OBJECTIVES: To describe a rare case of transplantation-mediated alloimmune thrombocytopenia (TMAT) following liver transplantation from a donor with immune thrombocytopenia (ITP), and to contextualize findings within the literature.
METHODS: We reviewed the clinical course of a 63-year-old man with hepatitis C cirrhosis and hepatocellular carcinoma who underwent orthotopic liver transplantation from a donor with severe thrombocytopenia consistent with ITP. Clinical, laboratory, and bone marrow findings were analyzed, and alternative causes of thrombocytopenia were excluded. Relevant literatur

Mycophenolic acid (MPA), administered as its prodrug Mycophenolate mofetil (MMF), is widely used to prevent rejection after organ solid transplantation. Few population pharmacokinetic studies of MPA in lung transplantation are available, so the present study was designed to describe the population pharmacokinetics (PK) of MPA and develop a maximum a posteriori Bayesian estimator of MPA area under the curve (AUC0-12h) in lung transplant (LT) recipients in the early post-transplant period. We conducted a single-center retrospective study in the Plessis-Robinson hospital (France), including LT re

BACKGROUND: The efficient allocation of donor kidneys to appropriate candidates is mandated in Korea, with a very long waiting time for deceased donor kidney transplantation (DDKT). This study evaluated the prognostic implications of organ matching using the Korean Kidney Donor Profile Index (K-KDPI) and Korean Estimated Post-Transplant Survival (K-EPTS).
METHODS: We analyzed 7,443 DDKT recipients between 2008 and 2022 using national databases: the Korean Network for Organ Sharing and the National Health Insurance Data Sharing Service. Patients were classified into classes 1 (low K-KDPI [<70]

Nutritional status is an important prognostic factor in patients with multiple myeloma (MM). The Controlling Nutritional Status (CONUT) score has shown promise in predicting outcomes in various malignancies; however, its role in autologous stem cell transplantation (ASCT) in patients with MM remains unclear. This study aimed to evaluate the significance of pre-transplant CONUT scores in predicting post-transplant engraftment kinetics and early complications in patients with MM undergoing ASCT. This single-center, retrospective study analyzed 59 multiple myeloma patients who underwent ASCT betw

The management of anticoagulation and antiplatelet therapy in stage V chronic kidney disease (CKD) patients undergoing renal transplantation remains controversial. Some centers advocate for the use of reversal agents or procoagulants preoperatively, while others suggest that transplantation can proceed safely without halting these treatments. This study aims to evaluate the incidence of hemorrhagic and thrombotic complications in the first 72 hours post-transplant in patients receiving anticoagulant or antiplatelet therapy compared to a control group without such treatments. We conducted a ret

BACKGROUND: Peri-transplant is a critical period which is associated with a myriad of complications that require pediatric intensive care unit (PICU) referral. PICU outcomes have been historically poor post-hematopoietic stem cell transplantation (HSCT), especially when associated with inotrope support, invasive ventilation, and renal replacement therapy. The study aimed to assess the outcomes of PICU referral in children undergoing HSCT.
PATIENTS & METHODS: A retrospective analysis was performed of children between 1 to 18 years of age who underwent HSCT between 2016 to 2023. A clinical dete

Heart transplantation is a well-established treatment for end-stage heart failure, but many patients present in poor physical condition, complicating outcomes. The Impella 5.5 device, used in cardiogenic shock, may support pretransplant rehabilitation by stabilizing organ function and promoting functional improvement. This retrospective cohort study assessed the relationship between functional status, measured by standardized Activity Measure for Post-Acute Care Basic Mobility (AM-PAC) scores, and days alive outside the hospital within 30 days post-transplant (DAOH-30). BACKGROUND: Infant lung transplantation (LuTx) has been rarely performed in Europe and poses unique challenges.
METHODS: We reviewed referrals for LuTx to our center for patients below 1 year of age from January 1, 2018 to December 31, 2022. Clinical data and outcomes of transplanted infants and data on patients who were declined for evaluation, not listed after evaluation, or died before LuTx were collected. The number of donor lungs from organ donors younger than 1 year of age offered to our center and their utilization were analyzed for the same time period.
RESULTS: A total of 18 referra

Post-transplant diabetes mellitus (PTDM) is a common metabolic complication following kidney transplantation and is associated with an increased risk of graft failure, cardiovascular disease, infections, and mortality. We prospectively analyzed the risk factors and clinical consequences of PTDM in patients who underwent kidney transplantation at our center. The study included 130 transplant recipients without preexisting diabetes (79 males, 51 females; mean age: 47.6 years). PTDM was defined as the requirement for pharmacologic treatment at least 3 months after transplantation or based on the

BACKGROUND: Hematopoietic cell transplantation (HCT) is the only curative treatment for chronic active Epstein-Barr virus infection (CAEBV). While HCT is needed at the appropriate time, there are sometimes difficulties in securing an appropriate donor, making HLA haploidentical donor an alternative option. Recently, post-transplant cyclophosphamide (PTCy) has rapidly gained popularity as a safe graft-versus-host disease (GVHD) prevention strategy for HCT from HLA haploidentical donors; however, there are only a few reports of its use for CAEBV.
METHOD: A retrospective chart review was complet

BACKGROUND: Polyomavirus ureteritis is a rare but increasingly recognized complication in kidney transplantation, with the potential to cause ureteral stenosis and graft loss. We report two pathologically confirmed cases of polyomavirus ureteritis presenting as ureteral stenosis, successfully treated with ureterovesical reimplantation surgery and adjustments in immunosuppressive therapy.
CASE PRESENTATION: The first case describes a 41-year-old male renal transplant recipient, who presented with progressive hydronephrosis and an elevated serum creatinine (Scr) level of 308 μmol/L 5 months pos

Post-transplant lymphoproliferative disorder (PTLD) is a rare and severe complication that occurs after kidney transplantation. Most cases of PTLD are associated with reactivation of the Epstein-Barr virus (EBV), which often happens during the early phases following transplantation. Hemophagocytic lymphohistiocytosis (HLH) is an inflammatory syndrome characterized by a cytokine storm originating from activation of the reticuloendothelial system, leading to multi-organ failure (MOF). Herein, we present a case of late-onset EBV-positive hairy cell leukemia in a kidney transplant recipient who co

INTRODUCTION: Kaposi sarcoma is a vascular tumour associated with human herpesvirus-8 (HHV-8) infection. Due to immunosuppressive treatment, solid organ transplant recipients infected with HHV-8 have increased risk of Kaposi sarcoma. The risk of being infected with HHV-8 is associated with higher age, male sex, non-White ethnicity, being a man who has sex with men (MSM) and use of recreational drugs. Post-transplant Kaposi sarcoma is rare in HHV-8 low-prevalence areas. We report the first case of a liver transplant recipient developing Kaposi sarcoma following donor-transmitted HHV-8 infection

BACKGROUND: Allogeneic hematopoietic cell transplantation (allo-HCT) is a life-saving treatment for pediatric patients but is often associated with severe gastrointestinal complications. Identifying reliable biomarkers of gut integrity and inflammation is crucial for early detection and management of these issues. This study evaluates pre- and post-transplant levels of citrulline, zonulin, and calprotectin in pediatric allo-HCT patients.
METHODS: A prospective cohort study was conducted on 100 pediatric patients at the Children's Hospital from 2023 to 2025. Citrulline, zonulin, and calprotect

Chimeric antigen receptor T-cell therapy (CAR-T) has been used to treat refractory post-transplant lymphoproliferative disorder (PTLD) in solid organ transplant patients, including heart, kidney, liver, intestine, and pancreas. We report the use of CAR-T for treating refractory PTLD in a 73-year-old female who was 7 years post bilateral lung transplantation for idiopathic pulmonary fibrosis. We discuss the immunosuppression management in this patient, as well as her clinical course and outcome.

Allogeneic hematopoietic cell transplantation (HCT) remains the sole curative treatment for most patients with hematologic malignancies. A well-matched donor (related or unrelated) remains the preferred donor for patients undergoing allogeneic HCT; however, a large number of patients rely on alternative donor choices of mismatched related (haploidentical) or unrelated donors to access HCT. In this retrospective study, we investigated outcomes of patients who underwent mismatched donor (related or unrelated) HCT with a radiation-based myeloablative conditioning MAC regimen in combination with f
