Guidelines for the diagnosis and treatment of vitiligo in Japan.

Vitiligo is an acquired pigment disorder in which depigmented macules result from the loss of melanocytes from the involved regions of skin and hair. The color dissimilarity on the cosmetically sensitive regions frequently induces quality of life impairment and high willingness to pay for treatment in patients with vitiligo. The Vitiligo Japanese Task Force was organized to overcome this situation and to cooperate with the Vitiligo Global Issues Consensus Conference. This guideline for the diagnosis and treatment of vitiligo in Japan is proposed to improve the circumstances of Japanese individ

Opportunistic filamentous mycoses: aspergillosis, mucormycosis, phaeohyphomycosis and hyalohyphomycosis.

Opportunistic filamentous mycoses are widely distributed all over the world. They are rarely observed in Europe but are common in developing countries. The most common are the aspergilloses (due to Aspergillus spp.) mostly in neutropenia and immunosuppression; the mucormycoses characterized by rapid progression in patients with diabetic ketoacidosis; the phaeohyphomycoses due to pigmented fungi causing either a mild superficial or a very serious deep disease and the hyalohyphomycoses due to hyaline filamentous fungi (Fusarium spp., Pseudallescheria spp., Scopulariopsis spp.). Cutaneous manifes

Childhood granulomatous periorificial dermatitis in children with extra-facial involvement.

Childhood granulomatous periorificial dermatitis (CGPD) is a self-limiting and well-recognized entity. A six-year-old male child, a known case of juvenile rheumatoid arthritis (JRA) presented with multiple red raised and yellowish lesions over the face, neck, trunk and upper extremities since one month with occasional itching. Cutaneous examination revealed multiple erythematous scaly papules of size up to 5 mm around the mouth, nose and periorbital areas, neck, trunk and upper extremities with few excoriations. Lesional skin biopsy was pathognomic of CGPD. We report a six-year-old Indian male

Multiple nevoid hypertrichosis as an isolated developmental defect.

A 3-year-old girl presented with longer hair on the left side of her scalp, coarse hair of abnormal length on her extremities, and a tuft of hair in the lumbosacral region, with all hair distributed on normally pigmented skin. Neither similar or relevant family history nor associated extracutaneous abnormalities was detected after a thorough examination. Clinical diagnosis of patchy nevoid hypetrichosis was confirmed by histology. Nevoid hypertrichosis is a rare hair growth disorder that usually presents at or soon after birth. It is characterized by patches of hypertrichosis distributed in a

Cranial nerves and brain fiber systems of the medaka fry as observed by a whole-mount staining method.

The cranial nerves and the brain fiber systems of the medaka (Oryzias latipes) fry are revealed by a whole-mount staining method. Newly hatched fry of an albino strain of the medaka were fixed, partially digested with trypsin, treated in 1% Triton X-100, and finally immunohistochemically stained using anti-neurofilament protein (70K+160K+210K) antibodies. Since both head skin and eyes were colorless in the albino fish, the three-dimensional distribution of of nerve fibers in the brain could be readily observed in whole specimens without interference of pigment cells. All cranial nerves and mai

Hyperpigmented small spots induced by long-term PUVA therapy. A clinical, light and electron microscopic study.

Among 308 photochemotherapy (PUVA)-treated patients, 15 psoriatics and 1 case of mycosis fungoides developed persistent disseminated hyperpigmented small spots at the trunk and limbs as a side effect of the therapy. The histological, histochemical and electron microscopical studies performed on 5 of the patients revealed a greatly increased number of melanocytes in the macules, hyperactivity of the melanocytes and increased transfer of pigment to dermis and keratinocytes. Moreover, binucleated cells were found as well as multifarious signs of melanocytic damage in varying degrees. Similar alte

Daily Photoprotection Mitigates Seasonal Skin Changes and Supports Skin Longevity: A 1-Year in vivo 3D Multiphoton Microscopy Study.

INTRODUCTION: While the detrimental effects of intense sun exposure are well-recognized, the cumulative impact of daily moderate UV exposure remains less explored. This year-long study investigated the effects of seasonal variations on facial skin, aiming at highlighting subclinical skin changes using non-invasive in vivo multiphoton microscopy. We also explored the impact of applying daily a well-balanced photoprotection formula with enhanced UVA filtration. METHODS: Fifty-nine European descent women (aged 55-65, Fitzpatrick phototypes II or III), participated in this year long, real-life ex

Metastatic melanoma from an unknown primary site presenting as skin-colored nodules and multiple visceral involvement.

A 55-year-old man presented with multiple gradually progressing asymptomatic swellings on his body for the preceding 6 months. He had no personal or family history of any skin disease. There was no systemic symptom apart from occasional constipation. Examination revealed multiple discrete, firm, nontender, skin-colored nodules of varying sizes, fixed to the skin but free from the underlying structures on his chest, abdomen, and back. The overlying skin of the nodules was erythematous at places (Figure 1). A solitary depigmented, nonanesthetic patch (measuring 3 cm x 3 cm) was noted around a ce

Black and Brown Oro-facial Mucocutaneous Neoplasms.

Black and brown-colored mucocutaneous lesions present a differential diagnostic challenge, with malignant melanoma being the primary clinical concern. The vast majority of pigmented lesions in the head and neck region are the result of benign, reactive factors such as post-inflammatory melanosis. However, it is not uncommon to discover a range of muco-cutaneous black and brown neoplasms in the oro-facial area. The majority of black/brown pigmented neoplasms are melanocytic in origin; these are neoplasms of neural crest derivation. Melanocytic nevi are a diverse group of benign neoplasms that a

Infantile Congenital Nevus With Atypical Proliferative Nodules and NTRK Gene Fusion.

Infantile congenital melanocytic nevus (CMN) is a rare type of benign skin lesion originating from neural crest cells. A proliferative nodule (PN) may develop within CMN, and in some cases, atypical proliferative nodules (APNs) with distinct histopathological features may arise, complicating clinical diagnosis and management. This report describes a case of a giant infantile CMN with APN and novel genetic findings, including TPR::NTRK1 gene fusion and MCL1 amplification. An 8-day-old male presented with a large, uniform, hairless black lesion on the plantar skin. Histological examination revea

Does Skin Color Influence Differential Diagnosis Generation in a Simulated Cardiac Arrest?

INTRODUCTION: It is not known whether skin color influences inclusion of substance use on medical students' differential diagnoses for a simulated cardiac arrest. Our objective in this study was to investigate whether the presence of a dark-skinned or light-skinned manikin in the lab correlated with their decision to include substance use on the differential as a possible cause of cardiac arrest. METHODS: In this study at a single institution, all fourth-year medical students in an emergency medicine course participated in a manikin-based cardiac arrest simulation case. The simulation session

Safety and efficacy of Simvastatin in the treatment of vitiligo: a systematic review and meta-analysis of randomized controlled trials.

Vitiligo is an autoimmune skin disorder characterized by progressive depigmentation due to melanocyte destruction. Despite various treatment options, achieving complete repigmentation remains challenging. Simvastatin, a 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitor, has demonstrated anti-inflammatory and immunomodulatory effects, making it a promising candidate for vitiligo treatment. This systematic review and meta-analysis aimed to evaluate the efficacy and safety of Simvastatin in vitiligo treatment. A systematic search of PubMed, Scopus, Web of Science, and Cochrane L

A facial lentiginous nevus with atypical clinical features in a child: The importance of in vivo reflectance confocal microscopic findings.

A 14-year-old girl presented with a facial-pigmented lesion suspicious of melanoma clinically and dermoscopically. In vivo, reflectance confocal microscopy (RCM) findings excluded melanoma by revealing typical epidermal honeycomb and cobblestone patterns. Well-defined follicular contours were seen at the dermal-epidermal junction; there were no elongated, "medusa head-like" follicular protrusions or folliculotropism, which are classical findings seen in lentigo maligna. With this report, we aim to demonstrate the significance of utilizing RCM technology in difficult to diagnose lentiginous pig

Grading Melanocytic Dysplasia: Updated Histopathologic Criteria.

Dr. Martin C. Mihm, Jr.'s innovative work on the dysplastic nevus achieved a milestone in his chapter in the World Health Organisation Classification of Skin Tumours (WHO-C). WHO-C presents a dichotomous classification (high-grade versus low-grade dysplastic nevi) and a quantitative metric to assess melanocytic nuclear enlargement. The Duke classification is a related approach that provides mostly quantitative histopathologic criteria for dysplastic nevi and gives due weight to architectural features as well as cytology. This paper proposes and illustrates updated criteria for scoring and grad

Skin color reporting in squamous cell carcinoma-related randomized controlled trials in top dermatology journals: a systematic review.

The objectives are to determine the frequency that skin color is reported in randomized controlled trials (RCTs) involving squamous cell carcinoma (SCC) detection and treatment in leading dermatology journals. A systematic review of RCTs involving SCC was conducted among the top ten most impactful dermatology journals from inception to July 10th, 2023. Studies were included if they reviewed the treatment, prevention, or detection of SCC, involved patients directly and were classified as traditional RCTs. Studies were considered positive for reporting SOC if there was any demographic data in th

Uncovering a new SASH1 mutation associated with dyschromatosis universalis hereditaria using whole-exome-sequencing: A case report.

RATIONALE: Dyschromatosis universalis hereditaria (DUH) is an uncommon form of pigmented genodermatosis that is typically inherited autosomally and dominantly. In the previous study, the pathogenic genes of DUH have been identified in ATP-binding cassette subfamily B, member 6 and SASH1. However, the mutational screening of the causative gene remains incomplete and still lacks sufficient proof in the etiology. PATIENT CONCERNS: A 2-generation Chinese family clinically diagnosed with DUH were enrolled. They showed pigmented spots from their childhood and came to the hospital for medical advice

Transformed Giant Congenital Melanocytic Nevus on 18 F-FDG PET/CT.

A 50-year-old man experienced cough and bloody sputum for 1 month. CT detected a mass in the right lung. Staging 18 F-FDG PET/CT revealed multiple hypermetabolic lesions in the lung, mediastinum, liver, and bones. Further physical examination revealed black patches in the skin covering most parts of the body, which presented at his birth and were growing very slowly, consistent with giant congenital melanocytic nevus. Pathology examination after biopsy of the lung demonstrated metastatic melanoma.
