Outcomes from laparoscopic versus robotic mesh removal after inguinal hernia repair.

INTRODUCTION: Preperitoneally placed mesh for inguinal hernia repair may require removal to address hernia recurrence, mesh reaction, meshoma, or other chronic pain. These are best approached either laparoscopically or robotically, but there is no consensus on which is the best approach for mesh removal nor are there any studies to evaluate and compare their outcomes. METHODS: All patients who underwent inguinal mesh removal via laparoscopic and robotic approaches from 2011 to 2020 were analyzed. Data regarding demographics, preoperative, intraoperative, and postoperative outcomes were collec

Robotic Harvest of the Deep Inferior Epigastric Perforator Flap for Breast Reconstruction: A Case Series.

BACKGROUND: Robotic surgery is emerging as a viable tool in reconstructive surgery. Harvesting of the deep inferior epigastric perforator flap is typically performed through an anterior approach, which involves a long fascial incision. A robotic approach allows the deep inferior epigastric pedicle to be harvested from the posterior surface. This approach reduces the length of the fascial incision and should decrease the abdominal morbidity associated with large fascial dissections. METHODS: A case series study of 21 patients who underwent a robotic deep inferior epigastric perforator or durin

Interaction dynamics modeling and adaptive impedance control of robotic exoskeleton for adolescent idiopathic scoliosis.

Adolescent Idiopathic Scoliosis (AIS), an abnormal lateral curvature of the patient's spine with vertebral rotation, is one of the biggest problems in the orthopedic profession. Non-surgical treatments of AIS, including bracing, has the following limitations: i) sensor-less design and lack of closed-loop control on the motion and forces exerted on the torso result in no adaption to the skeletal changes during the treatment, ii) lack of interaction control may also make the brace inconvenient and insecure for the patient. In this paper, a novel robotic exoskeleton equipped with linear actuators

Does Surgeon Experience Correlate with Crowd-Sourced Skill Assessment in Robotic Bariatric Surgery?

BACKGROUND: The Global Evaluative Assessment of Robotic Skills (GEARS) rubric provides a measure of skill in robotic surgery. We hypothesize surgery performed by more experienced operators will be associated with higher GEARS scores. METHOD: Patients undergoing sleeve gastrectomy from 2016 to 2020 were analyzed. Three groups were defined by time in practice: less than 5, between 5 and 15, and more than 15 years. Continuous variables were compared with ANOVA and multivariable regression was performed. RESULTS: Fourteen operators performing 154 cases were included. More experienced surgeons ha

Development and Electromyographic Validation of a Compliant Human-Robot Interaction Controller for Cooperative and Personalized Neurorehabilitation.

BACKGROUND: Appropriate training modalities for post-stroke upper-limb rehabilitation are key features for effective recovery after the acute event. This study presents a cooperative control framework that promotes compliant motion and implements a variety of high-level rehabilitation modalities with a unified low-level explicit impedance control law. The core idea is that we can change the haptic behavior perceived by a human when interacting with the rehabilitation robot by tuning three impedance control parameters. METHODS: The presented control law is based on an impedance controller with

Nonlinear Intelligent Control of Two Link Robot Arm by Considering Human Voluntary Components.

This paper proposes a nonlinear intelligent control of a two link robot arm by considering human voluntary components. In general, human arm viscoelastic properties are regulated in different manners according to various task requirements. The viscoelasticity consists of joint stiffness and viscosity. The research of the viscoelasticity can improve the development of industrial robots, rehabilitation and sports etc. So far, some results have been shown using filtered human arm viscoelasticity measurements. That is, human motor command is removed. As a result, the dynamics of human voluntary co

Foot contact forces can be used to personalize a wearable robot during human walking.

Individuals with below-knee amputation (BKA) experience increased physical effort when walking, and the use of a robotic ankle-foot prosthesis (AFP) can reduce such effort. The walking effort could be further reduced if the robot is personalized to the wearer using human-in-the-loop (HIL) optimization of wearable robot parameters. The conventional physiological measurement, however, requires a long estimation time, hampering real-time optimization due to the limited experimental time budget. This study hypothesized that a function of foot contact force, the symmetric foot force-time integral (

A prospective cohort study of the accuracy and safety of robot-assisted minimally invasive spinal surgery.

BACKGROUND: Robot-assisted open surgery (RA-OS) is now commonly used in traditional open-exposure spinal screw placement surgery. With the help of robots, robot-assisted minimally invasive surgery (RA-MIS) can achieve less bleeding and less tissue damage in percutaneous screw insertion. While the research comparing the safety and accuracy of screw placement between RA-MIS and RA-OS is insufficient. This study aims to compare the effects of RA-MIS and RA-OS in thoracic and lumbar spine. METHODS: This was a prospective cohort study evaluating 208 patients undergoing robot-assisted screw inserti

First completely robot-assisted retroperitoneal nephroureterectomy with bladder cuff: a step-by-step technique.

INTRODUCTION: While various surgical techniques have been reported for open and minimally invasive treatment of upper tract urothelial cancer (UTUC), the procedure of robot-assisted nephroureterectomy (NU) with bladder cuff has never been reported using only retroperitoneum without entering abdominal cavity. We developed a novel port placement and technique allowing to perform robot-assisted NU by a unique retroperitoneal approach. METHODS: Between February and June 2021 patients with history of UTUC were treated by robot-assisted NU completely restricted to retroperitoneal space using a sing

Transoral robotic surgical excision of hypopharyngeal ectopic parathyroid adenoma causing primary hyperparathyroidism.

Ectopic parathyroid adenomas are a rare congenital anomaly despite the complex migration parathyroid glands undergo during development. Rarer still, these can be found in the hypopharynx, become adenomas and be the cause of primary hyperparathyroidism. We present a case of a woman in her 50s who underwent transoral robotic surgery (TORS) to remove her hypopharyngeal ectopic parathyroid adenoma and cure her primary hyperparathyroidism without complication. This is a radical change in practice compared with traditional open, transverse cervical approaches. We demonstrate the efficacy of using TO

Robotic radical distal gastrectomy for gastric cancer using the soft coagulation scissors technique.

We have developed a novel technique for safe and precise lymph-node dissection during robotic gastrectomy for gastric cancer using monopolar curved scissors with soft coagulation. This technique is called the soft coagulation scissors technique. The technical details of this technique are as follows: a long bipolar grasper in the first arm and monopolar curved scissors in the third arm are primarily used for lymph-node dissection, maximizing the use of the robotic arm articulation. The monopolar curved scissors were energized in the soft coagulation mode of the Valleylab™ FT10 energy platform

Practising transoral robotic surgery in a middle-income country: surgical outcomes and early challenges.

Transoral robotic surgery is a minimally invasive surgical technique that recently debuted in Malaysia. However, there are concerns over its cost, practicality, and feasibility in local settings. Our study aims to evaluate the surgical outcomes of transoral robotic surgery and discuss its learning curves. The clinical records of all patients who underwent transoral robotic surgery in a university hospital were reviewed. 25 patients were identified with a mean age of 43.9 years. The commonest indication was obstructive sleep apnoea (OSA) (76%), followed by base of tongue carcinoma (16%), recurr

Robot-Assisted Partial Cystectomy versus Open Partial Cystectomy for Patients with Urachal Cancer.

INTRODUCTION: Localized urachal cancer (UrC) can be treated with an open partial cystectomy (OPC) with en bloc resection of the urachal remnant and umbilicus. Robot-assisted partial cystectomy (RAPC) is an alternative approach, of which its safety and efficacy for UrC remains to be determined. In the present study, we analyze these outcomes after RAPC, compared with OPC. METHODS: We retrospectively evaluated 55 cN0M0 UrC patients who underwent RAPC (n = 8) or OPC (n = 47) between 1994 and 2020. Overall survival (OS) and recurrence-free survival (RFS) were assessed using Kaplan-Meier methods.
