Separating Out Pulmonary Sequestration.

Pulmonary sequestration (PS) is a rare congenital anomaly that accounts for 1% to 6% of all pulmonary malformations at birth. It is characterized by a focal area of pulmonary tissue that does not have direct communication with the tracheobronchial tree and does not get blood supply from the pulmonary circulation. We present the case of a 28-year-old female with a history of recurrent pulmonary infections who was found to have intralobar sequestration and underwent curative surgical excision. Because pulmonary sequestration is commonly misdiagnosed, as it can mimic other conditions on chest X-r

Brain Death: Diagnosis and Imaging Techniques.

Brain death (BD) is an irreversible cessation of functions of the entire brain, including the brainstem. The diagnosis of BD is made on clinical grounds and neurologic examination. In the United States, clinical criteria set by the American Academy of Neurology (AAN) emphasize 3 specific clinical findings to confirm BD, which include coma, absence of brainstem reflexes and apnea. Ancillary tests are needed when neurologic examination or apnea test cannot be performed. AAN recommended ancillary tests include electroencephalogram, which confirms electrical activity loss; catheter cerebral angiog

Management of spontaneous haemorrhagic intracranial vertebrobasilar dissection: review of 21 consecutive cases.

INTRODUCTION: Haemorrhagic intracranial vertebrobasilar dissection is an uncommon cause of nontraumatic subarachnoid haemorrhage (SAH) and accounts for only 1-10% of non-traumatic SAH. Treatment in the acute phase is considered to be essential because of the high risk of rebleeding and the consequent unfavourable outcome. However, the location, the potential for involvement of eloquent vessels and the histopathological characteristics of the vessel wall make treatment demanding from both a technical and anatomical point of view. We report our experience in the management of this disease. PATIE

Application of the "five-plane method" for the prenatal diagnosis of fetal extrahepatic portosystemic shunts: a retrospective cohort pilot study.

BACKGROUND: A congenital extrahepatic portosystemic shunt (CEPS) is a rare condition. Currently, CEPS is primarily diagnosed in the postnatal period. Most cases are detected using imaging techniques such as computed tomography (CT), digital subtraction angiography (DSA), and magnetic resonance imaging (MRI), typically after severe complications arise. Due to the effect of abdominal intestinal gas, the diagnosis of CEPS via postnatal ultrasound is relatively challenging. Conversely, prenatal ultrasound offers a distinct advantage in the diagnosis of CEPS. However, a lack of understanding of thi

Post Cholecystectomy Bile Duct Injury in an Acute Setting: Categorization, Triaging, and Management Algorithm.

Background Postcholecystectomy bile duct injury (BDI) is a management challenge with significant morbidity, mortality, and effects on long-term quality of life. Early referral to a specialized hepatobiliary center and appropriate early management are crucial to improving outcomes and overall quality of life. In this retrospective analysis, we examined patients who were managed at our center over the past 10 years and proposed a triage and management algorithm for BDI in acute settings. Methods Patients referred to our center with BDI from January 2011 to December 2020 were reviewed retrospecti

Contralateral acute vascular occlusion following revascularization surgery for moyamoya disease.

OBJECTIVE: Revascularization surgery is a safe and effective surgical treatment for symptomatic moyamoya disease (MMD) and has been shown to reduce the frequency of future ischemic events and improve quality of life in affected patients. The authors sought to investigate the occurrence of acute perioperative occlusion of the contralateral internal carotid artery (ICA) with contralateral stroke following revascularization surgery, a rare complication that has not been previously reported. METHODS: This study is a retrospective review of a prospective database of a single surgeon's series of re

Indocyanine Green Angiography Aids in Prediction of Limb Salvage in Vascular Trauma.

BACKGROUND: In cases of severe vascular trauma, traditional means for assessing viability using Doppler and angiography can have limited application. Indocyanine green angiography (ICGA) allows for a rapid qualitative assessment of tissue perfusion that serves as an important adjunct to these traditional methods, which can also be particularly helpful in guiding therapy. METHODS/RESULTS: We present a case of complicated ankle fracture with severe vascular compromise, which illustrates the role of ICGA as an assessment tool to guide therapy and decision-making in cases of acute limb threat. Management of flush superficial femoral artery occlusions with combined open femoral endarterectomy and endovascular femoral-popliteal angioplasty and stent-grafting.

BACKGROUND: Flush occlusions of the superficial femoral artery (SFA) often preclude endovascular interventions for femoral-popliteal lesions. Furthermore, some investigators have found poor results with angioplasty and stenting or stent-grafting of such lesions. For suitable patients with TransAtlantic Inter-Society Consensus (TASC) C and D femoral-popliteal lesions involving a flush occlusion of the SFA, we have adopted a technique to manage this disease pattern by combining femoral endarterectomy through a single, small groin incision with distal superficial femoral-popliteal stent-grafting.

Assessment of in-stent stenosis in small children with congenital heart disease using multi-detector computed tomography: a validation study.

OBJECTIVES: Our purpose was to investigate the diagnostic reliability of multi-detector computed tomography (MDCT) in assessing in-stent stenosis compared to digital angiography (DA) in small children. BACKGROUND: Little is known about the feasibility of using MDCT to assess stents placed to treat children with congenital heart disease (CHD). METHODS: Twenty-two children (median age [range], 2(3/4) [(1/2) to 12] years) with 42 transcatheter placed stents (median diameter: 7.2 [3.4-16.3] mm) in the pulmonary arteries (n = 36), aorta (2), PDA (1), and SVC (3) underwent both MDCT and DA due to

Graft flow predictive equation in distal bypass grafting for critical limb ischemia.

OBJECTIVE: Graft flow (GF) seems to be an important prognostic predictor in distal bypass for critical limb ischemia, but previous studies have failed to clarify the association between GF and the graft prognosis. GF differs significantly among grafts, and each graft seems to have an optimal GF depending on various factors. We hypothesized that comparison between the measured GF (mGF) and optimal estimated GF (eGF) would be important in predicting graft prognosis. Herein, we aimed to develop a GF predictive equation by assessing GF determinants and to validate the equation against a clinical d
