Small fiber pathology in patients with periodic limb movement during sleep.

OBJECTIVE: Periodic leg movements during sleep (PLMS) characterize a common movement disorder occurring at nighttime in comorbidity with several neurological conditions. Its pathogenesis is still a puzzling issue. We aimed at exploring the presence of small fiber pathology (SFP) in patients with PLMS. METHODS: We examined skin punch biopsies from the distal and proximal sites in 22 patients with periodic limb movement disorder (PLMD) (17 males, with a mean age of 58.50 ± 13.3), without predisposing causes for peripheral neuropathy, and 20 age- and sex-matched healthy controls (HCs), quantifyi…

Proteomic profiles in inclusion body myositis and polymyositis with mitochondrial pathology.

BACKGROUND: Idiopathic inflammatory myopathies (IIMs) are autoimmune muscle diseases with distinct clinical, histopathological, and molecular features. Among them, inclusion body myositis (IBM) is refractory to immunotherapy and characterized by combined inflammatory and degenerative changes. Polymyositis with mitochondrial pathology (PM-Mito) has been proposed as a prodromal stage of IBM, but molecular profile underlying this spectrum remains poorly defined. METHODS: Skeletal muscle biopsies from 38 IBM, 14 PM-Mito, 5 anti-synthetase syndrome (ASyS), 3 dermatomyositis (DM), 5 immune-mediated …

Evolving cerebrovascular pathology in fibromuscular dysplasia leading to aneurysmal subarachnoid hemorrhage.

Fibromuscular dysplasia (FMD) is a rare, nonatherosclerotic vascular disorder characterized by arterial stenosis, aneurysm formation, dissection, and tortuosity. It predominantly affects women and commonly involves the renal and internal carotid arteries (ICA). While classically associated with renovascular hypertension, cerebrovascular involvement may cause headache, neck pain, or, rarely, intracranial aneurysm and subarachnoid hemorrhage (SAH). We report a case of progressive FMD in which 3 sequential imaging findings drove the diagnostic course: an initial cervical ICA string-of-beads appea

Inorganic phosphate beyond a mineral: Signal transduction in multi-organ pathology.

Inorganic phosphate (Pi) is a fundamental cellular metabolite whose excess triggers active signalling cascades that drive pathological remodelling across multiple organ systems. This review synthesises evidence from vascular, renal and oncological research to reveal how hyperphosphataemia deregulates eight conserved signalling pathways-AMPK, ERK1/2, HIF-1α, JAK-STAT, KEAP1/NRF2/p62, NF-κB, TGF-β1 and Wnt/β-catenin-and how these converge on shared regulatory hubs to produce vascular calcification, renal fibrosis and tumour progression. Phosphate overload suppresses the cytoprotective kinase AMP

Surgical Management of Dentigerous Cyst: A Silent Pathology With Destructive Potential.

Dentigerous cysts are common odontogenic cysts associated with the crowns of unerupted teeth and are often detected incidentally due to their asymptomatic nature. This case series presents three patients with radiolucent lesions associated with impacted teeth, all of which were clinically silent yet demonstrated significant cystic expansion on radiographic examination. Surgical enucleation of the cysts along with the extraction of the associated impacted teeth was performed in all cases, and histopathological analysis confirmed the diagnosis of dentigerous cyst. Postoperative follow-up reveale

Spatiotemporal characterization of Alzheimer disease pathology in living human brain tissue.

Alzheimer disease begins in the brain many years before symptoms, but early changes are usually studied after death or indirectly through fluid and imaging biomarkers. Here we show that small brain biopsies collected during ventriculoperitoneal shunt surgery can be used to study Alzheimer disease pathology and protein turnover during life. We analysed biopsies from 18 individuals with suspected normal pressure hydrocephalus, alongside matched ventricular and lumbar cerebrospinal fluid and post-mortem control brain tissue. Amyloid plaques were present in 9 of 18 biopsies, while mature tau tangl…

Astrocyte-Predominant Tau Pathology in a Patient With VCP R191Q Variant.

Variants in the valosin-containing protein (VCP) gene cause multisystem proteinopathy, typically characterized by TDP-43 pathology. However, there are few reports describing tau pathology in VCP variant carriers. Here, we report a Japanese woman carrying a heterozygous VCP R191Q variant, who developed progressive muscle weakness, frontotemporal dementia, and parkinsonism beginning in her mid-forties and died at the age of 61. Neuropathological examination demonstrated FTLD-TDP type D pathology involving the frontal and temporal cortices, limbic structures, brainstem, spinal cord, and skeletal …

Neurovascular large artery dilatation increases the risk for Alzheimer's disease pathology.

Alzheimer's disease and related dementias are increasing globally, highlighting the urgent need to clarify disease mechanisms to improve diagnosis and treatment. Vascular alterations are a major pathological feature of Alzheimer's disease. Beyond the established contributions of small vessel disease and large artery atherosclerosis, our group has shown that dilatation of large cerebral arteries is associated with an increased risk of dementia and Alzheimer's pathology. The most severe manifestation of this non-atherosclerotic vascular phenotype is dolichoectasia, characterized by abnormal enla…

Challenges in umbilical pathology: far beyond the obvious-a case report.

BACKGROUND: Umbilical disorders in neonates are frequently benign, with umbilical granulomas, hernias, and transient inflammatory changes representing the majority of presentations. However, persistent or atypical umbilical findings may signal rare congenital anomalies originating from incomplete involution of the urachus or the omphalomesenteric duct. Early recognition of these conditions is crucial to avoid misdiagnosis, unnecessary antibiotic exposure, or delayed referral. CASE DESCRIPTION: We describe a 27-day-old term newborn, previously healthy, who initially presented with a single ves

Sulfated glycosaminoglycans inhibit LCMV entry and modulate antiviral immunity and pathology.

Viral infections remain a major challenge due to the limited availability and efficacy of current treatments. Existing antivirals primarily … immune activation, leading to diminished T-cell responses, prolonged infection, and increased immunopathology. By contrast, administering dextran sulfate during the acute infection phase decreased viral load, improved effector T-cell function, and reduced liver pathology. These findings highlight the therapeutic potential of sulfated GAGs against Arenavirus …

STING agonists reduce genital chlamydial infection and attenuate inflammation and pathology.

The cGAS-STING pathway is activated during chlamydial infection and plays a critical role in controlling Chlamydia trachomatis infection in the mouse lower genital tract. The current study evaluated whether intravaginal administration of exogenous STING agonists could inhibit chlamydial infection and attenuate upper genital tract pathologies in a mouse model of Chlamydia muridarum infection. We found that both the STING agonists 2'3'-cGAMP and diABZI significantly reduced the shedding of live chlamydial organisms recovered from the vaginal swabs and markedly decreased hydrosalpinx and inflamma

Vulvar Squamous Cell Carcinoma: Scientific Progress and Impacts to Pathology Practice.

Despite vulvar cancer's rarity, meaningful advances have been made to improve the clinical care of affected patients-particularly with respect to vulvar squamous cell carcinoma (VSCC). These advances are reflected in changes to tumor classification, surgical strategies, early detection of precursors, and targeted therapeutic exploration. Refinements in histopathologic and molecular classification have enhanced diagnostic precision, enabling more accurate prognostication and facilitating personalized therapeutic decision-making, particularly tailored surgical strategies that can be explored thr

VEGFR Blockade Reduces Mycobacterium tuberculosis-Induced Lung Pathology in Immunocompromised Mice.

Mycobacterium tuberculosis (Mtb) remains a significant public health threat, responsible for 1.6 million deaths in 2021. The development of new treatments is particularly urgent for immunocompromised individuals, including those with Mtb/HIV coinfection, who experience severe disease outcomes. Previous studies demonstrated that blockade of VEGFR1, a receptor expressed on monocytes that mediates their recruitment to infection sites, limits Mtb-induced pathology in immunocompetent mice of both Mtb-resistant (C57BL/6J) and Mtb-susceptible (B6.C3H-sst1) strains. The present study extends these fin…

Pathology-validated prevalence and clinical characteristics of coronary medial arterial calcification.

AIMS: Medial arterial calcification (MAC) is well characterized in peripheral arteries, but its pathology-validated prevalence in the coronary circulation remains uncertain. Because intravascular imaging cannot routinely delineate the elastic laminae in advanced atherosclerotic lesions, deep intimal calcification (DIC) accumulating adjacent to the internal elastic lamina (IEL) may fundamentally mimic MAC. This study aimed to determine the strict pathology-validated prevalence of true coronary MAC and morphologically characterize DIC as a potential imaging mimic using a large-scale autopsy regi…

Beyond Personal Pathology: Disability and Suicidality among Indigenous Peoples in Canada.

Suicide rates among Indigenous Peoples in Canada remain significantly higher than those in non-Indigenous populations, yet explanations have mostly focused on individual and clinical factors. This study offers a sociological perspective by framing suicidality as an anomic response to structural frustration and strain caused by settler colonialism. In this view, patterns of suicidality, like patterns of drug use, are seen not as symptoms of personal pathology but as effects of structural harm experienced by Indigenous communities. Using data from the 2017 Aboriginal Peoples Survey, the analysis…

IRAP deficiency preserves renal function and attenuates pathology in aged mice.

Senescent cell accumulation and fibrosis play a fundamental role in kidney ageing. Our aim was to determine whether the angiotensin IV/insulin-regulated aminopeptidase (IRAP) axis of the renin-angiotensin system contributes to renal function decline, fibrosis, and senescent cell accumulation in aged mice. We studied the role of IRAP in kidney ageing using IRAP knockout (KO) mice, as well as in wild-type (WT) mice treated with an IRAP inhibitor. Complementary proximal tubule and collecting duct cell models were used to determine the mechanisms by which IRAP inhibition exerts protective effects.

CTLA-4 haploinsufficiency leads to late-onset autoimmune pathology in mice.

In humans, the loss of function in a single copy of the CTLA4 gene leads to an immune disorder characterized by a spectrum of immunodeficient and autoimmune manifestations. To investigate the pathogenicity of CTLA-4 haploinsufficiency, we utilized a heterozygous mouse model carrying a patient-derived Y139C mutation. We found that young heterozygous mice exhibited lymphoproliferative features and an activated T cell phenotype, without displaying autoimmunity. Co-culture assays revealed impaired trans-endocytosis. Despite this dysfunction, they remained resistant to induced inflammation. With ag

[Therapeutic Targets by Pathology AI and Spatial Transcriptomics in Breast Cancer].

Recent advances in pathology foundation models have markedly improved the accuracy and generalizability of histopathological image analysis in breast cancer. However, the mechanisms of resistance to CDK4/6 inhibitors in hormone receptor-positive, HER2-negative advanced breast cancer remain incompletely understood. This article outlines a strategy to identify therapeutic targets by integrating pathology AI with spatial transcriptomics. We developed AI-directed spatial transcriptomics (AID-ST), a framework that compares gene expression profiles between drug-sensitive and drug-resistant regions i…
