Spinal cord reserve is associated with multi-dimensional clinical resilience in multiple sclerosis.

OBJECTIVE: Spinal canal area (SCaA)-a proxy for maximal lifetime spinal cord growth-has recently been associated with concurrent disability and future worsening in multiple sclerosis (MS). Prior studies focused mainly on the Expanded Disability Status Scale (EDSS). This study evaluates whether SCaA also relates to broader clinical outcomes. METHODS: We performed a retrospective longitudinal study at the Belgian National MS Center. SCaA and spinal cord area (SCoA) were quantified on 714 brain MRI scans from 426 patients with MS using newly developed deep learning models. Two approaches were ap…

Proinflammatory Epstein-Barr Virus Antibody Functions Track with Disease Activity in Multiple Sclerosis.

OBJECTIVE: Epstein-Barr virus (EBV) has been implicated in the pathogenesis of multiple sclerosis (MS). Elevated immunoglobulin G (IgG) titers against EBV nuclear antigen 1 (EBNA1) represent the most consistent serological marker of MS risk, with levels remaining persistently elevated following disease onset. METHODS: We conducted high-dimensional profiling of virus-specific antibody-associated immune features in 60 individuals with relapsing-onset MS followed over time and in 54 age- and sex-matched EBV-seropositive healthy controls. RESULTS: EBNA1-specific IgG in MS patients exhibited a di…

Circulatory immune cell counts and clinical outcomes in multiple sclerosis relapse versus remission.

Multiple sclerosis (MS) relapse diagnosis is impeded by 'pseudo-relapses' whilst relapse treatment lacks specificity. Before we can improve clinical care, we must first improve our understanding of relapse pathogenesis. Currently, the circulatory immune mechanisms underpinning relapse are poorly understood. We aimed to determine changes in circulatory cell counts from people with MS during relapse versus remission and their association with clinical outcomes. Data was collected retrospectively by screening 2316 patient files through which we identified 78 episodes of MS relapse and remission. …

Epstein-Barr virus and multiple sclerosis: from associations to mechanisms to potential therapies.

BACKGROUND: Epstein-Barr virus, a widespread herpesvirus that establishes lifelong latency after primary infection, has emerged in serological and epidemiological studies as a strong and consistent risk factor for multiple sclerosis. RECENT DEVELOPMENTS: Emerging evidence indicates that Epstein-Barr virus can interact with several multiple sclerosis susceptibility loci to modify the transcriptional programmes and differentiation trajectories of B cells. Specifically, Epstein-Barr virus infection has been associated with the expansion of neuroinvasive, atypical B cells, which are enriched in i…

[Digital transformation in multiple sclerosis: Advances in diagnostics, monitoring and patient-centred care].

Digital transformation is fundamentally changing the diagnosis, monitoring and treatment of multiple sclerosis. The integration of multimodal data from imaging, laboratory tests, clinical assessments, patient-reported outcomes and continuous measurements via wearables is creating high-resolution, longitudinal profiles of disease progression. Based on this data, modern analysis methods and artificial intelligence enable predictive models for disease activity, progression and therapeutic response, supporting personalised decision-making. Digital patient pathways and patient portals open up new o…

Assistive device prescriptions in multiple sclerosis: a cross-sectional study of disability distribution.

PURPOSE: Multiple sclerosis (MS) is a progressive neurological disease often leading to functional decline. This study aimed to assess disability distribution-based on assistive aid needs-for MS patients within the Local Health Authority (ASP) of a metropolitan urban area of Italy over six months. MATERIALS AND METHODS: A cross-sectional analysis was conducted on 600 prescriptions for 133 MS patients between January and July 2025. Data were stratified by demographics, healthcare district, device type and prescribing physicians. Chi-squared tests assessed statistical associations (p < 0.05). …

Evidence of musculoskeletal pain sensitization in patients with multiple sclerosis: Case-control study.

BACKGROUND: Chronic musculoskeletal pain is a common symptom in individuals with multiple sclerosis (MS), however the underlying pain mechanisms remain complex and not fully understood. Therefore, this study aims to investigate the underlying pain mechanisms using different quantitative sensory testing (QST) and other self-reported measures compared to healthy controls. METHODS: A total of 133 participants (93 MS patients and 40 healthy controls) were included. Participants completed self-reported measures including brief pain inventory (BPI), pain sensitivity questionnaire (PSQ), painDETECT …

Language Model Embedding Classifiers Enable Identification of Multiple Sclerosis-Associated BCRs and Repertoires.

Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease. It affects over 2 million people worldwide but has historically been challenging to diagnose, categorize, and treat. MS has an autoimmune component that involves the production of B-cell receptors (BCRs) and immunoglobulins (Igs) that are associated with disease pathophysiology. We reanalyzed all publicly available RNA sequencing data from MS patients and extracted over 11 million BCR immunoglobulin heavy chain (IGH) sequences obtained from a variety of tissue sources. We developed a new decoder-only BCR DNA embedding mod…

Insights from a Danish nationwide implementation of patient-reported outcomes in multiple sclerosis.

BACKGROUND: Multiple sclerosis (MS) monitoring often relies on traditional methods like the expanded disability status scale (EDSS) and magnetic resonance imaging, which may not fully reflect patients' health. Patient-reported outcomes (PROs) provide valuable insights into patients' experiences of their disease. OBJECTIVES: This study evaluated the Danish implementation of PROs for people with MS (PwMS) 3 years post-launch, comparing clinical and demographic characteristics between PRO users and non-users, and assessing PRO adherence. RESULTS: As of 1 January 2025, 2121 PwMS had signed up fo…

Possible Contribution of Multiple Sclerosis Pathology to Treatment-Resistant Schizophrenia: A Case Study.

Multiple sclerosis (MS) can present with neurological and psychiatric symptoms that may overlap with those of schizophrenia, making differentiation difficult particularly because their typical onset ages are similar. We present the case of a woman in her 50s who developed persecutory delusions and a subjective sense of being under surveillance in Year X-31 and was diagnosed with schizophrenia. Despite the long-term antipsychotic treatment, the patient's symptoms persisted and worsened. In Year X-1, brain magnetic resonance imaging revealed multiple white matter lesions, and then cerebrospinal …

Choroid Plexus MRI Features and Cognitive Outcomes in Multiple Sclerosis: A Scoping Review.

BACKGROUND/OBJECTIVES: Multiple sclerosis (MS) is frequently accompanied by cognitive impairment, yet the neurobiological mechanisms underlying cognitive heterogeneity remain incompletely understood. The choroid plexus (CP), a blood-CSF barrier structure involved in cerebrospinal fluid production and neuroimmune signaling, has recently emerged as a potential MRI marker of inflammatory and neurodegenerative activity in MS. This scoping review mapped evidence on associations between CP features and cognitive functions in adults with MS. METHODS: A broad search of PubMed, Scopus, Web of Science …

Temporal trends and regional disparities in multiple sclerosis burden in Italy, 1990-2023.

BACKGROUND: Multiple sclerosis (MS) is a major cause of chronic neurological disability, and its burden has shifted from premature mortality toward long-term disability. Italy, with its marked north-south gradients and the exceptional epidemiology of Sardinia, offers a unique setting to examine whether this transition has occurred uniformly across regions and sexes. METHODS: Using Global Burden of Disease 2023 subnational estimates, we analysed MS burden in Italy from 1990 to 2023 across five macro-areas. Age-standardized disability-adjusted life years (DALYs), years of life lost (YLLs), year…

High-throughput primary B cell cytokines assay for drug screening in multiple sclerosis.

BACKGROUND: Remarkable success of anti-CD20 B cell depleting therapies in multiple sclerosis (MS) highlights antibody-independent role of B cells in driving inflammatory relapsing-remitting stage of the disease. Yet, long-term B cell depletion leads to increased infections, pointing to the need for more targeted treatments. OBJECTIVES: To establish a high-throughput translational assay of primary B cells for drug screening. RESULTS: We present an assay of primary B cells isolated from MS patients and healthy individuals. We tested various methods of B cell isolation, compared impact of combi…
