BACKGROUND: The Upper West region of Ghana is mostly made up of rural communities and is highly endemic for lymphatic filariasis (LF), with a significant burden of disability due to lymphedema and hydrocele. The aim of this paper is to describe an enhanced, evidence-based cascading training program for integrated lymphedema management in this region, and to present some initial outcomes.

Embarrassment and concealment of chronic leg wounds can delay presentation, increase complication risk, and perpetuate social isolation. Recognition of psychosocial barriers is essential for timely assessment and dignified care. This case illustrates how embarrassment and self neglect can delay presentation and exacerbate wound severity. Community networks can play a pivotal role in identifying vulnerable individuals. Effective management requires integrated clinical treatment alongside supportive, non-judgemental approaches that preserve dignity and encourage disclosure to enable timely care.

We report an unusual case of scleral and lid necrosis from suspected self-harm and the management of the resultant scleral perforation with a tarsoconjunctival 'Hughes' flap. To our knowledge, no previous literature describes such a technique in the repair of toxic scleral melts. Our case describes an alternative use for a Hughes flap in providing tectonic support and helping to restore the integrity of the globe in a complex case where conventional methods of 'patching' had failed.

Erythema multiforme (EM) is an acute, self-limiting hypersensitivity reaction that ranges from a mild cutaneous, exanthematous variant with minimal oral involvement to a progressive fulminating severe variant with extensive mucocutaneous epithelial necrosis (SJS & TEN). Knowing the etiological spectrum is the key for management in EM where sometimes leads to mortality. There are no specific diagnostic tests for EM, diagnosis is mainly clinically supported. Here in we report series of cases of EM with different etiological spectrum.

A 46-year-old female patient exhibited massive endobronchial bleeding after dilation of a left bronchial anastomotic stenosis after lung transplantation, consistent with a bronchopulmonary artery fistula (BPAF). The BPAF was treated with a bronchial covered self-expandable metallic stent and percutaneous transcatheter pulmonary artery stent placement. BPAF is rare and leads to death in most cases because of massive hemoptysis. We describe a case of successful combined management of BPAF using both bronchial and pulmonary stent placement.

Hiccups (singultus) is often a benign, common and self- limited condition. A case of profound electrolyte disturbances presenting with chief complaint of hiccups is presented in which chlorpromazine was not administered and could have been problematic had it been given. For those who present to the ED with chief complaint of hiccups, it is critical to consider a life threatening etiology as the cause of their symptom. This case presents a potential pitfall in the management of singultus.

While epistaxis is self-limiting in most cases, patients who are anticoagulated are at increased risk for life-threatening hemorrhage. The management of epistaxis in patients with a left ventricular assist device is further complicated by an increased risk for hemodynamic instability. Here, we detail the successful treatment of massive epistaxis in a left ventricular assist device patient by endovascular particle embolization. We hope to increase awareness in the cardiovascular community of this minimally invasive, potentially life-saving procedure.

Perthes disease refers to self-limiting idiopathic avascular necrosis of capital femoral epiphysis in a child. There is no consensus for the optimum treatment of Perthes disease even 100 years after the first description. The prime aim of the treatment is to maintain the sphericity of the femoral head and the congruency of the femur-acetabulum relationship to prevent secondary degenerative arthritis. Early diagnosis and management can help the collapse of femoral head, progressive femoral head deformity, and impingement.

Chronic bullous disease of childhood (CBDC) is generally a self-limited disease of prepubescent children that resolves within months to years. However, as discussed in the previous issue of Dermatologic Clinics, the disease is associated with significant morbidity and usually requires systemic therapy. Treatment is aimed at controlling blistering while avoiding adverse reactions. There are several anecdotal reports of treatment options, but controlled or comparative studies are lacking. This article discusses the management options of CBDC in more detail.

The fetus of a 35-year-old woman, gravida 5, para 3, was diagnosed with isolated left-sided congenital talipes equinovarus on routine ultrasound at 22 weeks of gestation. Parents were counselled about the abnormality but still ultrasound pictures made it difficult for them to understand the nature of the defect. A three-dimensional ultrasound scan helped the couple to understand the abnormality, make an informed choice-with self-confidence-regarding pregnancy continuation, and plan for postnatal management.

The effect of intravesical oxybutynin hydrochloride on detrusor behaviour was studied in 15 patients with urinary incontinence due to detrusor hyper-reflexia by ambulatory bladder monitoring. Twelve patients had a reduction in the frequency and amplitude of hyper-reflexic contractions and incontinence episodes following oxybutynin administration. Intravesical oxybutynin may have a role in the management of patients on intermittent self-catheterisation with detrusor hyper-reflexia who cannot tolerate the side effects of anticholinergic agents, including oxybutynin.

Adipsia combined with diabetes insipidus after hypothalamic damage may produce major difficulties in clinical management. If there is an associated memory impairment it may be impossible to teach self-regulation of fluid balance, necessitating long-term hospital supervision. The successful use of a behaviour modification technique to achieve independent drinking and allow discharge from hospital into the community is described in a patient with adipsia, diabetes insipidus and memory impairment resulting from the removal of a craniopharyngioma.

AIM: To assess the prevalence of burnout, secondary traumatic stress, and compassion satisfaction and explore their impacts on self-reported hand hygiene among medical aid teams in the COVID-19 period in Wuhan, China.
DESIGN: Cross-sectional study.
METHOD: A total of 1,734 healthcare workers from 17 medical aid teams were surveyed. The survey included burnout, secondary traumatic stress and compassion satisfaction measured by the professional quality of life scale and self-reported hand hygiene.
