated upper eyelid crease observed in our pa

Pharmacokinetics of red blood cell phenothiazine and clinical effects. Acute dystonic reactions.

Pharmacokinetics of the phenothiazine, butaperazine, were studied in relationship to acute dystonic reactions. Dystonias appeared on falling drug concentrations, more than one half-life after plasma and red blood cell (RBC) peak butaperazine concentrations. Red blood cell butaperazine kinetics differentiated better than did plasma butaperazine levels those subjects in whom dystonias would develop from those in whom they did not. We conclude that RBC phenothiazine levels may more clearly reflect drug concentration at critical brain sites than do simple plasma drug levels. Furthermore, dystonic
