Corrective osteotomy is an effective method of treating distal radius malunions with good long-term functional results.

INTRODUCTION: Malunion occurs in approximately 23% of non-operatively treated and 11% of operatively treated distal radius fractures. The decision whether to correct a malunion is primarily based on functional impairment and wrist pain. The purpose of this study was to assess the long-term functional outcomes of corrective osteotomies for symptomatic malunited distal radius fractures. METHODS: All consecutive corrective osteotomies of the distal radius performed in one centre between January 2009 and January 2016 were included. The primary outcome was the functional outcome assessed with the

Aetiology of injuries and the need for protective equipment for surfers in the UK.

BACKGROUND: Surfing is a popular UK water-sport. Previous studies investigating surfing injuries have been principally based on trauma from large waves and reef breaks in the US and Australia. Subsequent conclusions and recommendations for the use of protective equipment in surfing may therefore not be relevant in the UK. AIMS: This study examines the aetiology of surfing injuries in the UK. METHODS: Data was collected in 2012 using a web-based survey distributed to UK surfing clubs. Surfers recorded time/experience as a surfer, number of episodes spent surfing per month, different breaks su

Analysis of the complications of palmar plating versus external fixation for fractures of the distal radius.

PURPOSE: To evaluate whether there was a difference in complication rates in our patients treated with external fixation versus volar plating of distal radius fractures. We also looked for a difference in radiographic results; in the clinical outcomes of flexion, extension, supination, pronation, and grip strength; and in scores on the visual analog scale (VAS) for pain and the Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire. METHODS: We reviewed 115 patients with comminuted intrarticular distal radius fractures. Of those patients, 59 were treated with external fixation and 5

Methylprednisolone after shoulder arthroplasty leads to decreased opioid prescriptions without an increased risk of infection: A matched cohort analysis.

BACKGROUND: Due to the increasing demand for total shoulder arthroplasty, safe opioid stewardship paired with appropriate pain management is imperative for long-term patient care. A recent study has shown the potential for methylprednisolone use in the peri-operative period in order to decrease opioid consumption postoperatively. METHODS: The large TriNetX database was queried to identify all patients who were methylprednisolone naïve that underwent total shoulder arthroplasty with a minimum follow-up of 3 months. These patients were then further separated into two cohorts - those who receive

Load carriage using packs: a review of physiological, biomechanical and medical aspects.

This paper reviews the biomedical aspects of transporting loads in packs and offers suggestions for improving load-carriage capability. Locating the load mass as close as possible to the body center of gravity appears to result in the lowest energy cost when carrying a pack. Thus, the double pack (half the load on the front of the body and half the load on the back) has a lower energy cost than the backpack. However, backpacks provide greater versatility in most situations. The energy cost of walking with backpack loads increases progressively with increases in load mass, body mass, walking sp

Concomitant body-wide trauma patterns in patients with head and neck injuries: a comparison based on the trauma register DGU® by the German trauma society and the dortmund maxillofacial trauma registry.

INTRODUCTION: Interdisciplinary trauma patients with multiple injuries require special attention from the first minute of care. This study aimed to determine the overall injury distribution in patients with head and neck injury (HNI) with respect to the head and neck injury location. METHODS: Data on patients with HNI were collected in the Dortmund Maxillofacial Trauma Register 2007-2017 based on a review of patient radiographs and files. All patients with concomitant injuries to central body parts were selected and analysed further by an expert traumatologist collecting data on GCS, ISS, inj

Reverse total shoulder arthroplasty is safe and effective in patients ≥90 Years old.

BACKGROUND: Surgeons may be hesitant to perform reverse total shoulder arthroplasty (rTSA) in elderly patients due to medical complexity, frailty, and possibility of higher rates of complications and mortality. Credence is given to this notion by reports from the hip and knee arthroplasty literature, which show higher rates of postoperative complications and mortality in elderly patients. As such, the purpose of the current study is to analyze outcomes and complications following rTSA in patients ≥90 years old. METHODS: All patients ≥90 years old who underwent primary rTSA from 2010 to 2024 w

Suprapectoral Onlay Biceps Tenodesis With Metal Button or Soft-Body Anchor Is Safe and Effective During Total Shoulder Arthroplasty.

INTRODUCTION: A well-documented association exists between long head of the biceps tendon (LHBT) pathology and glenohumeral arthritis. Given this association, the LHBT is often managed with tenodesis during total shoulder arthroplasty (TSA). Many LHBT tenodesis techniques exist, including soft-tissue tenodesis or anchor fixation. No literature currently compares fixation techniques for onlay LHBT tenodesis; thus, the purpose of this study was to compare onlay fixation with a metal button and soft-suture anchor. MATERIALS AND METHODS: This retrospective study included patients undergoing prima

Open Versus Arthroscopic Reduction for Perilunate and Lunate Dislocations: A Systematic Review and Network Meta-Analysis.

PURPOSE: Perilunate and lunate dislocations represent complex wrist injuries requiring urgent reduction and stabilization. While open reduction has been the gold standard, arthroscopic techniques have emerged as less invasive alternatives. However, comparative evidence remains limited, and the management strategies are controversial. METHODS: We conducted a systematic review and network meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Dedicated searches of major databases were performed up to June 16, 2025. Studies comparing open and

Outcomes of Free Vascularized Fibular Grafts in Treating Massive Forearm Skeletal Defects.

INTRODUCTION: Reconstructing long bone defects in the upper limbs, particularly the radius and ulna, poses significant challenges. These defects, resulting from trauma, tumors, infections, or congenital anomalies, require precise surgical intervention for functional restoration. Traditional non-vascularized autogenous bone grafts have limitations, such as resorption and limited biological activity. To address these challenges, free vascularized fibular grafts (FVFGs) have been developed, offering enhanced recovery by supplying nutrients and structural support, particularly in large defects or

Surgical vs. Conservative Treatment of Distal Radius Fractures in the Elderly: A Systematic Review and Meta-Analysis.

Elderly patients with distal radius fractures (DRFs) pose a significant medical challenge due to their high incidence and related healthcare costs. Both surgical methods like volar plate fixation and conservative approaches such as casting are common, yet their relative effectiveness remains unclear. This review and meta-analysis compare surgical and conservative treatments, focusing on wrist functionality, upper extremity performance, grip strength, and pain after one year. A literature search was conducted in PubMed, Cochrane Library, Scopus, and Web of Science up to November 6, 2024, to fin
