Duodenal Crohn's disease complicated by common bile duct obstruction: report of a case and review of the literature.

A 28-year-old woman with Crohn's disease involving the duodenum and terminal ileum presented with biochemical evidence of incomplete, extrahepatic biliary obstruction. Percutaneous transhepatic cholangiography showed partial obstruction of the intramural duodenal portion of the common bile duct. Cholangiography was complicated by cholangitis. Surgical decompression of the biliary system and bypass of the affected duodenal segment were performed after intraoperative confirmation of Crohn's disease involving the ileum, duodenum, and ampulla of Vater. The patient made a complete recovery and all

Posterior fossa subdural haematoma associated with anticoagulant therapy.

This is a report of spontaneous posterioir fossa subdural haematoma associated with anticoagulation therapy. The possibility of posterior fossa lesions related to spontaneous haemorrhage is suggested by the combination of severe headache and increasing disturbance of consciousness associated with signs of brain-stem decompensation. A thorough neurological evaluation including appropriate contrast studies will help rule out a supratentorial lesion. This is a neurological emergency which can be successfully treated by early detection and prompt surgical decompression. This is the second reported

Model-assisted marsupialization of a large odontogenic keratocyst in the maxillofacial region using a multicolored 3D-printed model: A novel approach in surgical planning and teaching.

UNLABELLED: 3D printing can improve surgical planning and coordination between the healthcare team and serve as a valuable educational tool for students. ABSTRACT: Although the occurrence of odontogenic keratocysts (OKCs) in the maxillofacial region is not uncommon, their aggressive growth behavior requires advanced surgical techniques for minimal recurrence. The current case report describes the use of a multicolored 3D-printed model as an interactive visual aid for the surgical planning and management of an OKC treated with a minimally invasive surgical decompression technique. Recurrent Multilevel Calcified Lumbar Disk Herniation Successfully Managed by Lumbar Spinal Fusion: A Rare Case Report.

INTRODUCTION: Calcified lumbar disk herniation (LDH) is a rare subtype of LDH, and recurrent multilevel presentations in adults have not been reported. Its optimal management remains unclear, especially in cases resistant to standard decompression and discectomy. CASE REPORT: A 58-year-old woman presented with a 6-month history of bilateral lower extremity pain, numbness, and intermittent claudication, with walking limited to 10-20 m. Magnetic resonance imaging revealed L4/5 spinal canal stenosis without evidence of disk calcification on computed tomography. Posterior decompression at L4/5 re

Full-endoscopic Laminotomy Is Effective for Lumbar Spinal Stenosis with Low-grade Spondylolisthesis: A Comparative Cohort Study.

The optimal management of lumbar spinal canal stenosis with spondylolisthesremains controversial, particularly when choosing between decompression alone or decompression with fusion. Current evidence is based on conventional open or endoscopy-assisted surgeries, with limited data on full-endoscopic decompression. This study aimed to assess the impact of spondylolisthesis on outcomes after full-endoscopic laminotomy by evaluating back pain-related disability scores in patients with lumbar spinal canal stenosis, with and without spondylolisthesis. A retrospective analysis was conducted at Iwai O

Reverse Remodeling in Pediatric Myocarditis With DCM Phenotype Impact of rASD and PAB.

BACKGROUND: Myocarditis progressing to a dilated cardiomyopathy (DCM) phenotype carries a high risk of end-stage heart failure in early childhood. OBJECTIVES: Given the age-dependent regenerative capacity of the immature myocardium, we evaluated a staged, pathophysiology-guided strategy combining left atrial decompression and pulmonary artery banding (PAB) to promote myocardial recovery in selected infants and young children. METHODS: We retrospectively analyzed 31 children <3 years (55% female; median age 312 days) with biopsy-proven myocarditis and a DCM phenotype treated between 2013 and

Rib-Sparing Anterior Scalenectomy Versus First Rib Resection for the Treatment of Thoracic Outlet Syndrome.

BACKGROUND AND OBJECTIVES: Thoracic outlet syndrome is a group of disorders resulting from dynamic compression of the brachial plexus, subclavian artery, and/or subclavian vein. Patients who experience progressive symptoms despite conservative measures may benefit from surgical decompression, which is most commonly performed via first rib resection or resection of the anterior scalene muscle. Prior studies have suggested lower morbidity associated with rib-sparing scalenectomy, although few direct comparisons exist. METHODS: Retrospective cohort study of all patients undergoing first-time tho

Endoscopic Lateral Lumbar Interbody Fusion: Technical Note and Case Series.

BackgroundOur objective is to describe a minimally invasive endoscopic surgical technique for performing lateral lumbar interbody fusion (LLIF). LLIF is a common approach to lumbar fusion in cases of degenerative lumbar disease; however, complications associated with psoas and lumbar plexus injury sometimes arise. The endoscopic modification presented here diminishes the requirement for sustained muscle retraction, minimizing complication risk while allowing for adequate decompression in select cases.MethodsEndoscopic LLIF (ELLIF) was performed in 3 patients from 2019 to 2021. Surgeries were p

Lumbar Endoscopic Unilateral Laminectomy for Bilateral Decompression in Degenerative Spondylolisthesis.

BACKGROUND: Degenerative spondylolisthesis is an important cause of chronic low back pain and radiculopathy in the adult U.S. POPULATION: Open decompression with or without fusion is considered the standard for management, yet optimal treatment remains controversial. Full endoscopic spine surgery offers an alternative surgical approach with possible advantages. There is a paucity of data on the use of full endoscopic spinal surgery in degenerative spondylolisthesis. Therefore, we present the clinical and radiographic outcomes of 73 patients with low-grade degenerative spondylolisthesis with s

The Relationship Between Wild-Type Transthyretin Amyloid Load and Ligamentum Flavum Thickness in Lumbar Stenosis Patients.

BACKGROUND: One key contributor to lumbar stenosis is thickening of the ligamentum flavum (LF), a process still poorly understood. Wild-type transthyretin amyloid (ATTRwt) has been found in the LF of patients undergoing decompression surgery, suggesting that amyloid may play a role. However, it is unclear whether within patients harboring ATTRwt, the amount of amyloid is associated with LF thickness. METHODS: From an initial cohort of 324 consecutive lumbar stenosis patients whose LF specimens from decompression surgery were sent for analysis (2018-2019), 33 patients met the following criteri

The Utility of Intraoperative Lateral Spread Recording in Microvascular Decompression for Hemifacial Spasm: A Systematic Review and Meta-Analysis.

BACKGROUND: Microvascular decompression (MVD) is the surgical treatment of choice for hemifacial spasm (HFS). During MVD, monitoring of the abnormal lateral spread response (LSR), an evoked response to facial nerve stimulation, has been traditionally used to monitor adequacy of cranial nerve (CN) VII decompression. OBJECTIVE: To assess the utility of LSR monitoring in predicting spasm-free status after MVD postoperatively. METHODS: We searched PubMed, Web of Science, and Embase for relevant publications. We included studies reporting on intraoperative LSR monitoring during MVD for HFS and sp

Suction Decompression Assisted Clipping of a Large Middle Cerebral Artery Aneurysm: 2-Dimensional Operative Video.

This operative video demonstrates the important considerations and details to perform a suction decompression technique to assist in clipping of a large middle cerebral artery (MCA) aneurysm (Video 1). We present the case of a 58-year-old man with a 5-day history of dizziness. A computed tomography angiography revealed a 15-mm-diameter aneurysm in the left middle cerebral artery and a 6-mm-diameter aneurysm in the anterior communicating artery. Characterization of both lesions was obtained with a cerebral angiogram. Given the wide nature of the base of the MCA lesion, a surgical obliteration w

Outcomes of a novel minimalist approach for the treatment of cubital tunnel syndrome.

We describe a minimalist approach to perform in situ decompression of the ulnar nerve. Our technique employs a unique small skin incision strategically placed to minimize postoperative scarring over the ulnar nerve and potentially decrease the risk of iatrogenic injury to the medial antebrachial cutaneous nerve. We retrospectively report the outcome of patients who have undergone this procedure at our institution, the Michael E. DeBakey Veterans Affairs Medical Center, from January 1 2007 through November 29 2010. All individuals underwent in situ decompression via the previously described min
