Endoxifen for mammographic density reduction-results from the KARISMA endoxifen trial.

BACKGROUND: (Z)-endoxifen is the tamoxifen metabolite that possesses the highest affinity to the estrogen receptor and is evolving as an alternative to tamoxifen. Mammographic breast density (MBD) change has been shown to be a proxy for tamoxifen therapy response. The objective was to measure the effect of 2 different doses of (Z)-endoxifen on MBD, safety, and side effects in healthy women. METHODS: Healthy premenopausal women included in the national Swedish screening program in Stockholm were invited to KARISMA Endoxifen, a proof of principle, dose determining, double-blinded, randomized, p

Hydrogen-rich jelly exhibits a positive effect on heart rate variability during orthostatic loading in healthy adults: a randomized crossover trial.

Hydrogen gas may improve autonomic regulation. The effects of hydrogen-rich jelly (HRJ) on autonomic responses during postural transition have not yet been investigated. This double-blind, randomized, placebo-controlled crossover trial evaluated HRJ effects on heart rate variability (HRV) in 50 healthy adults (21 men; mean age, 44.2 ± 11.3 years). HRJ (30-40 ppm, 10 g) or placebo was orally administered 24 h, 1.5 h, and immediately before testing. HRV was recorded during an orthostatic loading protocol with four 1 min phases: seated rest, stand-up, standing, and resitting. Among the 48 partici

Recombinant human hyaluronidase PH20 facilitates rapid subcutaneous infusion: two randomized, double-blind, self-controlled phase III clinical trials.

BACKGROUND: Hyaluronidases (HAase) increase tissue permeability by depolymerizing hyaluronan in the extracellular matrix. Consequently, the subcutaneous (SC) delivery of substantial volumes of fluids or biotherapeutics is facilitated. Recombinant human hyaluronidase PH20 (rHuPH20) has been developed to overcome the allergic and immunologic risks associated with animal-derived HAase. However, standalone rHuPH20 products remain unavailable in China despite increasing clinical demand. METHODS: Two randomized, double-blind, self-controlled phase III clinical trials (phase IIIa and IIIb) were cond

The influence of bapineuzumab and semagacestat on rapid progressors: A retrospective cohort study.

BACKGROUND: A subset of Alzheimer's disease patients progresses much more rapidly than average. These rapid progressors exhibit accelerated cognitive and functional decline. Potential differences in Alzheimer's biomarkers for rapid progressors and their responses to disease-modifying treatments remain poorly understood, with previous clinical trials and studies producing limited biomarker data and inconsistent results. OBJECTIVES: Examine differences in rapid progressor versus non-rapid progressor outcomes in two AD treatment trials (bapineuzumab and semagacestat) and investigate cognitive an

The goal attainment scale in primary mitochondrial disease: Construct validity and lessons learned from a randomized controlled trial.

BACKGROUND: Primary mitochondrial diseases (PMD) are rare heterogeneous disorders caused by defective oxidative phosphorylation, with symptoms varying widely between individuals with PMD. Despite extensive research, no consensus exists on outcome measures that adequately reflect function, activities, and participation for adults with mitochondrial diseases. The Goal Attainment Scale (GAS) offers a personalized, patient-centered way to capture these outcomes. However, its validity and standardized use in trials remain unclear. This study assessed GAS construct validity in a PMD trial, including

Gut microbiota and metabolic alterations in participants with flatulence identify Faecalibacterium prausnitzii as a key microbial target for clinical intervention.

Flatulence is closely associated with gut dysbiosis, yet the characteristic microbial signatures, metabolic alterations, and actionable intervention targets remain unclear. This limited mechanistic understanding has hindered the development of precise microbiota-based strategies for managing flatulence. Here, we found that participants with flatulence exhibited marked shifts in gut microbial functions and fecal metabolic profiles compared with healthy controls, characterized by enhanced abnormal fermentation, enrichment of oxidative stress-related functions, elevated low-grade inflammatory sig
