Early urinary retention after catheter removal following radical prostatectomy predicts for future symptomatic urethral stricture formation.

OBJECTIVES: Urinary retention after urethral catheter removal is a well-established complication of radical prostatectomy (RP). Its effect o … ). Compared with the patients without EUR, the patients with EUR had a greater rate of bladder neck contracture (94% versus 68%, P = 0.04) and exhibited a significantly decreased time to stricture formation (1.6 months versus 3.0 months, P = 0.002). After adjusting for patient age, clinical stage, surgeon, and Gleason score, the odds of developing a symptomat …

Mechanical distraction for the treatment of posttraumatic stiffness of the elbow in children and adolescents. Surgical technique.

BACKGROUND: Elbow contracture is a recognized sequela of elbow injuries in children and adolescents, but previous studies of operative treatment with formal capsular release have demonstrated unpredictable outcomes and unfavorable results. METHODS: Over a period of five years, fourteen children and adolescents with a mean age of fourteen years who had posttraumatic stiffness of the elbow were managed according to a prospective protocol. Eleven patients had undergone a mean of three previous operative procedures before the index operation. After intraoperative distraction with an external fixa…

Functional cast bracing in the management of open forearm fractures: a preliminary report.

BACKGROUND: Management of any injury is expensive to the individual and the community. For this reason modification of methods of managing trauma very often is necessary in resource poor countries. OBJECTIVE: To verify the effectiveness of metal hinge joints in prevention of contractures and ankylosis of the elbow joint in cast immobilisation of reduced open forearm fractures. PATIENTS: 20 patients who had sideswipe injuries were managed from the year 01/01/2000 to date (20/5/2004) by the orthopaedic unit of Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria. They were referred f…

Wire mesh as a post-operative physiotherapy assistive device following peripheral nerve graft repair in the rat.

Traditional methods of assessing nerve recovery following injury, including histomorphometry and electrophysiology, do not necessarily correlate with return of motor and sensory function. Accordingly, many investigators have used walking track analysis as an assessment of global functional recovery following sciatic, peroneal or tibial nerve injury. However, walking track reliability may be compromised by the development of flexion contractures secondary to neurologic loss. To prevent this, regular manual physiotherapy is recommended which is a time-consuming and often frustrating process for …

A clinical and laboratory profile of symptomatic women with silicone breast implants.

One hundred seventy-six patients with breast prosthetic implants were evaluated. All women were symptomatic and were referred by either attorneys (152) or physicians (24) for rheumatic evaluation. The women ranged in age from 24 to 72 with a mean of 45 years. Indications for surgery were cosmetic (128), cancer (34), and other (14). Implants had been in place for 7 years or more in 120 patients and < 2 years in only 8. Eighty-three women required explantation of their original prostheses, and 63 had new implants inserted of which 47 were silicone and 16 were saline. Capsular contractures were p…

Skin tension in the aetiology of Dupuytren's disease; a prospective trial.

Tension in the palmar fascia has been proposed as a factor causing Dupuytren's disease. If tension does stimulate the growth of new Dupuytren's tissue, relieving longitudinal tension should reduce the recurrence rate following surgery. Thirty patients with palmar Dupuytren's contracture of a single ray that affected only the metacarpophalangeal joint were divided into two groups. Both groups had a fasciotomy: one group through a transverse incision that was closed directly and the other through a longitudinal incision with Z-plasty closure. Half the patients (seven of 14) who had direct closur…

A Case of FBN1-Related Early Onset Marfan Syndrome with Multi-Suture Craniosynostosis.

Early-onset Marfan Syndrome (eoMFS) is caused by pathogenic variants in the fibrillin-1 (FBN1) gene. Typical clinical findings include a prematurely aged appearance, severe atrioventricular valve dysfunction (mitral/tricuspid valve insufficiency), and skeletal findings-primarily arachnodactyly, multiple joint contractures, and pectus deformity. Craniosynostosis has been reported in rare cases of eoMFS, but not in association with the c.3037G>A, p.Gly1013Arg variant in FBN1. Here we present a case of a 2-month-old male with bilateral lambdoidal and sagittal craniosynostosis diagnosed with eoMFS…

The Groove Sign.

A 74-year-old patient initially presented with rapid onset of bilateral leg swelling, leukocytosis (monocytosis and eosinophilia), hand arthritis, elevated inflammatory markers, and 20% weight loss. Extensive workup, including a bone marrow biopsy, did not show evidence of malignancy. The patient was evaluated many months after initiation of symptoms and found to have deep hardening skin and multiple joint contractures. Pictures of his forearms are shown above. Tackling Tone in Tiny Tots: Hypertonia Management in Young Children.

In young children, hypertonia results from early brain insults leading to upper motor neuron lesions, which cause changes in muscle tone and pathology. These changes contribute to contractures and deformities that impair activity and participation. The management of hypertonia requires a multifaceted approach, combining phyisical and occupational therapy and pharmacologic interventions. However, limited clinical studies exist for pharmacologic treatments in children aged under 3 years, leaving clinicians to rely on experience when prescribing off-label medications. This article reviews the pha…

Use of multiple fasciocutaneous flaps for the management of extensive hidradenitis suppurativa.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease with a relapsing nature that has a significant impact on the patient's quality of life. The clinical presentation of the disease includes deep dermal abscesses and sinus tracts. Long-term affliction of the disease may lead to contractures, scars and fibrosis. Management is still challenging and varies from medical to surgical options. We report the following case of a man in his 30s who presented with severe HS in the axillae, groins, perineum and lower sacral region. The patient has been treated by excision of the whole lesi…

Several challenges associated with the anesthetic management of Emery-Dreifuss muscular dystrophy patients: case report.

Emery-Dreifuss Muscular Dystrophy is a very rare type of muscular dystrophy, associated with contractures, atrophy, and muscle weakness, besides cardiomyopathy with severe arrhythmias. Published studies focusing on this disorder are scarce. We describe the anesthetic management of a male patient with Emery-Dreifuss Muscular Dystrophy, to be submitted to umbilical and inguinal hernioplasty and hydrocele repair under epidural anesthesia. The anesthesia approach enabled us to circumvent the patient...s susceptibility to malignant hyperthermia and his potentially difficult airway, in addition to m…

Adult diagnosis of congenital serine biosynthesis defect: A treatable cause of progressive neuropathy.

A woman with ichthyosis, contractures, and progressive neuropathy represents the first case of phosphoserine aminotransferase deficiency diagnosed and treated in an adult. She has novel compound heterozygous mutations in the gene PSAT1. Treatment with high dose oral L-serine completely resolved the ichthyosis. Consideration of this diagnosis is important because early treatment with L-serine repletion can halt progression of neurodegeneration and potentially improve neurological disabilities. As exome sequencing becomes more widely implemented in the diagnostic evaluation of progressive neurod…
