Race and Sex Disparities in Lower Extremity Total Joint Arthroplasty: A Retrospective Database Study.

INTRODUCTION: Total joint arthroplasty (TJA) is successful in improving health-related quality of life. However, outcomes vary in the literature due to modifiable and non-modifiable factors. Modifiable factors consist of body mass index (BMI), nutrition, and tobacco use. Non-modifiable risk factors include age, race, sex, and socioeconomic status. Prior literature has focused on racial disparities in terms of the utilization of lower extremity arthroplasty. The purpose of this study is to determine the effect of race and sex on the in-hospital complication rate, length of stay, and charges ass

Risk factors for hypertensive crisis in adult patients: a systematic review.

OBJECTIVE: The objective of this review was to examine the best available evidence on the risk factors for hypertensive crisis in adult patients with hypertension. INTRODUCTION: Hypertensive crisis is an acute severe elevation in blood pressure, which can present as hypertensive urgency or emergency. In contrast to hypertensive urgency, which is a markedly elevated blood pressure without acute target organ damage, hypertensive emergency is associated with equally high blood pressure in the presence of potentially life-threatening target organ damage, such as myocardial infarction, stroke, pul

; RESULTS OF RESECTION METHODS OF TREATMENT IN PATIENTS WITH GIANT PYLORODUODENAL ULCERS COMPLICATED BY PERFORATION AND BLEEDING.

We studied the immediate and long-term results of various methods of gastric resection in 35 patients operated at the S.S. Yudin State Medical Center in Moscow from 2000 to 2019. 20 (57.1%) patients were operated on for perforation, and 15 (42.6%) patients underwent surgery for ulcerative bleeding. There were 27 (77.2%) males and 8 (22.8%) females. The average age of patients was 48.3±2.3 years. The diagnosis of peptic ulcer disease was previously made in 24 (68.6%) patients, the duration of the disease was 7.7±2.7 years. Of the 35 patients, 29 (82.8%) underwent standard gastric resection with

Outcomes of antegrade and retrograde open mesenteric bypass for acute mesenteric ischemia.

BACKGROUND: Acute mesenteric ischemia (AMI) is a challenging clinical problem associated with significant morbidity and mortality. Few contemporary reports focus specifically on patients undergoing open mesenteric bypass (OMB) or delineate outcome differences based on bypass configuration. This is notable, because there is a subset of patients who are poor candidates for endovascular intervention including those with flush mesenteric vessel occlusion, long segment occlusive disease, and a thrombosed mesenteric stent and/or bypass. This analysis reviewed our experience with OMB in the treatment

Short- and long-term readmission rates after infrainguinal bypass in a safety net hospital are higher than expected.

OBJECTIVE: Readmission rates are expected to have an increasing effect on both the hospital bottom line and physician reimbursements. Safety net hospitals may be most vulnerable. We examined readmissions at 30 days, 90 days, and 1 year in a large safety net hospital to determine the magnitude and effect of short- and long-term readmission rates after lower extremity infrainguinal bypass in this setting. METHODS: All nonemergent extremity infrainguinal bypass performed at a large safety net hospital between 2008 and 2016 were identified. Patient demographic, social, clinical, and procedural de

Effect of complications within 90 days on patient-reported outcomes 3 months and 12 months following elective surgery for lumbar degenerative disease.

OBJECT There is a paradigm shift toward rewarding providers for quality rather than volume. Complications appear to occur at a fairly consistent frequency in large aggregate data sets. Understanding how complications affect long-term patient-reported outcomes (PROs) following degenerative lumbar surgery is vital. The authors hypothesized that 90-day complications would adversely affect long-term PROs. METHODS Nine hundred six consecutive patients undergoing elective surgery for degenerative lumbar disease over a period of 4 years were enrolled into a prospective longitudinal registry. The foll

The meaning of elevated troponin I levels: not always acute coronary syndromes.

BACKGROUND: Troponin elevation can be caused by etiologies other than acute coronary syndromes (ACS). Our hypothesis was that elevated troponins occur more frequently in non-ACS cases but that ACS cases (type 1 ST-elevation myocardial infarction [STEMI] and type 1 non-STEMI [NSTEMI]) have significantly higher troponin elevations. METHODS: This was a cross-sectional cohort analysis of a random subset of all patients with elevated troponins (defined as ≥0.06 ng/mL) over a 1-year period from July 2013 to June 2014. The first positive troponin I and the peak were used in this study. All included

Trancranial Doppler: value in clinical practice.

The value of TCD in clinical practice is well established since it can be used to measure cerebral vasomotor reactivity and to detect and grade vasospasm (VSP) following subarachnoid haemorrhage and cerebral blood perfusion consequences of extracranial ICA stenosis or occlusion. Intracranial steno-occlusive disease can be detected more reliably by transcranial color-coded imaging (TCCI) that provides a two-dimensional imaging of parenchymal and vascular anatomy of brain too. In patients with suspected brain TCD diagnostic criteria for brain death have a sensitivity of 91 to 100% and specificit

Risk-adjusted 30-day outcomes of carotid stenting and endarterectomy: results from the SVS Vascular Registry.

OBJECTIVE: As the first operational societal registry of carotid procedures, the Outcomes Committee of the Society for Vascular Surgery (SVS) developed the Vascular Registry (VR) in response to the Centers for Medicare and Medicaid Services' (CMS) National Coverage Decision on carotid artery stenting (CAS). Although CMS requires data submission only on CAS, the VR collects similar data on carotid endarterectomy (CEA) to allow comparison of outcomes, as well as potential for expansion to other procedures. METHODS: SVS-VR on-line provider-reported data include baseline through follow-up visits

Mid-term results after endovascular repair of the abdominal aortic aneurysm.

PURPOSE: As a minimally invasive strategy for the treatment of patients with abdominal aortic aneurysm (AAA), endovascular repair has been embraced with enthusiasm because of the promise of achieving a durable result with a reduced risk of perioperative morbidity and mortality. Our mid-term experience with endovascular AAA repair was assessed by examining early and late clinical outcome in concurrent cohorts of patients stratified either as low-risk or as at increased-risk for intervention. METHODS: From April 1994 to December 1999, endovascular AAA repair was performed in 104 patients with c

Hospital-acquired complications alter quality of life in adult burn survivors: Report from a burn model system.

INTRODUCTION: Successful burn care should facilitate comprehensive, functional recovery after an injury. But we have a poor understanding of which risk factors influence long-term outcomes after burn injury. Studies have correlated hospital-acquired complications (HACs) with poor long-term outcomes in some populations. The purpose of this study was to determine whether HACs alter patient-reported quality of life in adult burn survivors. METHODS: We followed 496 adults with major burn injury longitudinally as part of a burn outcomes study (1993-2014). Study participants completed SF-12® Health

Feasibility and outcomes of anaesthesiology- and intensive care-led ECPR in a hospital without cardiac surgery: a 2.5-year prospective registry.

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) has been demonstrated to yield positive outcomes in cases of refractory cardiac arrest when implemented within strict time frames. The majority of ECPR programmes are operational within the confines of cardiac surgery centres, with the assistance of surgical teams and perfusionists for the processes of cannulation and circuit management. The present study evaluated the feasibility, safety and outcomes of an anaesthesiology-intensive care-led ECPR programme delivered in a university hospital without on-site cardiac surgery. METHODS

Delirium and adverse clinical outcomes: a matched cohort study in the UK Biobank.

BACKGROUND: Delirium complicates up to one in four hospitalisations among older adults, but its clinical sequelae beyond cognitive impairment, functional decline, and mortality are not well characterised. Whether delirium signals broader multisystem vulnerability leading to later adverse outcomes remains uncertain. We aimed to examine the association of in-hospital delirium with the occurrence of a range of adverse outcomes during later hospitalisations in the UK. METHODS: We performed a matched cohort study among hospitalised UK Biobank participants recruited in England, Scotland, and Wales

Outpatient Follow-Up and 30-Day Readmissions: A Systematic Review and Meta-Analysis.

IMPORTANCE: Outpatient follow-up after discharge has been associated with reduced 30-day readmissions. Since universal follow-up is not feasible, identifying for whom and when outpatient follow-up is most beneficial is essential for optimizing resources and reducing readmissions. OBJECTIVE: To quantify the association between outpatient follow-up within 30, 14, and 7 days postdischarge and 30-day all-cause readmissions and assess differences in outcomes by disease, age, and baseline readmission risk. DATA SOURCES: MEDLINE (via PubMed), Embase, and CINAHL were searched for studies published b

Moderate-to-Severe Preoperative Anemia is Associated with Increased Postoperative Myocardial Infarction and Mortality in Patients Undergoing Transcarotid Artery Revascularization.

BACKGROUND: While preoperative anemia is prevalent among surgical patients, its impact on patients undergoing transcarotid artery revascularization (TCAR) remains poorly understood. This study aims to assess the relationship between the severity of preoperative anemia and outcomes following TCAR. METHODS: A retrospective analysis of the Vascular Quality Initiative database (2016-2021) was performed to identify patients who underwent TCAR for carotid stenosis. Anemia was defined according to World Health Organization guidelines as a hemoglobin (Hb) level <12 g/dL in females and <13 g/dL in mal
